[Endocrinological aspects of long-term lithium therapy].

Investigations were carried out on two groups of patients suffering from cyclophrenia, atypical psychosis and schizophrenia. Both groups as well as a control group consisted of 30 persons each. The first of the psychiatric groups was treated with lithium carbonate and neuroleptics during a period of time ranging from 6 months to 8 years. The second psychiatric group was treated only with neuroleptics. The following parameters were being specified: The level of cortisol, insulin, somatotropin (GH) and lithium in the blood serum and also the glucose level in blood. Furthermore, the influence of lithium carbonate upon cortisol secretion and GH after insulin stimulation was also examined. The results of our work prove that the long-lasting treatment with lithium, when proper doses are carefully chosen, does not cause any significant changes of all the examined parameters.

[Short-term use of lithium carbonate in the treatment of thyrotoxicosis].

The examinations were performed on 3 groups of altogether 65 persons with thyreotoxicosis, the cause of which was either Basedow's disease or struma nodosa. The first group received metizol (thiamazol) in a daily dosage of 60 mg, the second group lithium carbonate (1.0 up to 1.5 g/a day), the third group metizol together with lithium carbonate. The groups were of the same value, concerning the degree of the symptoms of hyperthyroidism. The examinations showed that when lithium carbonate is used alone at the earliest a significant decrease of the serum T4 and T3 concentration as well as of the T3 binding index appears. After a treatment of seven days the therapeutic effects even up. Under the lithium therapy an essential improvement of the clinical findings was achieved. The combined therapy with lithium and metizol did not exhibit any advantages in this respect. The side-effects observed under the lithium therapy are no essential clinical problem.

Changes of plasma arginine-vasopressin level in patients with hyperthyroidism during treatment.

BACKGROUND: Hyperthyroidism is associated with several kinds of changes in the circulatory system, including alterations in blood volume. Arginine-vasopressin (AVP) is known to be one of the major factors regulating plasma volume. The present study was designed to evaluate the plasma AVP level in patients with hyperthyroidism during treatment. MATERIAL AND METHODS: AVP was measured under basal conditions and after stimulation with a low-sodium diet and upright position. Seventy-four patients with hyperthyroidism and 37 controls were investigated. Measurements were taken before treatment, two weeks after pharmacological treatment, and after attaining euthyroid status. The following indices were determined: AVP, total and free thyroxin and triiodothyronine, thyrotropin, sodium, potassium, hematocrit, arterial blood pressure, cardiac index, and total peripheral resistance index. Plasma osmolality and changes in plasma volume were calculated indirectly. RESULTS: Plasma AVP was higher in patients with hyperthyroidism before treatment. After normalization of thyroid status, the AVP level was similar to that of the controls. The application of a low-sodium diet and upright position resulted in a greater decrease in plasma volume than the controls. AVP correlated with thyroxin level and plasma osmolality in patients with hyperthyroidism. CONCLUSION: Enhanced AVP level in patients with hyperthyroidism is suggested to be the result of alterations in plasma volume and is relatively independent of changes in plasma osmolality.

[Effect of misoprostol, a synthetic equivalent of prostaglandin E1, on bronchodilation in bronchial spasm].

A effect of a single dose 400 micrograms misoprostol (Cytotec-Searle) on a degree of the bronchospasm relief has been investigated in 8 patients, and the results have been compared with those produced by aminophylline i.v. infusion in the rate of 500 mg/hour. Prior to and 10, 20, 30, 60, 90, 120, 150 and 180 minutes after administration of both drugs, the following parameters of pulmonary ventilation have been determined: VC, VC%, FEV1, FEV1%, MVV, and MEFR. The results have shown that misoprostol given in a single oral dose of 400 micrograms to patients with bronchospasm has the bronchodilating activity which is statistically significant. However, this activity is rather weak and significantly lower than that of intravenous aminophylline infusion.

Markers of tissue response to thyroid hormones in patients with hyperthyroidism or hypothyroidism.

Changes in serum cholesterol and aminoterminal type III procollagen propeptide (PIIIPN) were compared in 33 patients with hyperthyroidism, 16 patients with hypothyroidism, and 23 healthy individuals. An increase in PIIIPN was found in patients with hyperthyroidism and an increase in serum cholesterol level was associated with hypothyroidism. There was no correlation between investigated indices. It is suggested that PIIIPN changes were a result of altered connective tissue metabolism, while serum cholesterol was related to the intensity of body metabolism.
