Antibody levels by indirect ELISA test in Trypanosoma evansi infected horses following treatment with quinapyramine sulphate.

An ELISA test was used to determine the persistence of antibody levels in horses following treatment for Trypanosoma evansi. In 17 horses with T. evansi from two farms treated and cured with quinapyramine sulphate, ELISA antibody levels fell progressively post-treatment, but remained with positive results for 22.6 months in one horse, 12.8 months in a second, 4.1 months in another four and 2.3 months in three, whilst the rest became negative at 2.3 months. In two horses that suffered a post-treatment infection relapse the decrease in ELISA levels was only temporary, and a new increase in antibody levels was proven. The follow-up of these antibody levels could prove useful in clinical cases and in epidemiological studies, as well as for assessing the efficacy of drug treatment.

Persistence of Tritrichomonas foetus in naturally infected cows and heifers in Argentina.

Tritrichomonas foetus infection was investigated in 76 pregnant and 64 non-pregnant cows slaughtered in the local abbattoir and in two different lots of first-service heifers that were found to be non-pregnant 60 days post breeding (PB). In live and slaughtered animals, mucus samples were obtained from the vagina and from the vagina and uterus, respectively, using a "screw-head scraper rod". In pregnant cows, samples of amniotic and allantoid fluid were also collected, as well as samples from the stomach contents of the fetuses. All samples were cultured in Modified Plastridge Medium. T. foetus was isolated from three pregnant and two non-pregnant slaughtered cows. Parasites were recovered from the vagina of these five cows, as well as from the uterus in two cases and from the fetus in one case. Lot I of first-service heifers consisted of 323 females from eight different farms. Bulls infected with T. foetus from these farms were culled or treated, and heifers found empty at diagnosis of pregnancy were culled. Lot II consisted of 120 heifers from a single farm where T. foetus was controlled only in bulls. All heifers from Lot I were T. foetus negative. In Lot II, 12 of 120 heifers (10%) were T. foetus positive. In ten of these the parasites were observed once, in one at 60 days PB, in seven at 160 days PB and in two at 240 days PB; in the remaining two infected heifers, an irregular pattern of isolation persisted during 300 days PB. On the basis of these results, control methods are discussed and analysed.

Arterial red cell indices in preterm and term infants at birth.

Baseline arterial red cell indices were determined at birth in 105 infants of various gestational ages. These arterial samples were obtained through an umbilical artery catheter or radial artery puncture from a selected group of patients who were evaluated for respiratory distress or possible infection. The red cell distribution width, a quantitative measure of heterogeneity of red cells in the peripheral blood, was consistently elevated, revealing a physiologic state of anisocytosis in the newborn. This new information has provided a useful baseline reference for normal red cell indices from arterial blood in neonates in the first 24 hours of life.

Epidemiologic features of pain in pediatric cancer patients: a co-operative community-based study. North Central Cancer Treatment Group and Mayo Clinic).

The prevalence, etiology, and management of pain in pediatric cancer patients seen at the Mayo Clinic and member institutions of the North Central Cancer Treatment Group were assessed. Participating centers, including both primary care and referral institutions, surveyed all patients seen during a 1-week period (Monday through Friday); procedure-related pain was excluded. Of the 160 children surveyed, 28 reported pain of which 57.8% was related to a side effect of anticancer treatment, 21.1% was unrelated to the malignancy, and 21.1% arose directly from the cancer. Pain intensity assessment was performed by both health-care professional and patient using a variety of measurement tools. Correlation between assessors was close except in young children. The predominance of treatment-related rather than cancer-related pain differs from results in series in adult cancer patients.

Evaluation of erythrocyte disorders with mean corpuscular volume (MCV) and red cell distribution width (RDW).

The red cell distribution width (RDW), which provides a quantitative measure of heterogeneity of red cells in the peripheral blood, and the mean corpuscular volume (MCV) are part of the routine red cell indices reported by automated blood analyses. This study evaluated 193 pediatric patients with a wide range of erythrocyte disorders and determined the diagnostic utility of the RDW in relation to the MCV. Six different groups of erythrocyte disorders by MCV and RDW values are described: low MCV/normal RDW, low MCV/high RDW, normal MCV/normal RDW, normal MCV/high RDW, high MCV/normal RDW, high MCV/high RDW. This combination established a useful differential diagnosis of erythrocyte disorders. The data provided a baseline against which future studies of infants and children can be compared, though each laboratory has to verify its own normals. It should be cautioned that different electronic counters yield different RDW values, so there have to be qualifications when reporting reference values. The RDW may find its best use as a guide in the differential diagnosis of anemia, rather than as a definitive test per se.

Acute nonlymphocytic leukemia in children.

Acute nonlymphocytic leukemia (ANLL) is a heterogeneous group of hematologic malignancies caused by a clonal proliferation of primitive myeloid cells. 30 Infiltration of the bone marrow by these cells causes impairment of normal hematopoiesis and results in anemia, granulocytopenia, and thrombocytopenia. Recent progress in bone marrow culture techniques, cytogenetics, and immunology have contributed to a better understanding of the pathophysiology of ANLL. Major therapeutic advances have also occurred because of improved supportive care and carefully designed prospective and randomized clinical trials. This article presents the current knowledge of the diagnosis and management of ANLL.

Hereditary red cell disorders in Southeast Asian refugees and the effect on the prevalence of thalassemia disorders in the United States.

This investigation established the frequency and prevalence of hemoglobin E, alpha and beta thalassemia, and glucose-6-phosphate dehydrogenase deficiency in a group of Southeast Asian individuals and determined the impact upon the prevalence of thalassemia disorders in the United States. The resettlement program of Southeast Asian refugees has significantly increased the total number of serious thalassemic disorders in the United States. For several generations Southeast Asian children will be the principal thalassemic group.

Anemia and edema as presenting signs in cystic fibrosis: case report.

Anemia, edema, and length and weight below the 10th tile were the presenting signs in an infant with cystic fibrosis. She had a peripheral blood smear characterized by poikylocytosis, acanthocytosis and anisocytosis; low serum total proteins, albumin, and tocopherol/total lipid ratio. Following two months of treatment with oral pancreatic replacement therapy and tocopherol, her hemoglobin concentration, serum proteins and tocopherol/total lipid ratio returned to normal. The peripheral blood smear showed normal red cell morphology and her length and weight were above the 50th percentile. The mechanisms of anemia in cystic fibrosis patients are discussed.

Hematologic genetic disorders among Southeast Asian refugees.

Resettlement of Southeast Asian refugees has introduced into the Western Hemisphere many persons of all major ethnic groups from Indochina. They represent several distinctive cultural, genetic, and linguistic groups, and the prevalence of genetic traits among them varies accordingly. We studied 778 Southeast Asian persons resettled in the upper Midwest who belonged to 182 unrelated families from the five major Southeast Asian ethnic groups. High prevalences of hemoglobin E, alpha- and beta-thalassemia disorders, and glucose-6-phosphate dehydrogenase deficiency were found. The prevalences of these four conditions in the refugees are among the highest known in the world. For these groups, iron deficiency is an uncommon cause of microcytosis; instead, the most frequent causes are hemoglobin E and alpha-thalassemia-1. Very serious thalassemic disorders occur with unusually high frequency in the refugees, especially in the Tai-Dam.

Anemia in infancy and childhood. A systematic approach to evaluation.

The evaluation of the infant or child with possible anemia requires accurate testing and use of age-specific reference standards for normal values. Simple laboratory tests such as a well-performed peripheral smear to establish specific red cell morphology and red cell indexes to determine red cell size are useful to classify anemias as microcytic hypochromic, normocytic normochromic, or macrocytic normochromic. Addition of the reticulocyte index to these tests allows subdivision of these anemias according to three different pathogenetic mechanisms: hypoproliferation, hyperproliferation, or abnormal maturation of red cells. Patient management depends on systematic determination of the pathogenesis of the anemia, because some anemias are of a genetic origin and no specific treatment is needed, some are best treated by specific replacement, and some are dependent on treatment of the primary disease.

Disseminated histiocytosis X complicated by diffuse erythrophagocytosis: report of two cases.

Two children with the clinical and histopathological features of disseminated histiocytosis X became refractory to chemotherapy and a diffuse erythrophagocytic process developed. No cause of the erythrophagocytosis could be found. Pancytopenia and marked erythrophagocytosis persisted until death ensued. The relationship of this disorder to familial erythrophagocytosis is discussed.

Indochinese refugee health assessment and treatment.

Many Southeast Asian refugees have resettled in the United States. This report reviews data from 426 refugees who underwent comprehensive examination and treatment at the Mayo Clinic. Health problems identified were primarily selected contagious illnesses and stress syndromes with functional complaints. Counseling was necessary in 17 percent of adults for psychosomatic problems or psychiatric disorders. When family-planning issues were applicable and addressed, 80 percent of women chose some method of contraception. The prevalence rate of intestinal parasites was 82 percent, and pathogens necessitated persistent treatment and follow-up. Tuberculosis skin-sensitivity testing was positive in 54 percent; the risk of active disease warrants prophylactic treatment until age 35 years. Hepatitis antigen was positive in 13 percent; precautions should be taken for potentially exposed professionals. Hematologic genetic disorders were very common and accounted for most of the 25 percent incidence of microcytosis. Physicians should become aware of cultural attitudes and treatment acceptance among Indochinese patients and should carefully investigate for infectious diseases.

Erythrocytic calmodulin correlates with red cell age.

Red blood cells from normal persons and from patients with reticulocytosis were tested for their calmodulin content. Elevated calmodulin correlated with high reticulocyte counts, rather than with any specific disorder. The correlation between reticulocyte content and elevated calmodulin was further tested by fractionating the erythrocytes from 12 patients into reticulocyte-rich and reticulocyte-poor fractions. In the samples from each patient, erythrocytic calmodulin increased with increasing reticulocyte count.

[An evaluation of the direct agglutination test for the diagnosis of "mal de caderas" in horses].

The usefulness of the direct agglutination test (DA) to diagnose Mal de Caderas disease was evaluated. Forty four sera samples from two lots of horses with natural T. evansi infection (Lot 1 and Lot 2) were used. Thirteen (81.2%) of sixteen horses in which parasites were isolated gave positive agglutination titres (> or = 1:512) in the DA test. Treatment of these positive sera with 2-mercaptoethanol drops three to eight dilutions the agglutination titres in twelve samples (92%), showing the IgM nature of these antibodies. The DA test was also positive in seventeen of twenty eight horses in which parasites could not be detected. Five T. evansi infected horses, Lot three, which had high antibodies levels in the DA test, were treated with Naganol (Bayer-Germany). In four animals these antibodies were mainly IgM. In agreement with negative control for parasites, two months after treatment, IgM could not be detected while IgG antibodies remained detectable in low titres 12 months in three of the five horses. Fifty control horses sera from a T. evansi free area were AD negative. The DA and DA+2-ME are recommended as a routine method to diagnose Mal de Caderas disease in combination with parasitological diagnostic methods.
