Peribulbar anaesthesia and needle length.

This retrospective audit of 87 consecutive patients undergoing routine cataract surgery compared the effect of peribulbar local anaesthesia using 16 mm and 25 mm, 25 gauge needles to administer the anaesthetic. The effect on optic nerve function was observed. There was a significant increase of complete amaurosis in the group where the 16 mm needle had been used. This may be explained by more effective anatomic placement of the 16 mm needle within the orbit, allowing access to the retrobulbar space via fascial septae. There was significantly more lid akinesia with the 16 mm needle. None of this group required an additional facial nerve block, as opposed to 14% of the 25 mm needle group. The use of a 16 mm needle is theoretically safer than a 25 mm needle to administer a peribulbar anaesthetic, in this review it was also demonstrated to be more effective.

Cataract surgery with intraocular lens implantation in Fuchs' heterochromic cyclitis.

The results of cataract extraction with posterior chamber lens implantation in twenty eyes with Fuchs' Heterochromic Cyclitis are reported. No serious operative complications were experienced. The visual outcome has been excellent in all but those eyes with coexistent glaucoma. The postoperative complications seem no more frequent than would be expected for the underlying condition.

Combined extracapsular cataract extraction and trabeculectomy using a separated corneal section.

A retrospective study of 35 cases of extracapsular cataract extraction with posterior chamber lens implant combined with trabeculectomy was undertaken. The cataract extractions were performed through a corneal section to separate the cataract wound from the trabeculectomy. Control of intraocular pressure was achieved without medication in 77% of cases. Median improvement in visual acuity was 3.5 lines at one year. This technique provided improved trabeculectomy drainage compared with previous reports for corneoscleral incisions. Improvement in visual acuity was poorer for a group of patients with severe preoperative field loss.
