An institutional outbreak of Salmonellosis due to lactose-fermenting Salmonella newport.

An institutional outbreak of salmonellosis predominantly due to a lactose-fermenting Salmonella newport is described. Control of the outbreak was hampered by delay in the initial recognition of the aberrant strain. On primary culture, salmonellae were detected on bismuth sulfite agar only; colonies that formed on MacConkey agar and Salmonella-Shigella agar could not be differentiated from lactose-fermenting nonpathogenic organisms. The reactions in triple sugar iron were atypical for Salmonella. The lactose-fermenting property was plasmid-mediated and was readily transferable. Phage typing suggested chicken as a possible source of the strain. The need for awareness of the occurrence of such strains of Salmonella that may not be recognized by cultural procedures in common use, the necessity of the routine use of bismuth sulfite agar in procedures for isolation of salmonellae, and the use of lysine iron agar in conjunction with the triple sugar iron agar are emphasized.

Evaluation of the Phadebact Gonococcus Test for confirmation of Neisseria gonorrhoeae.

Strains of Neisseria gonorrhoeae grown on Thayer-Martin medium and media with and without serum were examined by the Phadebact Gonococcus Test. By using the direct method, in which colonies from growth media were mixed directly with the reagents on a microscope slide, Thayer-Martin medium, contrary to the manufacturer's claim, was not found to be the medium giving best results, and presence or absence of serum in the media did not affect the results. A large number of inconclusive results were obtained. A modification of the alternative procedure, which uses a heated suspension of the gonococcal cells as the antigen, is described. Of the 432 strains of N. gonorrhoeae examined by this method, 427 were accurately identified. Five strains, however, gave false-negative results. None of the 80 strains of N. meningitidis gave positive results. Nine of nineteen strains of N. lactamica, however, gave clear-cut positive results. Use of the o-nitrophenyl-beta-D-galactopyranoside test in conjunction with the Phadebact Gonococcus Test, particularly on isolates from the pharynx, is recommended.

Haemophilus aphrophilus endocarditis in pregnancy.

The clinical and microbiological features of a case of Haemophilus aphrophilus endocarditis in pregnancy are described. The complicating effect of pregnancy on treatment and the difficulties in identifying the organism in the laboratory are discussed.

Pharyngitis 1987: a survey of physicians' attitudes and practices in southern Alberta.

A questionnaire survey concerning Streptococcal pharyngitis was completed by 85 southern Alberta family physicians. The data revealed a significant trend to overtreat pharyngitis with antibiotics because of the unreliability of clinical diagnosis and the lack of diagnostic manoeuvres with suitable ability to influence management. These data and a further questionnaire survey directed to the appreciation of other bacterial pharyngitides, demonstrate the importance of laboratory reports in biasing treatment, and further suggest that significant overtreatment of non-Streptococcal pharyngitis also occurs.
