Double-blind clinical and psychologic study of ergoloid mesylates (Hydergine) in subjects with senile mental deterioration.

A double-blind study of 24 weeks' duration was conducted to investigate the effects of ergoloid mesylates (Hydergine) on symptoms of senile mental deterioration. Fifty-eight residents of old people's homes were included in the trial. Thirty were treated with ergoloid mesylates and 28 with placebo, and the effects of treatment were determined by means of medical and psychological examinations. On the Sandoz Clinical Assessment Geriatric Scale, the group receiving ergoloid mesylates showed significant improvement in all items. The group receiving placebo showed slight deterioration. Psychological examination showed that no changes were observed for either group in quantitative psychometric test results, although qualitative aspects of performance such as attention and concentration did improve. There was a close correlation between improved cognitive function scores on the SCAG and improved evaluations of behavior during the psychological examinations. There were marked individual differences in the degrees of improvement.

[Invalidity and aphasia].

Aphasia may be conceived as a disturbance in one of the functional capacities. Most functional capacities may be 'measured' by performance scales (Katz, Barthel Index, Kenny Numerical Scale). These scales do measure more the 'amount' or 'need' of help than the disturbance itself. So, they measure in fact economic variables instead of medical ones. In 'Regina Pacis' we developed medical scales for the ADL and performance in ambulation. The question is raised whether in certain conditions, they might serve as parameters for the improvement of speech in aphasia. From this starting point these conditions are reviewed and it is thought that a common denominator for both disorders may be found: the ideamotoric disturbance, caused by the CVA. The question is raised if--in those accidents--man himself is fundamentally reduced (stricken) or only deprived from his instruments and tools.

A structural model of functional capacity in the aged.

The core problem in geriatrics is the continuous loss of adaptive capacity by bones, joints, brain, heart and lungs. Thus many old persons cannot take care of themselves any longer in matters of food, safety, body temperature and hygiene, the basic functions for human independence. Geriatricians agree about the basic functions that an elderly person should be able to perform. Only with such criteria can goals be defined more accurately and comparisons made of therapeutic efforts, leading to a higher level of efficiency in therapy, prognosis and care. A theoretical framework is proposed which may serve as a model for assessment of functional capacity in the aged patient. The model (hierarchic structure) has four components: household activities, mobility, activities of daily living, and function of the autonomic nervous system. No component covers more than five items. There is great need for studies on an international level so that assessments and treatments of elderly patients can be compared.
