[Immunologic studies in chronic urticaria].

Chronic urticaria is considered to be a heterogeneous disease, which can be divided into several subclasses. The authors investigated 126 patients suffering from chronic urticaria with 41 of unknown origin. The aim of the present study was to work out a strategy for the complete investigation of patients with chronic urticaria. The findings of the immunological study modified the results obtained by medical assessment of the 126 chronic urticaria cases, diminished the number of the patient with chronic urticaria unknown etiology.

[Clinicopharmacology of Arduan].

The paper presents clinicopharmacological investigations of Arduan carried out in Hungary and in other countries. The aim of the research work is to find an ideal muscle relaxant without side-effects, which are mainly cardiovascular. Pipecuronium bromide (Arduan) is a compound with steroid structure; it has a long duration of action and, most importantly, it does not affect circulation. The properties of pipecuronium bromide will be discussed and compared to those of pancuronium and vecuronium bromide.

[Chronic urticaria].

66 patients with chronic urticaria were examined using a three-stage examination protocol. Stage I. consisted of case history, general dermatological and internal physical examination and routine laboratory tests. Stage II. comprised investigation for focal infections (dental, oto-laryngeal, gynecological, bile culture), determination of AST, acidity tests of the stomach and aimed radiological examinations. Stage III. consisted of immunological laboratory tests (immune complex, Igs, IgE, special IgE determination) and allergological examinations (epicutaneous, intracutaneous probes, Prick test).

Alpha-fetoprotein in vaginal fluids. The early diagnosis of premature rupture of membranes.

Alpha-fetoprotein, one of the most characteristic proteins of pregnancy, is present in the amniotic fluid during the whole period of pregnancy. In the case of premature rupture of the membranes it is appearing in the vaginal fluid. The level of AFP was measured by the ELISA technique in the vaginal fluid of pregnant women. In 70 cases of the rupture of membranes and in 33 cases of splitting of the membranes the AFP was measurable in the vaginal fluid and it was not detectable in 31 cases of urine and in 16 cases of vaginal fluids of pregnant women without any sign of premature rupture of the membranes.

[Subacute lupus erythematosus of the skin--clinical aspects and laboratory findings].

UNLABELLED: The data of 20 patients with subacute cutaneous lupus erythematosus (SCLE) were evaluated according to clinical and laboratory criteria. We found the following laboratory deviations: elevated BSR, leucopenia, anti-dsDNA, anti-SSA/Ro and anti-SSB/La antibodies, positive LE cells, and elevated levels of IgE and immune complex. Aside from the characteristic skin eruptions in SCLE, the patients showed arthralgia, myalgia, and pronounced photosensitivity. THERAPY: corticoids, chloroquine, and light protection. Regular follow-up examinations are extremely important in these patients.

Serum IgE level in systemic lupus erythematosus.

The serum total-IgE level has been studied in systemic lupus erythematosus (SLE), in order to obtain data concerning the significance of IgE antibodies in the appearance of SLE symptoms. In most patients the serum IgE content was significantly higher in the active stage of the disease than during remission. The findings indicated a specific statistical distribution due to the multiple factors influencing IgE production and so an augmented IgE level cannot be regarded as a reliable feature of SLE activation.

IgE level in some dermatological diseases.

The total IgE level was measured by radioimmunosorbent test (Phadebas RIST, Pharmacia) in blood samples of 454 dermatological patients and 26 blood donors aged 14-70 years. The results showed different IgE values in the same groups of diseases. Elevated serum IgE values were observed in atopic dermatitis, drug allergy, discoid lupus erythematosus and chronic dermatomycosis. In drug allergy the average nonspecific IgE level of patients reacting with immediate reaction was higher than the normal value. Tested individually with Penicilloyl G RAST no strong correlation could be demonstrated between the quantity of specific and non-specific IgE.

[Double-stranded DNA antibodies in discoid lupus erythematosus].

Antibodies to double stranded ds DNA were comparatively studied in the sera of untreated DLE patients and in the sera of the same persons after one month's chloroquine administration by a modified ELISA technique. Antibodies were found in both IgG and IgM classes. Untreated DLE patients showed a significantly higher antibody level than the healthy controls. During the chloroquine administration, the level of antibodies decreased first in the IgG class.

[T & B-lymphocytes and immunoglobulins of the serum in drug eruptions of immediate type (author's transl)].

Lymphocyte subpopulations and serum immunoglobulins of patients with drug-hypersensitivity were studied. The results are as follows: Lymphocyte subpopulations: decrease of the E-rosette forming cells was observed in both active and symptom free state. The number of IgE-bearing lymphocytes in patients with drug-allergy was higher than that of the controls. Serum-immunoglobulins: The IgE-level in the serum of patients with drug-allergy was higher in active state than afterwards, but still higher than in the controls but it was lower than in atopic dermatitis patients. The level of the other immunoglobulins was normal.

Investigation of some cord blood components.

The practical result borne by our investigations has been the introduction of routine estimation of cord blood total protein. In every case when the concentration is below the 10 percentile value, the clinicians are warned. Further catamnestic studies are needed to judge the values of cord blood cholesterol estimations in screening for familial hyperlipoproteinemias. The hypercholeterolemic infants of the present series, who are 2-3 years old at present, as well as their families must be subjected to repeat analysis. The change in opposite directions of cord blood total protein and cholesterol levels with the advance of gestation was a remarkable finding. This correlation indicates a close interaction between protein and lipid metabolism. Less promising results have been obtained in investigations of the cord blood calcium and magnesium levels, but final conclusions can only be drawn after repeat analyses. Introduction of the routine estimation of these two parameters does not seem necessary at present. Besides clinical data, biochemical parameters have also been found suitable for differentiating small-for-date infants from true prematures. The functional maturity of these infants was shown by the fact that their different cord blood components were normal.

Amniotic fluid creatinine and uric acid levels in high risk pregnancy.

Comparison of two methods of creatinine and uric acid assay in amniotic fluid showed that the two-point-reaction rate measurement of creatinine and the enzymatic assay of uric acid give more reliable results than the methods used previously. At the end of pregnancy, increased creatinine and uric acid levels were found. In cases of blood group incompatibility the values were mostly in the normal range, while in diabetic or toxaemic mothers higher, normal and lower values were equally found. It has been concluded that a single investigation does not suffice for the estimation of fetal maturity. Only repeated tests showing an increase in the creatinine and uric acid levels offer reliable information.
