Generation and analysis of mice with a targeted disruption of the arylamine N-acetyltransferase type 2 gene.

Arylamine N-acetyltransferases (NATs) are polymorphic xenobiotic metabolising enzymes, linked to cancer susceptibility in a variety of tissues. In humans and in mice there are multiple NAT isoforms. To identify whether the different isoforms represent inbuilt redundancy or whether they have unique roles, we have generated mice with a null allele of Nat2 by gene targeting. This mouse line conclusively demonstrates that the different isoforms have distinct functions with no compensatory expression in the Nat2 null animals of the other isoforms. In addition, we have used the transgenic line to show the pattern of Nat2 expression during development. Although Nat2 is not essential for embryonic development, it has a widespread tissue distribution from at least embryonic day 9.5. This mouse line now paves the way for the teratological role of Nat2 to be tested.

[Rehabilitation of female urinary incontinence].

Perineal rehabilitation is an appropriate alternative to surgery in the treatment of urinary female incontinence. The most important factors influencing the success of this technique is the ability of the patient to identify correctly the muscles of the pelvic floor, to strengthen this muscles using exercises, electrical stimulation and biofeedback, to contract voluntarily the pelvic floor musculature during stress or sensation of voiding for having a preventive effect on loss of urine, and also to change, if necessary, the micturitional and drinking customs. Some conditions are required to complete a good result: strong motivation of the woman, ability of the physiotherapist or the midwife, quality of care and follow-up of the physician who must clearly know the place of this conservative treatment in selected patients, particularly in moderate stress incontinence, without important prolapse, urge incontinence, pregnancy and post-partum, two conditions in which this technique must have a preventive and curative efficiency.

[Cauda equina syndromes in ankylosing spondyloarthritis, detected urologically].

The authors stress the importance of sphincter disorders especially bladder, in these patients. These disorders may compromise the vital prognosis. An accurate neuro-urological work-up is absolutely necessary. It will guide the functional rehabilitation. Which, as a rule, will improve the patients' comfort and prevent the occurrence of urinary complications, with the use, as in the case reported, of non-sterile intermittent self-catheterization.

Early vesico-ureteral reflux following conus medullaris injury: case report.

The authors describe severe vesico-ureteral reflux and simultaneous renal insufficiency which occurred after a spinal cord injury to the conus medullaris. They point out the misleading character of these injuries when there are only minimal neurological signs in the trunk and limbs. The main clinical consequence may be an isolated neuropathic bladder which, if not detected, delays treatment. Additionally, they underline the role of mixed bladder and sphincter lesions in the development of renal insufficiency; also the role of increased intravesical pressure during filling and emptying with bilateral vesico-ureteral reflux.

[Amyloidosis in adult Still's disease. Apropos of 2 cases].

Two cases of amyloidosis were observed in a personal series of 42 patients with the adult from of Still's disease. Two other cases have been previously reported in the literature. In the first case, amyloidosis occurred 18 months after the onset of the disease and involved the kidneys and gastrointestinal tract with a fatal outcome in 6 months. In the second case, renal amyloidosis was observed 4 years after the onset of Still's disease and progressed to renal failure in 7 months, necessitating chronic haemodialysis. These cases show that the adult form of Still's disease may rarely be complicated by amyloidosis, the prognosis of which is particularly poor.

[Chronic polyarthritis in cystic fibrosis].

In a 8 year-old girl, presenting with cystic fibrosis and severe respiratory failure, a chronic polyarthritis with skin rashes occurred. Rheumatoid factor was lacking in the serum. Antibiotic treatments of the pulmonary infections resulted repeatedly in an improvement of both articular and cutaneous disorders. Thus, the trigger-action of the microbic infection was considered as the inducer of a "reactive arthritis".

[Chronic atrophic polychondritis. Report of three cases, with ultrastructural study by electron microscopy].

The authors report 3 cases of chronic atrophic polychondritis with clinical signs characteristic of the disease. The 3 patients had deafness of perceptive type, very severe in one case. In one case, the relationship between inflammatory rheumatism and rheumatoid arthritis is discussed. The 3 cases include a pathological study under the electron microscope of the cartilage of the ear. Certain ultrastructural peculiarities observed (dense perichondrocyte granules) are compared with the data in the literature.
