Severe constrictive pericarditis as an unsuspected cause of death in a patient with idiopathic hypereosinophilic syndrome and restrictive cardiomyopathy.

A 43-year-old man with idiopathic hypereosinophilic syndrome survived a relatively long term (6 1/2 years) before he succumbed to intractable heart failure. Six months before death, his chronic heart failure from restrictive cardiomyopathy was well compensated. Autopsy demonstrated severe constrictive pericarditis which was not suspected antemortem. Constrictive pericarditis as a late complication of idiopathic hypereosinophilic syndrome is discussed.

Endobronchial lipoma.

A symptomatic patient with obstructive pneumonitis, mimicking bronchogenic carcinoma, was bronchoscoped and found to have lipoma in his left mainstem bronchus. The diagnosis could not be obtained until the mass was totally removed by bronchoscopic excision. The English literature regarding endobronchial lipoma is briefly reviewed.

Hypercalcemia in squamous cell carcinoma of the skin.

Squamous cell carcinoma of the skin is an extremely rare cause of hypercalcemia associated with malignant neoplasms. The case of a patient with extensive squamous cell carcinoma of both buttocks and thighs, with severe hypercalcemia is reported. The data presented indicate secretion of a humoral factor by the tumor, producing hypercalcemia.

Renal plasmacytoma.

Plasmacytoma, a localized extramedullary mass of plasma cells, is one manifestation of multiple myeloma. Renal plasmacytomas are rare lesions with neovascularity on arteriogram. This may cause confusion with the much more common renal cell carcinoma unless the clinician is aware of this entity. A case is presented, the literature reviewed, and problems in management discussed.
