Electrochemical assembling/disassembling of helicates with hysteresis.

A series of eight tetradentate, ditopic, bisimino bisheterocyclic ligands (1-8), and their complexes with Cu(I) and Cu(II), have been studied in CH(3)CN solution, by means of (1)H NMR, mass, and UV/vis spectroscopy, while the crystal and molecular structure of the Cu(II) complexes [Cu(3)](CF(3)SO(3))(2) and [Cu(4)](CF(3)SO(3))(2) and of the Cu(I) complexes [Cu(2)(4)(2)](ClO(4))(2) and [Cu(2)(5)(2)](ClO(4))(2) have been determined by X-ray diffraction methods. The Cu(II) complexes are monomeric, almost square-planar structures, both in solution and in the solid state, while the Cu(I) complexes are two-metal, two-ligand dimers which can be both helical and "box-like" in the solid, while they adopt a simple helical configuration in acetonitrile solution. The systems made of ligands 1-8 and copper are bistable, as under the same conditions either the Cu(I) helical dimers or the Cu(II) monomers can be obtained and are stable. The electrochemical behavior of the 16 copper complexes has been studied in acetonitrile solutions by cyclic voltammetry. One reduction and one oxidation wave were found in all cases, which display no return wave and are separated by a 500-1000 mV interval. Irreversibility is due to the fast self-assembling process that follows the reduction of [Cu(II)(L)](2+) and to the fast disassembling process that follows the oxidation of [Cu(I)(2)(L)(2)](2+) (L = 1-8). However, the overall [oxidation+disassembling] or [reduction+self-assembling] processes, i.e., [Cu(I)(2)(L)(2)](2+) = 2[Cu(II)(L)](2+) + 2e(-), are fully reversible. Moreover, CV profiles show that solutions containing copper and L undergo hysteresis on changing the applied electrochemical potential: in the same potential interval, the systems can exist in solution as either [Cu(I)(2)(L)(2)](2+) or [Cu(II)(L)](2+), depending on the electrochemical history of the solution. Moreover, by changing the structural or donor features of the ligands it is possible to modulate the potentials at which the system undergoes a transition from one to the other of its two possible states, in the hysteresis cycle. In addition, the spectral properties of the Cu(I) and Cu(II) complexes of the considered ligands make these systems good candidates for storing information in solution, which can be electrochemically written or erased and spectroscopically read.

Role of NK2 receptors in gastric barosensitivity and in experimental ileus in rats.

This study was performed to evaluate the role of tachykinin NK2 receptors in gastric barosensitivity and in postsurgical intestinal atony, using a selective NK2 antagonist (MEN 11420). Gastric distensions were performed in rats equipped with a gastric balloon and electrodes implanted in the neck muscles. Ileus was produced by laparotomy and caecum palpation in rats previously prepared with electrodes implanted on the proximal jejunum. Fifteen minutes before gastric distension or laparotomy, the animals received MEN 11420 (10, 100 or 200 microg kg-1 intravenously) or saline. The first distending pressure to increase the integrated neck electromyogram > 100% was considered the pain threshold. MEN 11420 (100 microg kg-1) increased significantly pain threshold (20.5 +/- 1.2 vs. 17.0 +/- 0.8 mm Hg) but did not modify gastric volumes at the three doses tested. Abdominal surgery was followed by a total inhibition of jejunal spiking activity lasting 80.4 +/- 18.7 min. MEN 11420 (10 and 100 microg kg-1) shortened the duration of motor inhibition by 36 and 39%, and induced a premature recovery of the phase III of migrating myoelectric complex at the lowest dose tested (130 +/- 32 vs. 192 +/- 28 min). We conclude that NK2 receptors, probably located on afferent fibres, are involved in gastric barosensitivity and in postsurgical intestinal atony.

Role of tachykinin NK2 receptors in normal and altered rectal sensitivity in rats.

Irritable bowel syndrome is characterized by visceral hyperalgesia commonly associated with stress and inflammatory processes. We investigated the role of tachykinin NK2 receptors in the ability of trinitrobenzenesulphonic acid (TNBS) and stress to enhance the sensitivity of the rat rectum to distension using a selective tachykinin NK2 receptor antagonist (MEN 11420). Rats were fitted with electrodes implanted in the striated muscles of the abdomen. Rectal distension (RD) was performed with a balloon inflated by steps of 0.4 ml from 0 to 1.6 ml. Five groups were submitted to RD performed 3 days before and after intrarectal instillation of TNBS. Fifteen minutes before RD, rats were treated with saline or MEN 11420 (5 - 100 microg kg(-1) i.v.). Two other groups, submitted to 2 h restraint or sham stress sessions were randomly treated i.v. with saline or MEN 11420 (10 - 200 microg kg(-1)) prior to RD applied 20 min later. The basal response to RD was characterized by a significant increase in the number of abdominal contractions. This response occurred with a threshold volume of 0.8 ml and was dose-dependently reduced by MEN 11420 (5 - 100 microg kg(-1) i.v.). Rectal inflammation lowered the volume of distension producing abdominal contractions to 0.4 ml (allodynia). This effect was either reduced or suppressed by MEN 11420. A similar allodynia was observed after a stress session and this effect was reduced (49%) or suppressed by MEN 11420 at 200 and 100 microg kg(-1), respectively. Sutureless scleral fixation of intraocular lenses.

A technique is presented in which pars plana fixation of a posterior chamber intraocular lens (IOL) is achieved by the transscleral passage of the haptics. The IOL used has three equidistant supple loops, each attached to an armed suture, which serve as haptics. A specially designed needle driver is used to pass the sutures into and out of the eye through the pars plana and along the surface of the sclera, where they are severed by cautery. Stable fixation is achieved without contact with the inner tissues of the eye, and no additional sutures are necessary. Lens stability is maintained even in the presence of asymmetrical fibrosis of the capsular remnants.

Artificial maintenance of the relative ocular tissue pressure during eye surgery.

We describe an operating table in which the whole patient, apart from the eye undergoing surgery, is enclosed in a caisson within which the barometric pressure can be lowered at any time during surgery. This increases the effect of atmospheric pressure on the eye, enabling 'open sky' surgery to be carried out on patients of any age.

[Mesenteric fibromatosis. Anatomo-clinical studies of a case].

Mesenteric fibromatosis is a rare lesion associated with an insidious onset and with a movable mass representing frequently the only expression of disorder clinically. This mesenchymal lesion is a well differentiated fibrous growth perhaps catalogued as a pseudotumor. The authors describe one case treated surgically. The general series is reviewed and clinical and pathological comments are made about the diagnostic and etiological aspects of the disease.

Lack of correlation between sex hormone binding globulin, adrenal and peripheral androgens in precocious adrenarche.

Sex hormone binding globulin (SHBG) is a specific steroid-binding plasma glycoprotein regulated by several different factors. Sex steroids are currently considered to be the main physiological regulators of this protein. Testosterone (T) in adults seems to be the main hormone active in lowering SHBG. The role of dihydrotestosterone (DHT) in such regulation, particularly in the prepubertal age, is not well understood, and no data exist about the role of 3 alpha-androstanediol (3A alpha) and its glucuronide. In adulthood, in addition to T, 5-ene steroids seems to play a role in the regulation of SHBG plasma concentration. To assess the effect of adrenal and peripheral androgens in modulating SHBG levels in the prepubertal age, we studied subjects with precocious pubarche secondary to precocious adrenarche (PA). PA represents, in fact, a good model of study as it is characterized by an increased production and action of adrenal androgen in females under 8 yr of age and in males under 9. Sixty-five subjects (55 females and 10 males; chronologic age: 3.6 - 8.2 yr (6.9 +/- 1.3, SD); bone age: 3.6 - 11 yr (7.6 +/- 1.9); BMI 17.9 +/- 3 kg/m2) were studied. Fifteen age-matched normal children (BMI 15.2 +/- 0.8 kg/m2) were studied as controls. Androstenedione (A), dehydroepiandrosterone (DHA) and its sulphate (DHA-S), T, DHT, 3Ad and its glucuronide (3AG) and SHBG were evaluated in all subjects. In PA cases serum SHBG levels (50 +/- 27 nM) were significantly lower (p less than 0.05) with respect to normal prepubertal patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Tendon surgery in Brown's syndrome.

PURPOSE: Longitudinal tenectomy of the reflected tendon of the superior oblique muscle resulted in full recovery in a series of cases of Brown's syndrome after surgery by one of the authors. METHODS: A survey of the records of cases of Brown's syndrome was done to assess the criteria for patients requiring surgery and to evaluate the methods used. Included in the study were 9 eyes of patients who underwent surgery over the course of 1 year. Criteria identified in cases in this study include: preoperative compulsory head tilt in primary position of gaze, severe restriction of Hess chart in the affected eye, and an enlarged field in the healthy eye; intraoperative thickening of the tendon preventing free slippage through the trochlea in the forced duction test. The surgical method consisted of trimming a swelling that was consistently found on the reflected tendon of the superior oblique muscle in all patients undergoing surgery. Enough of the swelling was removed to allow free slippage of the tendon through the trochlea, as judged by the forced duction test. Sulphamide powder soaked in delayed action steroid compound (methylprednisolone acetate) was applied to prevent postoperative adhesions. RESULTS: A normal head posture and a satisfactory upward vergence were achieved in all patients from the first days after surgery, and were maintained throughout the follow up, as shown in the pre- and postoperative photographs and Hess charts. CONCLUSIONS: A preliminary exploration of the reflected tendon of the superior oblique muscle is warranted in cases of Brown's syndrome; when a thickening of the tendon is found, its surgical trimming is advised. Our findings suggest that delayed development of the trochlea may be a possible cause of Brown's syndrome.

Scleral fixation for a phakic anterior chamber disc intraocular lens.

BACKGROUND: Angle-supported phakic anterior chamber intraocular lenses (IOLs) require a high degree of anterior vaulting, but this can cause an inadequate distance between the IOL and the cornea, especially during rubbing or squeezing of the eye. METHODS: A vaultless implant was designed by anchoring 3 supple hepatics through the anterior chamber into the sclera. Thus, a suspended rather than a wedged implant was achieved. RESULTS: The lens was implanted in 3 eyes of 2 patients in 1990. Good tolerance of the IOL was maintained over 7 years of follow-up. CONCLUSION: A suspended anterior chamber phakic IOL maintains an adequate position with lack of bowing and shows promising results.

[Campylobacter fetus subsp. jejuni: etiological, epidemiological and preventive aspects].

The Authors, after a brief introduction, examine on the basis of the most recent literature, the taxonomy and the microbiological features of Campylobacter fetus subsp. jejuni, giving a detailed description of the chemical composition, of the biochemical, physiological and nutritional characters, of the antigenic composition and of the cultural methods. Later on, they state the problems arising from the epidemiology of the infections caused by this germ either in man or in animals, examining the most relevant sources of infection. Finally, the Authors describe the most recent laboratory diagnostic methods, the pathogenesis, the clinical manifestations and the prevention of infection, concluding with some therapeutical remarks.
