Reducing patient delay in seeking treatment for acute myocardial infarction.

Although there have been tremendous strides in therapies for acute myocardial infarction (AMI), patients still do not receive these treatments in a timely manner, or sometimes not at all. One major reason is inadvertent delays by patients and by those with whom they consult about their symptoms. One of the National Heart Attack Alert Program's (NHAAP) goals is to reduce the morbidity and mortality associated with AMI through the rapid identification and treatment of at-risk individuals. Medical-surgical nurses can play a crucial role in ensuring that patients and their family members know how to recognize the signs and symptoms of a heart attack, what steps to take to get early and appropriate evaluation and treatment, and the potentially lifesaving benefits of this information.

Coping and emotional response in cardiac surgery patients.

The purpose of this descriptive, correlational study was to examine coping and emotion in patients prior to and after cardiac surgery. The revised Ways of Coping Checklist (WCCL) and the Profile of Mood States (POMS) were completed by 120 patients before surgery and on the fifth postoperative day. Participants had significant (p < .01) differences in coping prior to and following surgery. There were significant (p < .05) postoperative decreases in the use of several coping strategies (seeking social support, blaming oneself, and wishful thinking); however, the use of other coping strategies (problem-focused coping and avoidance) remained unchanged. Emotion did not change. There were positive, significant (p < .05) relationships between several of the coping strategies and perioperative emotion. Preoperative emotion was found to be the best independent predictor of the postoperative emotion.

Coping and emotion in women undergoing cardiac surgery: a preliminary study.

There is limited knowledge about how women manage the experience of cardiac surgery. This pilot study examined how women coped with surgery and their emotions in the perioperative period. There was no change in coping or emotion from the day before surgery to the fifth postoperative day, and coping was unrelated to emotion. Preoperative and postoperative confusion and distress were positively correlated. While preliminary, these results raise implications for medical-surgical nursing practice and research.

Coping strategies of cardiac surgery patients in the perioperative period.

Clinical nurse specialists, critical care nurses, and advanced practice nurses can influence patients' coping before and after cardiac surgery. By understanding the patients' experience, clinicians can tailor interventions that maximize patient coping. This nursing research study was designed to explore patient coping before and after surgery. A repertoire of interventions based on study results is provided so critical care and advanced practice clinicians can best assist the patient.
