[Male fertility in the elderly].

Fertility in the elderly is still a less known argument. We report some news about seminal parameters (SP) and testicular hystological parameters (THP) in this age. About SP: the volume of the ejaculate, the number and the motility of spermatozoa are reduced. The morphology is typical: spermatozoa with coiled tails are common. About THP: some testes show a normal hystological architecture; others show a reduced volume and some aspects of hypospermatogenesis or maturative arrest; others show a very reduced volume, thickness of the tubular basal membrane and other signs of cellular regression. These signs are dependent upon the vascular insufficiency and the hormonal alterations which may occur in the elderly.

Gonadotropin evaluation in the diagnosis of polycystic ovary syndrome using either a monoclonal or a polyclonal antibody radioimmunoassay.

The LH/FSH ratio values between gonadotropins dosed with a monoclonal antibody assay (IRMA) in the micropolycystic ovary syndrome (PCOS), are discussed and compared to those obtained with the classic assays using polyclonal antibodies. Because of the higher selectivity of this IRMA assay it is noteworthy that the cut-off value between normal and PCOS patients is now equal to or above one. The evaluation of the LH/FSH ratio between the peak values of the two gonadotropins after a GnRH 100 micrograms iv bolus, may be useful in the diagnosis of PCOS in those patients who present an LH/FSH less than 1 in basal conditions even in the presence of clinical and ecographic aspects of PCOS.

The GnRH 10 micrograms test promotes a maximal gonadotropic response in normal women.

The authors have compared the results of gonadotropin stimulation with a gonadotropin releasing hormone (GnRH) intravenous bolus of 100 micrograms and of 10 micrograms in a group of eumenorrheic women. They conclude that there are no differences between the responses of the two dosages in normal women. Therefore the authors clarify that the dosage of 10 micrograms of GnRH as a bolus test may be used currently in clinical practice in order to assess gonadotropic function.

[The alpha blockers].

Because the bladder neck contains a large number of alpha-adrenergic receptors, use of alpha-blockers to facilitate micturition in early benign prostatic hypertrophy has been advocated. Effects on micturition and urinary flow were fairly favorable. No adverse effects were recorded. The effect of therapy was transient and slightly greater than the effect of the placebo.

A case of methimazole-induced hypothyroidism in a patient with endemic goiter: effects of endogenous TSH hyperstimulation after discontinuation of the drug.

Serum thyroid hormone and TSH concentrations were monitored in a patient with multinodular endemic goiter and severe methimazole (MMI) induced hypothyrodism up to 190 days after drug withdrawal. Serum concentrations of TT3, TT4 and TSH returned to normal values at the 6th., the 140th, and the 120th. day respectively. Within the first 20 days after MMI withdrawal the increase of serum T3 levels was correlated with the observed decrease of serum TSH concentrations. Successively T3 values decreased and T4 levels progressively increased. Six months after MMI withdrawal basal serum TSH concentration was normal while an exaggerated response to TRH was observed. We think that this peculiar hormone pattern is due to iodine depletion. In this case TSH hyperstimulation increases predominantly T3 secretion demonstrating the reduced thyroidal ability to produce T4 when hyperstimulated.

Antiandrogenic properties of spironolactone. Clinical trial in the management of female hirsutism.

The effect of spironolactone on female hirsutism was studied in 18 patients. The drug was administered at the dose of 400 mg for the first ten days and 300-200 mg later on in a first group of women (Group A); a second group (Group B) was given 200 mg spironolactone for the whole length of therapy. A significant decrease of the index of Ferriman and Gallwey (p = 0.01) was noted from the 100th day of treatment; acne and seborrhoea improved concomitantly. Plasma total testosterone values fell from 0.64 +/- 0.24 ng/ml to 0.32 +/- 0.12 ng/ml (p = 0.002) during the first 5 days only in the patients of Group A; in the other patients no significant changes were observed. PRL did not significantly change from pretreatment values; FSH and LH values at the 5th, 10th, and 15th day of therapy did not show a uniform course in both groups. On the basis of these results spironolactone administration appears promising in the therapy of female hirsutism.

Feminine urinary incontinence and urodynamic examination.

The Authors performed urodynamic examinations on all patients affected by urinary incontinence. This examination enabled them to classify incontinence precisely, from the etiopathogenetic point of view. Incontinence may be due to an altered transmission of the abdominal pressure, to sphincteral lesions or insufficiency, or to stimuli. This classification provides clear indications for the choice of the most suitable therapy (surgical, medical, instrumental). It thereby contributes to improving the outcome and decreasing the number of relapses so far reported by all Authors using any of the suggested surgical techniques.

Oral and topical spironolactone therapies in skin androgenization.

The most important clinical studies using spironolactone as an antiandrogen drug either per os or topically are referred. Menstrual disturbances very often occur during SP treatments thus limiting its systemic use. As far as the topical use is concerned SP seems to be highly effective with absence of systemic effects. Local mild side effects were present in a small number of patients.
