Detergent dissection of membrane proteins of Entamoeba histolytica and its effect on lymphokine release in in vitro.

Fifty-two amoebic liver abscess cases were assessed for the release of lymphokines (LMIF) using detergent dissected membrane proteins (DDMP) of axenic Entamoeba histolytica (NIH:200) obtained with sodium deoxycholate treatment. Lymphokines release by T lymphocytes in response to both DDMP and whole amoebic lysate (WAL) was tested by leukocyte migration inhibition test on blood samples from amoebic liver abscess cases. A significant increase was noted in the release of LMIF and 100% positivity was observed with DDMP compared to whole amoebic extract with a positivity of 73%. The difference between means of the above two with regards to release of LMIF was found to be highly significant (P less than 0.005). This shows the patients had high degree of leukocyte sensitization to surface antigens of E. histolytica compared to the whole amoebic lysate. These findings suggest that the antigens shed might have important role as a potent antigen in elicitation of CMI response in amoebic liver abscess cases.

HLA-DR antigens in gastric carcinoma.

Serological HLA typing of 56 gastric carcinoma patients and 53 healthy controls was performed by the long two-stage microlymphocytotoxic test. There was a significantly increased frequency of DR3 antigen (P less than 0.01) in the patients compared to controls. Relative risk analysis revealed a risk of 3.22 for subjects with DR3 antigen for predisposition to gastric carcinoma.

Immunological studies in HBV-related chronic liver diseases.

Humoral immune response in chronic sequelae of HBV infection was assessed in the present study. Serum immunoglobulins (IgG, IgM, IgA) serum complement component C3 and C4 and circulating immune complex (CIC) were estimated in forty cases of HBsAg positive chronic active hepatitis and cirrhosis and sixty cases of age and sex matched healthy control subjects. Hypergammaglobulinemia was observed in chronic liver diseased state. All the three immunoglobulins, IgG, IgA and IgM were elevated significantly. The complement C3 and C4 were significantly decreased in patient group, while the levels of CIC were significantly increased. The increased immunoglobulin levels may be attributed to disorganised Kupffer cell system and also to B cell hyperactivity. The decreased complement levels may be attributed to decreased synthesis and/or increased consumption by increased CIC. A primary or an acquired defect in infected host to generate immune response might result in defective elimination of infected hepatocytes in chronic liver disease.

Hereditary alpha-1-antitrypsin deficiency in duodenal ulcer.

An alpha-1-antitrypsin study was carried out on 184 patients with endoscopically proven duodenal ulcer, 66 first degree relatives of the patients and in 150 healthy controls, to establish the role of genetic factors in the etiology of duodenal ulcer. Alpha-1-antitrypsin served as a genetic marker in this study; it was estimated in terms of serum trypsin inhibitor capacity by the method of Jaccobson. Deficiency of alpha-1-antitrypsin level was recorded in patients as well as their family members when compared with controls.

Distribution of haptoglobin phenotypes in oesophageal and gastric cancer.

Haptoglobin (Hp) phenotypes were studied in 72 oesophageal and 104 gastric cancer patients and compared with 100 healthy controls to see if there is any association between oesophageal and gastric cancer and haptoglobin type. There was a significantly increased frequency of Hp 2-1 (59.7%) and Hp 2-2 (91.3%) in oesophageal and gastric cancer patients. Our results suggest that genetic factors play a role in the aetiology and pathogenesis of gastrointestinal tract malignancy.

Multilayer-enzyme linked immunosorbent assay (ML-ELISA) for detection of Entamoeba histolytica trophozoite coproantigen.

Multilayer ELISA (ML-ELISA) method for Entamoeba histolytica trophozoite coproantigen was developed for diagnosis of amoebiasis and its usefulness vis-a-vis microscopic examination analysed for paired specimens from 87 patients from Lucknow and Hyderabad, India. All the stool specimens positive for trophozoites were also found to be positive in ML-ELISA. Out of a total 32 samples positive for cysts in microscopy 21 (65.62%) gave positive reactions for coproantigen. Results on 26 samples drawn from otherwise healthy subjects with negative microscopical picture, 4 were positive for coproantigen. The results of the present study indicate that ML-ELISA test is suitable for early diagnosis of acute amoebic patients for timely treatment.

Study of duodenal ulcer disease in 100 families using total serum pepsinogen as a genetic marker.

Although various markers have been used in attempts to elucidate the mode of inheritance of duodenal ulcer, they have not significantly contributed to a clear understanding of the problem. In the present study total serum pepsinogen was used as a genetic marker and its concentrations were estimated in 100 ulcer patients and their family members up to three generations. Eighty three per cent of the ulcer patients had hyperpepsinogenaemia on a familial basis, and it followed an autosomal dominant mode of inheritance. Thus a large majority of ulcer patients have associated hyperpepsinogenaemia which forms a genetic basis of their disease. The remaining 17% ulcer patients did not have associated hyperpepsinogenaemia nor was the ulcer inherited by the family. Based on these observations we wish to suggest that duodenal ulcer associated with hyperpepsinogenaemia may be considered a genetic disease. This type may be termed 'primary duodenal ulcer'. In the remaining patients without hyperpepsinogenaemia or affected relatives the ulcer may be called 'secondary duodenal ulcer'. Thus total serum pepsinogen may be considered a reliable genetic marker in helping to delineate the genetic disorder from the non-genetic, thereby improving the predictive ability in duodenal ulcer.

HLA antigen frequency in the Koya tribe of Andhra Pradesh, India.

The frequencies of HLA-A, -B, and -C antigens were studied in a tribal population of Koya from Andhra Pradesh in southern India. No other well-defined tribal population has been studied with which the present results may be compared. However, the HLA profile of Koya showed distinct differences from the general HLA distribution in India in the frequency of a large number of antigens both at the A and B loci. This study indicates the distinctiveness of this tribal population and suggests the potential importance of the study of HLA frequencies in tribal groups of India.

Fingertip and palmar patterns in duodenal ulcer.

Dermatoglyphic studies were carried out on 90 adult males suffering from duodenal ulcer. Fingertip and palmar patterns were analysed to see if there was an association between duodenal ulcer and any of the dermatoglyphic traits. The patterns which were significantly different are: (1) increased frequency of whorls; (2) reduced frequency of loops on fingertips, and (3) increased frequency of patterns in the thenar I and IV interdigital area.
