[Assessing health state utility using the short form 36].

OBJECTIVE: To explore the methods of utility assessment using the short form 36. METHODS: The authors selected 70 people stratified by age, sex, and occupation in a city in Sichuan province, and investigated the preference of the population on each domain or item by paired comparison with other domains or items. Based on the preference, the weights of the domains and items were calculated. RESULTS: The weights of all the 8 domains and the items within 7 domains except the first domain "physical functions" were identified. The total score and the scores of the domains calculated by weighted adding method were significantly different from those calculated by simple adding method. However, only little variation of the scores of the domains between the two methods appeared. CONCLUSION: Utility calculated by weighted adding method may reflect people's preference to the health states more accurately. However, the value is limited in using weighted adding method for calculating the scores of domains.

[Human papillomavirus DNA test in nasal polypsis].

OBJECTIVE: To evaluate pathogenesis of nasal polypsis. METHOD: To detect HPV in 26 pathological samples of 13 cases by polymerase chain reaction(PCR). Ten cases of normal nasal mucous were used as control. RESULT: HPV-DNA was positive 12 cases(92.3%) in 13 cases of the first surgical operation by PCR. HPV-DNA was positive 5 cases (38.5%) in 13 cases of the second surgical operation by PCR. The most HPV type was HPV6. HPV-DNA was negative in 10 cases of control. CONCLUSION: The results showed that HPV plays an etiologic role in the development of nasal polypsis.

[The study of human papillomavirus(HPV) DNA expression in middle-ear carcinomas].

OBJECTIVE: To study the expressions of human papillomavirus DNA in the middle-ear carcinomas. METHOD: A PCR method using consensus primers for the detection of HPV6,11,16,18 was applied in 5 biopsies of middle-ear carcinomas. Eight biopsies of mucosa in middle-ear and mastoid were concluded as controls. RESULT: It was found that a resulting 80% (4/5) of the middle-ear carcinomas contained high-risk-type HPV DNA, while the 0%(0/8) of the controls. CONCLUSION: The high prevalence of high-risk-type HPV in carcinomas of the middle ear suggests that viral infections may be an important etiologic component in the carcinogenic process.

[High concentration ethanol continuous fermentation using yeast flocs].

Continuous ethanol fermentation using yeast flocs was carried out in 4 air-lift suspended-bed bioreactors operated in series. Drafted by CO2, with complete recycle of ethanol distilled effluent broth and at the dilution rate of 0.2/h, the average ethanol concentration of the fermentation broth was 96.6 g/L, while the average concentration of residual total sugar was 4.1 g/L and residual reducing sugar was 1.2 g/L.

[Construction of plasmid gene bank of V. cholerae O139 and detection of O-antigen genes].

Because O-antigen biosynthesis genes are a tandem gene cluster. Gnomic fragments of 4-20 kilobases (kb) were obtained by digesting genomic DNA of V. cholerae O139 with restriction endonuclease EcoRI, then plasmid gene bank was constructed. Recombinant colony, E. coli DH5 alpha (pMG320), expressing O-antigen of V. cholerae O139 was detected from the bank by immunological agglutinative reaction. The futher analysis showed O-antigen expressed by recombinant colony had both immunogenicity and reactogenicity, and the size of O-antigen biosynthesis genes was about 4.6 kb.

[The measurement of multifocal electroretinography].

PURPOSE: Discussing the testing procedure of multifocal electroretinography(ERG). METHODS: The testing procedure of 131 multifocal ERG records was reviewed. RESULTS: Some variable factors existed in the measurement of multifocal ERG. These factors included the subject's co-operation, the mounting of electrode, the fixation of tested eye and adjustment of focus. The good records of multifocal ERG depend upon the skill of the operator. CONCLUSION: The testing procedure of multifocal ERG must be standardized so as to ensure the reliability and repeatability of the results.

[Detection of hepatitis C virus from hepatocellular carcinoma and its relationship to hepatocarcinogenesis].

OBJECTIVE: To get an insight of the relationship between hepatitis C Virus (HCV) infection and hepatocellular carcinoma (HCC). METHODS: A molecular pathology research was performed. Liver specimens from 19 HCC cases with HCV infection only were detected for HCV antigen (HCAg) and HCV RNA with monoclonal antibody (McAb) against HCV NS3 antigen by means of immuno-histochemistry staining and in situ hybridization using Dig-labeled full-length HCV cDNA Probe. RESULTS: In 17 of 19 (89.5%) cases, positive expression of the antigen was observed; 11 of 19 (57.9%) cases were positive for HCV RNA. The viral antigen was expressed in cytoplasm and nucleus of para-cancerous hepatocytes and in cancer cells too, while the distribution of HCV RNA was only seen in cytoplasm of hepatocyte. CONCLUSION: These results strongly support the relationship between HCV infection and the occurrence of HCC. The nucleic distribution of HCAg suggests a possible interaction between HCAg and the host genome, which may lead to hepatocarcinogenosis.

[Factors that influence the quality of life of Sichuan population].

OBJECTIVE: To investigate the risk factors influencing quality of life with the aim of providing evidence for valid health promotion measures. METHODS: The authors used the short form 36 to assess the quality of life of 2249 residents in Sichuan Province. The data of potential risk factors including social economic status, culture, chronic condition and personal characteristics were collected. The Student t test, variance analysis, and stepwise multivariate regression methods were used to test the impacts of different risk factors on quality of life. RESULTS: Chronic condition, age, sex, educational level, marital status, occupation, personal income, number of family members, number of generation in a family, inhabitant places (urban or rural), and way of payment of medical expenditure were correlated with all or most of the domains of the short form 36 in t-test or one-way variance analysis. When the effect of interaction between factors were excluded by use of multivariate regression, the chronic condition, personal income, inhabitant places, age, and educational level remained to be significant risk factors. Marital status also had impacts on a few domains. CONCLUSION: The key steps of promoting the quality of life should include the prevention and control of chronic conditions, the development of economy, and the improvement of people's income and educational level. It is necessary to provide norms of the short form 36 stratified by inhabitant's place and age. Further research would be needed to provide evidence for the necessity of norms stratified by sex.

[Assessing the quality of life of people with chronic diseases using SF-36].

OBJECTIVE: To analyze the influence of chronic diseases on the quality of life by using short form of 36 (SF-36). METHODS: Four hundred and thirty-eight people with chronic diseases were surveyed in a township and a city in Sichuan Province. The diseases were classified according to ICD-9 and the quality of life of people with chronic diseases of different systems was analyzed. RESULTS: The quality of life of people with chronic diseases of different systems was lower than that of general people aged 40 or above. The quality of life varied among people with chronic diseases of different systems in four fields, namely PF, SF, VT and MH. The main diseases that influenced the quality of life were mental disorders, infectious and parasitic diseases, the diseases of blood and blood-forming organs. CONCLUSION: The chronic diseases have decreased the quality of life. The diseases of different systems have had different influences on the quality of life. SF-36 is suitable for assessing the quality of life of people with chronic diseases. Quality of life should be taken as an important health index in chronic disease control and prevention.

[Scaling the SF-36 in a Chinese population].

OBJECTIVE: To facilitate the cross-cultural comparison, we employed the principles of fuzzy mathematics in scaling the SF-36 in a Chinese population. METHODS: We stratified an urban population according to the age, sex, and occupation, and selected 650 people to participate in the study. A total of 646 valid questionnaires were returned. In the 8 domains of the SF-36, the interval estimation of the scores of different alternative answers of 6 domains which have more than 2 alternative answers were conducted. RESULTS: The fuzzy degrees of most interval estimations were less than 0.5, except 4 estimations in the domain "physical functions". The ordering of the scores of 8 domains was more similar to the original assumption by our scaling criteria than by the US scaling criteria. CONCLUSION: The scaling criteria set in this study is more suitable for use in Chinese population and we suggest that the alternative answers of the domain "physical functions" be simplified to "yes" and "no".

[Feasibility of using short form 36 in Chinese population].

OBJECTIVE: To explore the feasibility of using the SF-36 in Chinese population. METHODS: We tested the reliability and validity of the SF-36 in 1603 rural residents, 646 urban residents, 70 high school girls, 122 patients with osteoporosis, and 150 stroke patients. RESULTS: The reliability and validity of the SF-36 in general were acceptable, however, the reliability and validity varied among different domains and in different populations. The reliability and validity of the domains "limitations in physical activities", "limitations in usual role activities because of physical health problems", "bodily pain", and "general health perception" were better than those of the domains "vitality", "limitations in social activities", "limitations in usual role activities because of emotional problems", and "mental health". CONCLUSION: The results indicate that we can use the SF-36 in Chinese population, but we have to pay attentions to the variance of the reliability and validity in different populations, especially for the interpretation of the results of the domains related to mental and social functions.

[The norms of SF-36 scale scores in urban and rural residents of Sichuan province].

OBJECTIVE: To develop the norms of SF-36 in Sichuan, China. METHODS: A total of 1603 rural residents and 646 urban residents were surveyed. Both the US scaling criteria and China scaling criteria were used to score the 8 domains of the SF-36 and compared with the norms of Hong Kong and US populations. RESULTS: The scores of the 8 domains of the SF-36 using China scaling criteria were highly correlated with those using the US scaling criteria, with correlation coefficients greater than 0.98, however, the mean scores using the two scaling criteria were different significantly. The norms of the Sichuan population were more similar to the Hong Kong norms than the US norms. The scores of the SF-36 were different between different age and sex groups and between urban and rural populations. CONCLUSION: We have developed the norms of SF-36 by age/sex groups in urban and rural population of Sichuan, China.

[A kinetic study on the relationship between of IL-5, IL-10 and eosinophil apoptosis in asthmatic airway inflammation].

OBJECTIVE: To observe time-course profile of eosinophil apoptosis, in murine asthma models in airway explicate its implication for asthmatic inflammation remission, and investigate the role of IL-5 and IL-10 in modulating cell apoptosis. METHODS: An animal model of asthma was established by OVA sensitizing-challenging BALB/c mice. Histologic study of lung tissue was made with the use of electronmicroscope. At seven points (0 h, 8 h, 24 h, 48 h, 96 h, Day 7 and Day 14) in the time course after challenge, bronchoalveolar lavage (BAL) was performed obtain BAL cells and recover fluid. Levels of IL-5 and IL-10 were detected by ELISA. Cell apoptosis was assayed by PI stain on a flow cytometer. RESULTS: After challenge, apoptosis of eosinophils, lymphocytes, endothelial cells could be seen under the electronmicroscope. IL-5 in BALF elevated significantly whereas no significant difference was found between the levels of IL-10 in BALF before and after challenge. Eosinophil apoptosis was identified and a two-phase elevation of apoptosis percentages was observed, although no significant difference was found between average apoptosis in asthmatic group and that in control. Data analyses revealed a negative correlation between apoptosis percentages and the levels of IL-5, and a closer correlation between the percentages and IL-5/IL-10 ratio. CONCLUSION: IL-5 and IL-10 could up- and down-regulate asthmatic airway inflammation, respectively. EOS underwent apoptosis in situ and this was modulated by IL-5 and IL-10 interaction.

[An association study of histocompatibility leukocyte antigen-class II with Meniere's disease].

OBJECTIVE: To investigate the relation between histocompatibility leukocyte antigen (HLA)-class II DRB1 and Meniére's disease in Chinese. METHODS: Polymerase chain reaction-sequence specific primers (PCR-SSP) technique was used. HLA-class II allele distribution were measured in 60 patients with Meniére's disease, in comparison with those in 85 normal population. RESULTS: The results showed that the frequency of DRB1*09 allele was significantly lower in Meniére's disease patients than in those of controls (relative risk = 0.17, P < 0.01). CONCLUSION: HLA-DRB1*09 of the patients with Meniére's disease was significant low. It implied that DRB1*09 may be a protective gene for Meniére's disease.

[Spontaneous otoacoustic emissions and efferent control of cochlea].

OBJECTIVE: To study the relationship between spontaneous otoacoustic emissions(SOAE) and efferent control of cochlea and their clinical significance. METHODS: SOAE, transient evoked otoacoustic emissions (TEOAE), distortion product otoacoustic emissions (DPOAE) and contralateral white noise (60 dB SPL) suppression of TEOAE and DPOAE experiments were conducted in 312 ears of 95 patients with retrocochlear impairment and/or MOCS dysfunction and 64 normal young adults. RESULTS: MOCS dysfunction was shown in 126 ears of 65 patients (130 ears) with auditory neuropathy, 2 ears of 2 patients with unilateral acoustic neuroma, 4 ears of 2 patients with hyperacusis, 14 ears of 26 patients(48 ears) with normal hearing level in unilateral or bilateral tinnitus. Stronger EOAE could be recorded in total 146 ears with MOCS dysfunction at any pure tone hearing level. SOAE could be recorded in 126 of 146 ears (86.3%) with MOCS dysfunction and 44 of 128 ears (34.3%) with normal hearing. SOAE of ears with MOCS dysfunction was mainly at frequencies from 0.693 to 3.055 kHz and SOAE of normal ears was at frequencies from 1.135 to 2.746 kHz. Average value of maximum amplitude of SOAE spectrum (-3.4 +/- 6.4) dB SPL was significantly greater than that in normal ears (-6.8 +/- 7.8) dB SPL (P < 0.01). The major frequency range of SOAE (0.693-3.055 kHz) in MOCS dysfunction ears was essentially consistent with that of efferent suppression in normal ears (0.7-3 kHz). CONCLUSION: The modulation of the cochlear active mechanisms by MOCS mainly presents in the low- and mid-frequency regions, these frequencies correspond to the frequency range of SOAE. Stronger SOAE indicates pathophysiological significance. There is a clear clinical relationship between SOAE and the efferent modulation of the cochlea.

Combined intraoperative iliac artery stents and femoro-popliteal bypass for multilevel atherosclerotic occlusive disease.

OBJECTIVE: To review our preliminary experience and evaluate our early results of a combined intraoperative iliac angioplasty and stenting with infrainguinal revascularization in multilevel atherosclerotic occlusive disease. METHODS: From July 1999 to April 2000, intraoperative iliac angioplasty and stenting combined with simultaneous femoro-popliteal bypass were performed on 12 lower extremities of 10 patients suffering from multilevel atherosclerotic occlusive disease. There were 8 men and 2 women, average 72 years. The indications for procedures included disabling claudication in 3 and rest pain in 7 patients. RESULTS: Eleven iliac angioplasty and stent procedures combined with simultaneous 9 femoro-popliteal by-pass and 3 femoro-femoral-popliteal bypass were performed in 12 limbs of 10 patients. Angioplasty and stent placement was technically successful in all patients. One contralateral femoral-popliteal bypass was failure after femoro-femoral-popliteal bypass. There were no additional instances of procedural or postoperative morbidity or mortality. Mean follow-up was 5 months (range 1 approximately 10 months). During the follow-up period, one femoro-infrapopliteal graft became occluded after 7 months and above-knee amputation was required. The cumulative primary patency rate of stented iliac arteries, femoro-femoral bypass grafts and femoro-popliteal bypass grafts were 100% (11/11), 100% (3/3) and 90.9% (10/11) in the follow-up period, respectively. The amputation rate was 8.3% (1/12). CONCLUSIONS: Intraoperative iliac artery PTA and stent placement can be safely and effectively performed simultaneously with infrainguinal revascularization for multilevel atherosclerotic occlusive disease by skilled vascular surgeon, using a portable C arm fluoroscopy inthe operating room. Furthermore, iliac artery PTA and stenting was valuable adjunct to distal bypass either to improve inflow and outflow, or to reduce the extent of traditional surgical intervention, and also, any angioplasty and stenting-related complications can be immediately corrected as well.

[Studies on correlation between protonation and tautomerism and luminescence characteristics of quinolone].

The fluorescence characteristics of 8 kinds of quinolone in different pH medium were studied. Rules of wavelength and intensity of the fluorescence in different pH medium were discovered. The correlation between the protonation, tautomerism and luminescence characteristics of quinolone are studied. It is found that in pH 6-8 or 2-4 there are two luminescence species. By this, the new view is represented that the change of luminescence characteristics is arised from the protonation and tautomerism of quinolones in different pH medium and the material point of luminescence developed in equilibrium system of tautomerism. Additionally, the correlation between the protonation, tautomerism and luminescence is further discussed.

[Determination of deoxyribonucleic acid with rosaniline by a resonance light scattering method].

The resonance light scattering (RLS) spectra of Rosaniline with DNA have been studied. The RLS of Rosaniline is greatly enhanced by DNA in pH range of 10.5-10.8. There is a resonance light scattering peak ofat 485 nm, and the enhanced intensity of RLS at this wavelength is proportional to the concentration of DNA. The linear range of the calibration graph is 0-1.00 mg.L-1 with the correlation coefficient of 0.9980. The detection limit is 14.2 ng.mL-1. The relative standard deviation(n = 10) was within 2.1%. This method is simple, rapid and has been applied to the determination of DNA in mixed samples with satisfactory results.
