"Summary measure" statistic for assessing the outcome of treatment trials in relapsing-remitting multiple sclerosis.

OBJECTIVES: To review the outcome measures commonly used in phase III treatment trials of relapsing-remitting multiple sclerosis and to introduce a method of data analysis which is clinically appropriate for the often reversible disability in this type of multiple sclerosis. METHODS: The conventional end point measures for disability change are inadequate and potentially misleading. Those using the disability difference between study entry and completion do not take into account serial data or disease fluctuations. Rigid definitions of "disease progression" based on two measurements of change in disability several months apart, do not assess worsening after the defined "end point", nor the significant proportion of erroneous "treatment failures" which result from subsequent recovery from relapses that outlast the end point. Assessing attacks merely by counting their frequency ignores the variation in magnitude and duration. These problems can be largely circumvented by integrating the area under a disability-time curve (AUC), a technique which utilises all serial measurements at scheduled visits and during relapses to summarise the total neurological dysfunction experienced by an individual patient on any particular clinical scale during a study period. CONCLUSIONS: The "summary measure" statistic AUC incorporates both transient and progressive disability into an overall estimate of the dysfunction that was experienced by a patient during a period of time. It is statistically more powerful and clinically more meaningful than conventional methods of assessing disability changes, particularly for trials which are too short to expect to disclose major treatment effects on irreversible disability in patients with a fluctuating disease.

Ovarian development in mice bearing homozygous or heterozygous null mutations in zona pellucida glycoprotein gene mZP3.

The plasma membrane of all mammalian eggs is surrounded by a thick extracellular coat, the zona pellucida (ZP), whose paramount function is to regulate species-specific fertilization. The mouse egg ZP is composed of only three glycoproteins, mZP1-3, that are synthesized and secreted exclusively by oocytes during their 2-3 week growth phase. Disruption of the mZP3 gene by targeted mutagenesis in embryonic stem (ES) cells yields mice heterozygous (mZP3 +/-) or homozygous (mZP3-/-) for the null mutation. As expected, male mice bearing the null mutation are indistinguishable from wild-type males with respect to viability and fertility. Female mZP3 +/- mice are as fertile as wild-type animals, but their eggs have a thin ZP (approximately 2.7 microns thick) as compared to the ZP (approximately 6.2 microns thick) of eggs from wild-type animals. On the other hand, female mZP3-/- mice are infertile and their eggs lack a ZP. The infertility apparently is due to the lack of a sufficient number of eggs in oviducts of superovulated mZP3-/- females. Light micrographs reveal that development of ovarian follicles is often retarded in mZP3-/- mice as compared to wild-type animals. This is manifested as reduced ovarian weights, reduced numbers of Graafian follicles, and reduced numbers of fully-grown oocytes in mZP3-/- females. It seems likely that the pleiotropic effects of the homozygous null mutation on ovarian development may be due, at least in part, to disruption of intercellular communication between growing oocytes and their surrounding follicle cells.

Brain-derived HIV-1 tat sequences from AIDS patients with dementia show increased molecular heterogeneity.

HIV-1 infection results in a dementing illness affecting 20% of patients with AIDS. Several HIV-1 genes have been implicated in the pathogenesis of HIV-induced neurological disease. To search for distinct HIV-1 sequences associated with the development of dementia, brain-derived tat, env, and pol sequences were examined from AIDS patients defined pre-mortem as demented (HIV-D)[n=5] or non-demented (HIV-ND)[n=5]. Estimations of evolutionary distances and frequency of non-synonymous mutation rates revealed significant differences between brain-derived tat, env, and pol-encoded reverse transcriptase sequences. However, established zidovudine-associated resistance mutations in reverse transcriptase sequences were identified in only one HIV-D and one HIV-ND patient despite prolonged treatment of some patients. Non-synonymous/synonymous substitution rates among the tat sequences derived from patients with HIV-D were significantly higher compared to the HIV-ND group (P < 0.001). The ratios of transversions to transitions were also significantly higher among the HIV-D tat sequences (P< 0.01). Phylogenetic analyses showed clustering of sequences from each clinical group among the brain-derived tat and env sequences. These studies indicated that differing selective forces act on individual HIV-1 genes in the brain which may influence the development of dementia.

Cloning of 1-aminocyclopropane-1-carboxylate (ACC) synthetase cDNA and the inhibition of fruit ripening by its antisense RNA in transgenic tomato plants.

A 1.7 kb fragment of ACC synthetase cDNA, one member of the ACC synthetase multigene family, was amplified from total tomato cDNA through a polymerase chain reaction (PCR) and cloned in E. coli. Restriction mapping and sequencing analysis confirmed its fidelity and correctness. The cloned ACC synthetase gene was then inserted into a binary vector pBin437, in an inverted orientation between the CaMV 35S promoter with duplicated enhancers and the Nos 3' transcriptional termination sequence, to construct an expression vector pBACC. Transgenic tomato plants were obtained by A. tumefaciens-mediated transformation of cotyledons. PCR detection and Southern blot analysis confirmed the integration of the antisense ACC synthetase gene in the transformed tomato genome. The results from RT-PCR of RNAs isolated from transgenic tomato leaves confirmed that antisense ACC synthetase RNA was synthesized in these transgenic plants. The amount of ethylene released from transgenic tomato fruits was reduced significantly to about 30% of that released by non-transformed controls. The inhibition effect of antisense RNA on fruit ripening was observed in transgenic plants and their progeny (T1). The shelf life of transgenic tomato fruits was at least 60 days at room temperature without significant change in hardness and color. After 15-20 days of treatment of the transgenic fruits with ethylene, most of them reached the ripe stage. The antisense ACC synthetase gene was inherited as a single gene in the progenies of transgenic tomatoes determined by T1 progeny analysis, consistent with the results of Southern blot analysis. Transgenic homozygotes expressing antisense ACC synthetase RNA showed prolonged shelf life in the T2 progeny.

Application of acupuncture and moxibustion for keeping shape.

On the basis of TCM diferentiation, the authors have treated 359 adult female cases of non-obesity with undesirable body shape by combined application of body acupuncture and moxibustion and auricular acupuncture, and achieved quite good remoulding and orthopedic results, suggesting that acupuncture and moxibustion can very effectively regulate the somatotypic indexes of body weight, circumference of the chest, loin, hip and thigh, the ratio of the loin to hip, sebum thickness, obesity degree, body mass index and body fat percentage of the cases in the overweight group and orthopedic groups 1, 2. It is therefore concluded that acupuncture and moxibustion is a good therapy for obtaining a graceful body shape.

[A method of unilateral operation for early cleft palate repair].

OBJECTIVE: This paper presents a new method for cleft palate repair. METHOD: Six changes have been made to the conventional procedures. After all of these six changes have been carried out on one side of the palate, the operated side becomes completely relaxed. It can be moved both posteriorly and medially to lengthen the side of the palate and to contact with the cleft margin of the non-operated side without tension. Obviously, there is no need to perform the same surgical maneuvers on the other side again. The cleft is then closed by layers. RESULT: Totally 151 cases of unilateral and bilateral cleft palate have been repaired with this method. The age of these patients ranged from 4 months to 5 years. Postoperatively, there was neither one death nor one dehiscence. All of the baby patients who received treatment before they began to speak had good quality of speech or near normal speech function. CONCLUSION: In the unilateral operation, the surgical trauma, blood loss and time required for the operation are all less than that of conventional procedures which operate on both sides. It is a safer method for early cleft palate repair.

[Investigation of hepatitis B virus infection in blood donors].

To investigate the status of HBV-M and HBV DNA in blood donors, 583 serum samples were detected by ELISA technique and polymerase chain reaction. The results were that hepatitis B virus marker(HBV-M) positive rate was 55.9%(326/583), HBV DNA 9.78%(57/583). The data indicate that there is high incidence of HBV infection in blood donors. The donors should be strictly tested for HBV in the future.

[Polymerase chain reaction applied for detection of Toxoplasma gondii in lymph nodes].

In order to investigate the morbidity of toxoplasmic lymphadenitis, polymerase chain reaction (PCR) was used to detect DNA of Toxoplasma gondii within lymph nodes in 3 groups of 120 patients with different diseases. After extracting the DNA of each sample, PCR was employed to amplify toxoplasma DNA. The results showed that the amplification product of 210 bp was confirmed in 7 patients: 3 cases of Hodgkin's disease (HD), 2 cases of non-Hodgkin's lymphoma (NHL) and 2 cases of chronic lymphadenitis (CL). Each PCR product was then subjected to Southern blot hybridization. Besides the 7 cases proved by PCR, 1 case of CL was found positive. The positive percentages of HD, NHL and CL were 9.38% (3/32), 4.88% (2/41), and 6.38% (3/47), respectively. The total positive rate was 6.67% (8/120).

[Effect of adrenomedullin on bronchoconstriction responses induced by histamine].

OBJECTIVE: To observe the effect of synthetic human adrenomedullin (AM) on anesthetized guinea pigs bronchoconstriction induced by histamine. METHOD: The guinea pigs were divided into saline group; isoproterenol groups at the dosages of 0.03 microgram/kg, 0.3 microgram/kg, 3 micrograms/kg and 30 micrograms/kg; adrenomedullin groups at the dosages of 0.02 microgram/kg, 0.2 microgram/kg, 2 micrograms/kg and 20 micrograms/kg. There were seven guinea pigs a group. Guinea pigs were anesthetized by sodium pentobarbital, normal saline group, isoproterenol and adrenomedullin at different dose i.v., while histamine (10(-4) mol/L) were vasolated and delivered into the airway. Body plethysmogroph was used to measure airway resistance and lung compliance. RESULT: The increment of airway resistance and decrement of lung compliance induced by histamine were inhibited significantly by isoproterenol at the dosages of 0.3 microgram/kg, 3 micrograms/kg and 30 micrograms/kg (P < 0.05). The increase of airway resistance was inhibited significantly by adrenomedullin at the dosages of 2 micrograms/kg and 20 micrograms/kg (P < 0.05). The decrease of lung compliance induced by histamine was inhibited significantly by adrenomedullin at 0.2 microgram/kg, 2 micrograms/kg and 20 micrograms/kg (P < 0.05). CONCLUSION: Adrenomedullin inhibited the bronchoconstriction induced by histamine significantly.

[The tuberculin test in intrathoracic sarcoidosis].

OBJECTIVE: To highlight the diagnosis and treatment of intrathoracic sarcoidosis. METHOD: The clinical and laboratory data of 35 cases of intrathoracic sarcoidosis in Tianjin thoracic hospital from 1991 to 1996 were reviewed. Tuberculin tests for 35 cases of sarcoidosis were carried out and the results were evaluated. Simultaneously, the results have been compared with that of literature published. RESULT: Of 35 cases of intrathoracic sarcoidosis 17 cases were tuberculin positive, 4 were strongly positive, positive tuberculin reactivity (60%) was significantly higher than that published. CONCLUSION: The results showed a considerably higher tuberculin positive rate, which may be due to the difference of positive tuberculin reactivity in different country or regions, age of patients and expression of gamma delta T cell receptor. The authors suggested that "sarcoidosis should be diagnosed with negative tuberculis reastivity" has to be reconsidered.

[Determination of formic acid in urine by high performance capillary zone electrophoresis].

The amounts of formic acid in human urine has a direct bearing on the people's health. In this paper, high performance capillary zone electrophoresis was applied to separate formic acid of urine with a buffer system of 5 mmol/L phthalate-0.5 mmol/L hexadecyltrimethyl ammonium bromide (CTAB) and pH 6. Separations were performed in a 50 cm x 50 microns i.d. fused silica capillary (effective length 48.5 cm) at 25 degrees C. A negative potential of 30 kV was used for each experiment. Sample was introduced into the capillary by pressure at kPa for 10 s. Indirect UV detection was operated at 210 nm and reference wavelength at 380 nm for all experiments. Capillary was rinsed for 5 min with 0.1 mol/L sodium hydroxide and buffer solution before each run. Urine was injected directly after filtered through 0.45 micron membrane. The results are satisfactory.

[Determination of hyperin in Sanguisorba Officinalis L. by high performance liquid chromatography].

An HPLC method for the determination of hyperin in Sanguisorba officinalis L. was investigated. 0.5 g of dried powdered crude drug was weighed accurately in a 50 mL volumetric flask added with 40 mL methanol, supersonicated for 30 min and then made up to volume with methanol. The sample solution was separated on a Shim-pack ODS column(6.0 mm i.d. x 150 mm, 5 microns) with methanol-0.025 mol/L phosphoric acid (adjusted to pH 3.0 by using triethylamine)(50:50) as the mobile phase and detected at UV-370 nm. Calibration curve of hyperin was obtained for the concentration range of 0.046-0.2 microgram. The corresponding regression equation was Y = 154 x 10(4) X-0.28 x 10(4), r = 0.9998. The recovery was 98.7% and relative standard deviation was 1.9%. The results showed that this method is simple, specific and accurate.

[Changes of bone cytokines and compensating regulation in tail suspended rats].

The aim of the study was to determine the relationship between the changes in bone metabolism and insulin-like growth factor--I(IGF-I) as well as epidermal growth factor (EGF). 10 Male S.D. rats were tail-suspended for 28 d with another 10 free active rats as control. The results showed that mineral content increased significantly, osteocalcin and albumin content had no significant change, but IGF-I and EGF content increased significantly in L3 in the suspended rats. The results suggest that although there are no change in osteocalcin and albumin content, the phenomenon of bone mineral loss still exists in the suspended rats. The increase in IGF-I and EGF content may be one of the compensation responses of the bone in the suspended rats.

[Primary culture and immunocytochemical identification of spiral ganglion neurons in postnatal Wistar rats].

OBJECTIVE: To create a cytological model of spiral ganglion neurons in postnatal mammals in vitro. METHODS: The primary culture of spiral ganglion neurons (SGN) was carried out with seven-day postnatal Wistar rats. The process of cellular growth and differentiation of SGN were observed by fluorescent microscope and inverted/phase contrast microscopy. Immunocytochemical identification was performed on the cultured SGN of Wistar rats by methods of S-P and the monoclonal antibody of neurofilament protein (NFP-McAb). RESULTS: The present work showed that the dissociated SGN of Wistar rat could survive well and had a normal phenotypic differentiation in vitro. The cell amount and surviving period of SGN were able to meet the requirements of cytological experiments. The stable neuronal plasticity of SGN existed in the postnatal mammal under the present experimental conditions. CONCLUSION: This method extends the material sources of inner ear tissue culture and provides a useful approach to the neurobiologic studies on the peripheral auditory system in vitro.
