Interaction of multiple risk factors in the pathogenesis of experimental reflux nephropathy in the pig.

The importance of several pathogenetic factors in the development of reflux nephropathy was evaluated in 25 piglets with complete unilateral vesicoureteral reflux and urinary tract infection. Three independent risk factors were studied: roentgenographic intrarenal reflux, P-fimbriation of the bacterial strain, absence of previous immunization. E. coli with P-fimbriae produce mannose-resistant agglutination of pig red blood cells and are more adherent to pig uroepithelial cells than E. coli without P-fimbriae. Vesicoureteral reflux was surgically induced at 2 weeks, urinary tract infection introduced at 6 weeks and the animals killed at 12 weeks of age. Independently, the 3 risk factors had a borderline or insignificant effect on renal scarring. Animals with none or only 1 risk factor, however, had significantly less scarring, fewer glomerular lesions and lower serum creatinines than those with 2 or 3 factors present. Several independent pathogenetic factors seem to have a synergistic effect on the development of reflux nephropathy.

Focal nodular hyperplasia and hepatic adenoma: comparison of angiography, CT, US, and scintigraphy.

The authors reviewed 23 cases of focal nodular hyperplasia and 13 cases of hepatic adenoma, all of which were confirmed pathologically. All solitary masses that exhibited normal or increased uptake of technetium 99m-sulfur colloid were shown to be hyperplasia; while previous criteria such as a central blood supply on angiograms or a central scar on computed tomography (CT) or ultrasound (US) scans were helpful, they were relatively infrequent. A mass that was slightly hypodense and homogeneous on a CT or US scan and highly vascular with an intense capillary stain on an angiogram was almost always hyperplasia. Acute hemorrhage within a focal hepatic tumor was common in adenoma but did not occur in hyperplasia.

Mechanism of liver glycogen repletion in vivo by nuclear magnetic resonance spectroscopy.

In order to quantitate the pathways by which liver glycogen is repleted, we administered [1-13C]glucose by gavage into awake 24-h fasted rats and examined the labeling pattern of 13C in hepatic glycogen. Two doses of [1-13C]glucose, 1 and 6 mg/g body wt, were given to examine whether differences in the plasma glucose concentration altered the metabolic pathways via which liver glycogen was replenished. After 1 and 3 h (high-dose group) and after 1 and 2 h (low-dose group), the animals were anesthetized and the liver was quickly freeze-clamped. Liver glycogen was extracted and the purified glycogen hydrolyzed to glucose with amyloglucosidase. The distribution of the 13C-label was subsequently determined by 13C-nuclear magnetic resonance spectroscopy. The percent 13C enrichment of the glucosyl units in glycogen was: 15.1 +/- 0.8%(C-1), 1.5 +/- 0.1%(C-2), 1.2 +/- 0.1%(C-3), 1.1 +/- 0.1%(C-4), 1.6 +/- 0.1%(C-5), and 2.2 +/- 0.1%(C-6) for the high-dose study (n = 4, at 3 h); 16.5 +/- 0.5%(C-1), 2.0 +/- 0.1%(C-2), 1.3 +/- 0.1%(C-3), 1.1 +/- 0.1%(C-4), 2.2 +/- 0.1%(C-5), and 2.4 +/- 0.1%(C-6) in the low-dose study (n = 4, at 2 h). The average 13C-enrichment of C-1 glucose in the portal vein was found to be 43 +/- 1 and 40 +/- 2% in the high- and low-dose groups, respectively. Therefore, the amount of glycogen that was synthesized from the direct pathway (i.e., glucose----glucose-6-phosphate----glucose-1-phosphate----UDP-glucose---- glycogen) was calculated to be 31 and 36% in the high- and low-dose groups, respectively. The 13C-enrichments of portal vein lactate and alanine were 14 and 14%, respectively, in the high-dose group and 11 and 8%, respectively, in the low-dose group. From these enrichments, the minimum contribution of these gluconeogenic precursors to glycogen repletion can be calculated to be 7 and 20% in the high- and low-dose groups, respectively. The maximum contribution of glucose recycling at the triose isomerase step to glycogen synthesis (i.e., glucose----triose-phosphates----glycogen) was estimated to be 3 and 1% in the high- and low-dose groups, respectively. In conclusion, our results demonstrate that (a) only one-third of liver glycogen repletion occurs via the direct conversion of glucose to glycogen, and that (b) only a very small amount of glycogen synthesis can be accounted for by the conversion of glucose to triose phosphates and back to glycogen; this suggests that futile cycling between fructose-6-phosphate and fructose-1,6-diphosphate under these conditions is minimal. Our results also show that (c) alanine and lactate account for a minimum of between 7 and 20% of the glycogen synthesized, and that (d) the three pathways through which the labeled flux is measured account for a total of only 50% of the total glycogen synthesized. These results suggest that either there is a sizeable amount of glycogen synthesis via pathway(s) that were not examined in the present experiment or that there is a much greater dilution of labeled alanine/lactate in the oxaloacetate pool than previously appreciated, or some combination of these two explanations.

Primary polycystic ovary syndrome in a premenarchal girl. A case report.

A 12-year-old, premenarchal girl with hirsutism and virilization of nine months' duration was found to have highly elevated serum testosterone and a radiologic evaluation suggestive of bilaterally enlarged ovaries with a solid left ovarian mass. Serum gonadotropins were normal, with an LH:FSH ratio of 2:1. Exploratory laparotomy showed bilaterally enlarged polycystic ovaries, confirmed by wedge biopsies. No ovarian tumor was found. Oral contraceptives decreased the total serum testosterone to the normal female levels within three months. This was one of the youngest reported patients with primary polycystic ovary syndrome.

The face-lift incision.

Numerous variations of the "standard" face-lift incision are used to achieve adequate exposure, skin redraping , and minimal scar deformity. Our incision technique is based on careful study of over 1,000 of our patients who underwent rhytidectomies . We believe that our technique allows the goals of rhytidectomy to be achieved with minimal morbidity.

Effects of estradiol concentration on levels of nuclear estrogen receptors in MCF-7 breast tumor cells.

We have measured the time-course of estrogen receptor levels in nuclei of the estrogen-responsive breast tumor cell line MCF-7 during 90-120 min exposure of the cells to estradiol at physiologic (10(-10)M), pharmacologic (10(-6)M), and an intermediate (10(-8)M) concentration. Cells were preincubated for one week in a serum-free defined medium resembling that of Barnes and Sato, and then incubated in estradiol-containing medium. Nuclei were isolated at various times during the incubation, and filled and unfilled nuclear estrogen receptor levels were assayed. Increasing the concentration of estradiol in the incubation medium from 10(-10)M to 10(-8)M yielded increasing levels of filled nuclear receptor at all times studied, while further increase of the estradiol concentration of 10(-6)M decreased filled receptor levels from 10(-8)M values. Unfilled receptor levels dropped rapidly to zero under 10(-6)M and 10(-8)M estradiol incubation, but remained unchanged under 10(-10)M estradiol incubation. Together these results suggest that high-concentration estradiol may lead to "down-regulation" of filled nuclear receptors, which may be a contributing factor in inhibition of tumor growth. On the other hand, the continued presence of unfilled receptors only under physiological concentrations of estradiol may suggest a role for these receptors in sustaining tumor growth.

Token reinforcement during WISC-R administration. II. Effects on mildly retarded, black students.

In the first of two experiments, mildly retarded, elementary-aged, low-social status black children were administered the WISC-R under standardized-testing conditions or conditions employing token reinforcement for correct responding. Token-reinforcement subjects scored significantly higher on both the Verbal subtest and Full-Scale IQ, but not the Performance subtest. Results replicate previous findings with elementary-aged, nonretarded, black children. The same design was utilized in a second study employing mildly retarded, junior-high age, low-social status, black students. No significant differences were found between groups, suggesting that age may affect results when using tangible rewards. Coupled with previous studies, these findings imply that nonbiased intellectual assessment appears to require an analysis of motivational factors.

Protease inhibitor antipain reduces nuclear binding of estrogen-receptor complex in MCF-7 breast tumor cells.

We have initiated studies to ascertain the effects of protease inhibitors on specific steroid hormonal-related events as they occur in cultured cells, in an attempt to identify hormone-related processes perturbed by these inhibitors. We report here that the presence of antipain during incubation of cultured MCF-7 breast tumor cells with estradiol-containing medium considerably reduces the extent of nuclear binding of the estradiol-receptor complex.

Encapsulation of drugs in intact erythrocytes: an intravenous delivery system.

A number of drugs and the plasma antiprotease alpha 1-antitrypsin has been encapsulated in intact erythrocytes after hypotonic swelling, using a technique designed to preserve the viability of the cells. By labelling the cells with fluorescein isothiocyanate it has been shown that the cells survive exceptionally well when returned to the animal's circulation. Cell survival has been demonstrated in the rat, rabbit and guinea-pig. With encapsulation of cortisol-21-phosphate and methotrexate it was found that blood levels of the drug were maintained for a longer period than when the free drug was administered. Cortisol-21-phosphate was hydrolysed enzymatically by acid phosphatase located primarily in the erythrocyte membrane. An in vitro test involving the interaction or erythrocytes with phagocytes was developed to determine the viability or erythrocytes after being subjected to the encapsulation process. Preparations which did not interact with phagocytes survived when returned to the animal's circulation. The encapsulation procedure increased the fragility of the cell membrane compared to that of normal cells as measured by the leakage of haemoglobin after thermal treatment but it was found that encapsulated cortisol-21-phosphate in cells actually stabilized the membrane. The electrical charge on the membrane of encapsulating cells was the same as that of the normal cells. The charge on reformed ghosts was lower than that of normal cells. Reformed ghosts were rapidly removed when introduced into the circulation. The encapsulation procedure and its possible applications are discussed.

Midforehead lift.

Ptosis of the brow-glabellar tissues with or without vertical and horizontal wrinkling accompanies aging in other areas of the face. The direct browlift is effective in elevating and shaping brow tissues, but leaves a scar at the brow-forehead juncture and does little for glabellar deformity. The coronal forehead lift allows the correction of brow-glabellar ptosis and wrinkling with camouflage of the scar in the scalp. Male pattern hairline, thinning hair, or a high hairline may contraindicate this approach. Our technique for treating these patients is the midforehead lift. We review the anatomical considerations, patient selection, and surgical technique, showing representative cases.

Diagnosis and management of the opacified sphenoid sinus.

The growing number of surgical procedures at the base of the skull makes a clear understanding of sphenoid disease and its anatomic relationships important for the modern otolaryngologist. We present five cases demonstrating the spectrum of sphenoid sinus disease, its systemic as well as local complications, and its surgical management. Pertinent anatomic and embryologic relations are reviewed and their diagnostic evaluation is discussed. Pitfalls in the diagnosis and management of these diseases are reviewed noting possible complications and significant danger points. The necessity for cooperation among the otolaryngologist, neurosurgeon, and ophthalmologist in the management of these entities is also demonstrated.

Porcine cardiac valvular endothelial cells in culture. A relative deficiency of fibronectin synthesis in vitro.

We established culture lines of porcine endothelial cells derived from both the aortic valve and the ascending aorta. These cells had the typical morphology of vascular endothelial cells in culture and contained angiotensin-converting enzyme activity. The cells derived from the aorta grew well in standard tissue culture dishes; the valvular endothelial cells, however, could not be passed serially in vitro unless cultivated in dishes precoated with fibronectin. Moreover, under the conditions used, the valvular cells showed substantial changes in their morphology when deprived of fibronectin coating for several passages but reestablished typical vascular endothelial morphology when subcultured to fibronectin-coated dishes. The aortic cells did not show this behavior. Endogenous labeling of proteins synthesized in vitro by the two cell types with 35S-methionine revealed that a high molecular weight protein produced in substantial quantities by the aortic cells was synthesized only in trace amounts by the valvular cells. This protein could be isolated by gelatin-agarose chromatography and showed an apparent molecular weight under reduced conditions of 235,000 to 245,000 on sodium dodecyl sulfate gel electrophoresis. Synthesis of gelatin-binding proteins, an indirect estimation of fibronectin production, as well as direct measurement of porcine fibronectin present in the culture supernatants using an immunoradiometric assay, showed that aortic endothelial cells synthesized and released 10-fold more fibronectin in vitro than did valvular endothelial cells. We suggest that this relative deficiency of in vitro fibronectin synthesis by valvular cells, if paralleled in vivo, may have important implications for the pathogenesis of lesions unique to the cardiac valve.
