[Hormone receptors in gynaecology. Towards practical applications? (author's transl)].

A general review of hormone receptors in gynaecology describes the receptors of the membrane for polypeptide hormones and the receptors for steroids. The hormone stays intact in the case of the membrane receptor and activates by contact a second intra-cellular messenger which then induces protein biosynthesis. The constant of dissociation and the number of binding-sites in the human corpus luteum are established for HCG-LH during the cycle, during pregnancy or at the menopause. The working out of these receptors for levels of polypeptides by radio bio-assay gives biological specificity and radio-immunological exactness. Steroid receptors consist of a cytosol receptor which is activated and transferred to the nucleus to induce protein synthesis. They are well recognised for oestradiol and for progesterone in the endometrium and in pathological breast tissue. Variations in the cytosol receptors and the nucleus receptors are known for different phases of the menstrual cycle. Indications for hormone treatment in cancers of the endometrium and of the breast in human pathological conditions can be improved rapidly by recognition of these receptors.

Non-heart-beating donor, renewal of an old procedure: a Belgian experience.

The shortage of donated organs has become a problem in transplantation throughout the world. Transplant teams are looking for other ways to increase and improve the donor pool. Non-heart-beating donation may be a source to increase the number of donors, even if some technical, logistical, and emotional problems are encountered. The results obtained by our team should stimulate other centers to implement this kind of donation in their hospitals. We describe our experience in the policy of non-heart-beating donation and encourage transplant centers to develop such a program.

Orthotopic heart transplantation eleven years after left pneumonectomy.

Orthotopic heart transplantation was performed in a 21-year-old medical student 11 years after left pneumonectomy for a rhabdomyosarcoma. The cardiomyopathy was the result of the administration of doxorubicin (Adriamycin). The surgical procedure was largely facilitated as a result of an in-hospital donor and the absence of major adhesions. The early postoperative course was mainly uneventful. The patient is doing well 9 months after operation, without any episode of rejection or infection.

[Pregnancy and delivery following insemination with frozen donor sperm].

A total of 620 pregnancies were followed and only 25 were lost to follow-up (3.8%). The mean age of the women was 30 years. 42% of them had had ovulation induction treatment. There were 538 primiparous women (86.7%). A control series of 130 primiparous women from the same age group was constituted at random. The abortion rate was 15%. 93% of pregnancies did not pose any problems in the 2nd and 3rd trimesters (83% in the control series). 97% of the women delivered at term (the same as in the control series). The mean birth weight was 3,260 g and the sex ratio was 0.97%. There was a high malformation rate of 1.48% and peri-natal mortality was 1.1%. The caesarian rate in the primiparous women was 31.9% compared with 19.2% in the control series and it was 35.5% in the multiparous women. In the Tours series, which is more homogeneous than our control series, the caesarian rate was essentially related to maternal age: 14.7% caesarian sections before the age of 30 and 30.5% over the age of 30. It seems therefore that AID does not constitute a risk factor for the pregnancy.
