Lung function in Duchenne muscular dystrophy.

Over 90% of patients with Duchenne muscular dystrophy develop a scoliosis when they become wheelchair bound. The scoliosis is progressive and is associated with deteriorating lung function. The purpose of this study was firstly to assess whether a standing regimen, in patients who had gone off their feet, protected against the development of scoliosis and affected their lung function, and secondly to evaluate the effect of spinal stabilisation in patients who had developed a progressive scoliosis. The results of the first part of this study showed that a standing regimen significantly delayed the progression of scoliosis and that patients who complied with the standing regimen had a significantly better lung function, as measured by vital capacity and peak expiratory flow rate, than those patients who did not stand. Spinal stabilisation prevented deterioration in the scoliosis, whereas the deformity continued to progress relentlessly in patients who did not undergo surgery. The patients who underwent spinal stabilisation maintained a significantly better lung function and had an improved survival compared with the patients who refused surgery.

Criteria for managing audiometric data in occupational hearing conservation.

Hearing conservation programmes usually include hearing testing, although it is not always clear whether the aim of such testing is to identify individuals with a hearing disability, or those who show evidence of having been affected by noise. The requirements for hearing testing in both cases relate to three main considerations: the choice of frequencies at which hearing is assessed; the way in which this threshold data is quantified or otherwise managed; and whether this index is compared to some static limit, or to the individual's own baseline audiogram to assess hearing change. Central to the assessment of hearing for the purpose of identifying individuals at risk for noise-induced hearing loss is a measure that is both sensitive and specific to the effects of noise. A case is made for a choice of frequencies around 4000 Hz, the substitution of hearing loss configuration for the three-frequency average, and an emphasis on hearing change rather than status as a means of interpreting audiometric data for hearing conservation purposes.

Synthesis, biological activity, and tritiation of L-3, 4-dehydroproline-containing peptides.

A series of analogs of thyroliberin (TRH) ([L-delta3Pro3]-TRH, [D-delta3Pro3]-TRH, [L-3-MeHis2, L-delta3Pro3]-TRH) in which proline was replaced by L- or D-3, 4-dehydroproline was synthesized. The analogs exhibited approximately the same biological activity as the corresponding proline-containing peptides. These analogs and others in which 3, 4-dehydroproline is present at the NH2-terminal, COOH-terminal or internal positions in the peptide were successfully reduced with deuterium or tritium to provide the 3, 4-2H2-proline or 3,4-3H2-proline analogs, respectively, with near theoretical values of substitution. A novel procedure for the chemical resolution of DL-3, 4-dehydroproline is also described.
