[Resection of hepatocarcinomas in liver cirrhosis: the value of intraoperative echography].

In the last 3 years 21 cirrhotic patients with hepatocellular carcinoma were operated. All patients fulfilled the criteria of localized tumor and good liver function and underwent a laparotomy with the aim of resecting the tumor. Intraoperative ultrasound examination was essential to localize a non visible and non palpable tumor in 2 cases and it was useful to detect other nodules in 3 of the 16 patients who underwent a hepatic resection of the tumor. In 5 patients resection was considered not indicated. Therefore, the initial therapeutic approach of tumoral resection was changed in 23.8% of patients. Also, intraoperative ultrasonography was essential for performing a guided resection of hepatocellular carcinomas in cirrhotic liver with the aim of preserving as much liver parenchyma as possible.

Neuromesenchymal hamartoma of the small bowel.

We present the case of a long small bowel stricture with pathological features similar to those described as "neuromuscular and vascular hamartoma," showing in addition extensive fatty submucosal infiltration and fibrous intramural nodules. In the controversy about the nature of this disorder, we consider it a separate entity. Taking into account that other mesenchymal tissues than the originally described can participate, we propose the alternative term of "neuromesenchymal hamartoma" of the small bowel.

[New perspectives in liver-based metabolic errors: liver transplantation].

Liver transplantation offers an actual alternative to patients end-stage liver disease. The aim of this study is to show our results of liver transplantation in paediatric patients with hepatic-based metabolic disorders. Survival rates in these indications can be high and these good results may enlarge the indications. Liver might transplantation therefore be offered earlier to patients with this kind of metabolic disease.

[Computerized tomography findings in annular pancreas in an adult].

We present a case of annular pancreas in an adult female studied by computerized tomography. CT demonstrated an enlarged pancreatic head in the center of which was visualized the contrast-filled duodenal lumen. We consider these findings to be sufficiently characteristic of annular pancreas to permit differential diagnosis with neoplastic processes of the pancreatic head without having to resort to more invasive diagnostic methods like endoscopic retrograde cholangiography.
