Is writing as difficult as it seems?

Experiment 1 assessed the time and effort allocated to writing subprocesses while generating written and verbal protocols over 10 weekly writing sessions. Within a 40-min session, planning time consumed about 45% in the first 5 min, but stabilized at near 30% thereafter. Generating text initially consumed 40% of the writers' time, peaked at 50% midway, and then declined to its original level. The time spent revising and reviewing was negligible early in writing sessions, but increased substantially late in the sessions. The highest and lowest quality documents could be differentiated on the basis of the amount of time the writers devoted to revising and to the magnitude of their RTs in a secondary interference task. Writers showed consistent, distinctive patterns of transitional probabilities between writing subprocesses both within and across sessions, yielding quantitative representations of their writing styles. In Experiment 2, writers overestimated the amount of time they devote to revising and overestimated the amount of effort they allocate to planning and text generation. Their estimations did not improve after 10 weeks of composing. A time-and-effort-based analysis of writing is proposed to account for these data.

Outcome and long-term results following total hip replacement in elderly patients.

The authors reviewed the preoperative and postoperative charts and radiographs of 100 patients who were at least 80 years old when they underwent total hip arthroplasty. Seventy-six of these patients were available for further reexamination and evaluation. The average patient age was 85.2 years old (range, 80-97 years). The average followup period was 59.4 months (range, 26-146 months). The female to male ratio was 4 to 1. Osteoarthritis was the most common diagnosis. Hospital stay ranged from 12 to 39 days. Preoperative Charnley pain and walking scores averaged 5.1 points of a possible 12 points, and postoperative scores averaged 10.6 points. Eighty-eight percent of patients remained community walkers, and 60% used a cane. Four percent of the patients had 1 dislocation. There were no instances of mechanical loosening and no deaths. Subjective satisfaction was high. The level of independent living was maintained in 96% of patients at long-term followup. A satisfactory and cost-effective health outcome can be anticipated after total hip arthroplasty in octogenarian and nonagenarian patients.

Kallmann syndrome in two sisters with other developmental anomalies also affecting their father.

The classical features of Kallmann syndrome with anosmia and hypogonadotropic hypogonadism were observed in two sisters aged 13 and 19. They had additional malformations including anosmia, bilateral vesico-ureteral reflux and unilateral hearing loss. One of the girls had unilateral coloboma of the optic nerve. The father had unilateral hearing loss and duplication of the left ureter; he died of an unrecognized coarctation of the aorta. He had no clinical signs of hypogonadism or anosmia. It is suggested that the malformations observed in these patients may be due to a dominant inherited defect of embryonic cell migration, resulting in different phenotypic expressions within the same family, including the Kallmann syndrome.

Ornithine decarboxylase activity, prolactin blood levels, and estradiol and progesterone receptors in human breast cancer.

Ornithine decarboxylase (ODC) activity in human breast cancer tissues was correlated with prolactinemia (Prl), estradiol and progesterone cytosol receptors (ER and PR), and histopathologic pattern. Ninety-two cases of breast cancer, six benign mammary disease, and three normal breast tissues were studied for ER, PR, and ODC. Prolactinemia was assessed in 59 cancer patients, 14 of whom showed hyper-Prl along with significantly higher ODC than in patients with normal-Prl [(20.01 +/- 6.33) 10(-2) vs (5.20 +/- 0.90) 10(-2) pmol CO2/micrograms protein/h; P less than 0.0125]. A direct correlation was found between Prl and ODC in postmenopausal women (n = 40). Prl was assayed in seven of 13 ER-PR breast cancer patients; a highly significant, direct correlation was found between Prl and ODC in this group (r = 0.934, P less than 0.0025). ODC did not correlate with ER or PR. Carcinomas with higher ODC (n = 17) had higher cellularity, lower histologic differentiation, and higher nuclear anaplasia than those in which ODC was not detectable (n = 13). In normal breast and five of six benign mammary disease tissues, ODC was not detectable. These findings suggest that ODC could be a reliable marker for prognosis.

The stability of the sigma sleep spindle.

The present study closely examined the distribution of sigma sleep spindle activity or six normal human males who slept undisturbed for approximately 8 h/night for 3 consecutive nights. Sigma spindle activity was monitored by an automatic spindle detector system which performed at 92.5% accuracy when judged against established visual criteria. Individuals differed markedly in total sigma spindle production across nights, but the failure to detect significant differences among nights and the large intra-class correlation suggests noteworthy inter-night stability of the sigma spindle.
