[Astrocytoma of the cerebellum in children. Apropos of 57 cases].

57 children with a cerebellar astrocytoma have been operated on between 1964-1982. Since 1977, CT Scan has led to safer and earlier diagnosis. CT Scan has documented three macroscopical types of cerebellar astrocytomas; a cystic form with a mural nodule enhanced and the wall of the cyst non enhanced, a cystic form with a nodule and the wall of the cyst enhanced, and a massive form. 6 patients were submitted to a ventricular-atrial shunt before direct surgery. All the patients underwent posterior fossa surgery in the sitting position. Removal of the tumor was macroscopically total in 48 patients. Removal is limited to the nodule for cystic form with the wall non enhanced. Removal is extended to the wall of the cyst when it is enhanced. In 9 patients, the CSF pathway was restored at the end of the procedure by cannulation of the Aqueduct with silastic tube. 3 patients died postoperatively (5.2%), 2 others later on (4 and 8 years). 3 patients had a recurrency. In 6 patients, radiation therapy was used after surgery.

[Intracranial arteriovenous angioma in adults. A review of 100 medico-surgical cases (author's transl)].

The most frequent clinical presentations were haemorrhage (36 cases) and epilepsy (33 cases). Among the 68 subjects operated upon, 55 benefited from total removal of the angioma. The mortality rate was 10%. The long-term outcome was favourable in 75% of patients. Excision suppressed the risk of bleeding, but its efficacy on epilepsy was more limited. The clinical course in patients not operated upon was less favourable. However, only 4 of them died, and only one recurrence of bleeding was noted.

[Intra operative embolization of unresectable cerebral arterio-venous malformations. Occlusion attempt by means of a delayed polymerization mixture. Report of two cases (author's transl)].

Two cases of unresectable intra-cranial arterio-venous malformation (AVM) are reported, treated by direct intra-operative embolization. A 50% mixture of Butyl-2-Cyano-Acrylate and Mono-Iodo-Stearate of Ethyle (Duroliopaque) was used after catheterization of some of the feeding cortical arteries. No intra-operative angiography was performed. The first patient, admitted after subarachnoid hemorrhage and presenting with a mild transient right hemiparesia and aphasia, showed evidence of a left temporal AVM with a left middle cerebral artery supply and contribution from the left cerebral posterior artery. The AVM was embolized with 3 cc of the mixture. A mild aphasia occurred after surgery and completely recovered after two weeks. Roughly half of the AVM was occluded on the postoperative angiograms. The second patient suffering from a long history of seizures had a huge right frontal AVM, with a right middle cerebral artery and right anterior cerebral artery supply, and contribution from a right lenticulostriate artery, the left anterior cerebral artery and the right posterior cerebral artery. The AVM was embolized with 7 cc of the mixture. A complete palsy of the left upper limb occurred after surgery, with complete recovery after two weeks. On the post-operative angiograms, more than three fourth of the AVM was occluded.

[Lumbo-peritoneal CSF shunt (author's transl)].

In a preliminary technical report, the authors describe a procedure for lumbo-peritoneal shunt consisting of the percutaneous introduction of a sub-arachnoid proximal catheter. There would appear to be many advantages in the treatment of communicating hydrocephalus when compared with classical shunt methods taking the CSF from the ventricle.

Intracerebral myiasis in a child.

Larvae of Hypoderma bovis accidentally parisitize man. These larvae can penetrate into the brain by a path which remains unknown, causing an intracerebral haematoma and clinical signs. Computerized tomography shows the haematoma, and carotid angiography shows the absence of a vascular malformation, but these examinations do not allow etiological diagnosis. This is suggested by seroimmunological examinations and by the discovery of the larva during the operation. We report findings in a 6 1/4 year old boy.

The so-called ectopic pinealomas. A cooperative study of 15 cases.

We present the results of a cooperative study of 15 cases of suprasellar dysgerminomas (ectopic pinealomas) and a review of cases of the literature with histological confirmation. In all the cases, histological similarities to the germinomas which appear extracerebrally, particularly testicular seminomas, were observed. We were unable to find in the sellar region tumors of true pineal histology and tend therefore to confirm the germinal origin of these neoplasms. From a clinical point of view, we define a syndrome that these tumors produce, consisting of diabetes insipidus as the first sign, visual impairment and endocrine disturbances, with few or no radiological signs in plain studies.

[Extra-intracranial artérial by-pass. A clinical experience based upon 40 cases (author's transl)].

UNLABELLED: Extra-intracranial by-pass between a branch of the external carotid artery and a branch of middle cerebral artery has been used since 1972 in the treatment of cerebral ischemia. MATERIAL AND METHODS: 40 patients have been operated on (34 men and 6 women) aged from 28 to 68. Clinical data were TIA's or PRIND in 11 patients and stroke in 29 patients. The left hemisphere was involved in 25 cases and the right one in 15 cases. No patient was operated on in emergency; operation was performed after 2 or 3 weeks in most cases. The branch of external carotid artery was the superficial temporal artery in 36 patients and the occipital artery in 4 patients. As for the cerebral artery, were used the anterior temporal artery (9), the middle temporal artery (2), the posterior temporal artery (15), the angular artery (6), a frontal artery (15) and a parietal artery (1). In 8 patients, two anastomosis were performed using both branches of STA. Patency of the by-pass was confirmed in most cases by means of angiography, performed early after operation, and if possible one year later.

[Diastematomyelia. About a series of 6 cases (author's transl)].

The authors report 6 cases of diastematomyelia in children. They stress the rarety of this spinal cord diformity whose pathogenesis is obviously of dysraphic origin. From the clinical point of view, "orthopedic syndrom" is the most common complaint (cypho-scoliosis and - or - malformation of the feet) in a child with dorsal or lumbar cutaneous dysplasia. Neurological disorders or sphincterian disturbance are less frequent. Radiologic examination shows the high frequency of spina occulta. The diagnosis is assessed when bony spur is demonstrated on films. Vertebral body diformities are not constant and often far seated from spur. Air myelography demonstrates site and length of spinal cord cleft and often the low position of conus medullaris. Surgery allows the liberation of bifid cord by resection of spur. It should be suitable to cut filum terminale when it is accessible by laminectomy. Exposure of results stress the preventive effect of surgery for diastematomyelia.

Sarcoidosis of the pineal gland. Case report.

The authors report a case of sarcoidosis of the pineal gland with no other organ manifestation of the disease. The tumor was successfully removed by a direct surgical approach.

[Subdural hematoma and anticoagulant therapy].

The authors report a series of 22 cases of subdural hematomas in patients submitted to anticoagulant therapy. A review of the literature finds 150 cases. Subdural hematomas occurs in about one third of the patients presenting hemorrhage of central nervous system related to anticoagulant therapy. Among subdural hematomas of any cause, the possible role of a previous anticoagulant therapy is stressed in 4,8 to 14% of cases. In most cases, long term anticoagulants were indicated for arterial or heart (ischemic) lesions (16/22 cases). In 6 cases, anticoagulants were indicated for prevention or treatment of pulmonary embolies. In 18 cases, anticoagulant drug is from the group of dicoumarol or phenylindanedione. In 3 cases, the only anticoagulant given to patient was heparin. Pathogenic study suggests that hypocoagulability might not always be the only factor of bleeding: high blood pressure, other drugs and head trauma (10 cases) are often associated. Clinical features, in our series, are similar to those encountered in cases of subdural hematoma of any cause. According to the existence of a cranial injury and to the chronology of anticoagulant therapy, the authors divide their 22 patients into 3 groups. The use of protamin sulfate or human plasma fraction PPSB provides in few minutes a normal coagulability. Neurosurgical treatment in all our cases evacuated in 11 patients a chronic subdural hematoma, in 2 cases an acute, and in 9 cases a subacute hematoma. Results were fair in 19 patients with no sequelae. 3 patients died during the immediate post-operative period. The authors conclude by stressing several preventive measures.

[Chronic pulmonary heart disease during ventriculo-atrial shunts in children].

This grave complication is a major cause of mortality in ventriculo-atrial shunts in children with hydrocephalus. It occurs at a much higher rate than with long-term indwelling intracavitary pacemakers, which suggests that the shunt procedure is responsible for either chronic infection or the introduction of cerebral thromboplastin.
