Gastric epithelial atypia complicating hepatic arterial infusion chemotherapy.

Severe gastric lesions developed in five patients during hepatic arterial infusion chemotherapy employing 5-fluorouracil plus either leucovorin or mitomycin C. Ulceration with marked epithelial atypia was observed with striking structural distortion and cellular pleomorphism of the affected gastric mucosa. These features should be considered to be a complication of such treatment and should not be misinterpreted as malignancy.

"Asymptomatic" ruptured aneurysm: a report of two cases of aortocaval fistula presenting with cardiac failure.

Two cases of aortocaval fistula are described in patients with an otherwise asymptomatic abdominal aortic aneurysm. Both presented because of cardiac symptoms, one with chest pain and acute heart failure and electrocardiogram signs of acute coronary ischaemia, the other with a long history of chronic cardiac failure resistant to therapy. In the first case the fistula was proven by means of a CAT scan. Positive proof of a fistula or leakage is important because asymptomatic aneurysms should not be operated on in cardiac compromised patients. On the other hand, if an aortocaval fistula is present, operation is necessary to prevent fatal cardiac failure.

[Left heart by-pass in case of ventricular fibrillation (author's transl)].

The authors have studied in 11 mongrel dogs the hemodynamic conditions created by a transarterial closed chest left by-pass (TaCLVB) in ventricular fibrillation. The attention was focussed on the function of the right atrium. This function is recorded with closed chest by means of a catheter in the external jugular vein. The experiments show a left heart by-pass can function only if there are good contractions in the right atrium. The right atrium insures pulmonary circulation during ventricular fibrillation. This well continue for 30 to 60 minutes by means of the TaCLVB then the right atrium will fail and fibrillate. The animals died of right heart failure.
