In this prospective clinical study of the early radiation effects on blood supply to the mouth and face, 44 patients (31 men-13 women, mean age 63.4 yrs) with oral tumors underwent surgery and adjuvant postoperative radiotherapy with conventional fractionation (mean dose at the neck approximately equal to 50 Gy). Blood flow parameters 1 cm below the bifurcation of the common carotid artery (mean velocity time averaged, lumen diameter, resistivity index-RI) as well as perivascular reaction, were recorded on Color Doppler Imaging video tapes in a series of five consecutive examinations up to six months postirradiation. There were no statistically significant changes shown between the initial and follow-up examinations for any of the parameters investigated. There was no difference in blood flow between the ipsilateral (operated-irradiated) and contralateral side of the neck. Results did not seem to correlate with known vascular disease risk factors such as sex, arterial pressure, cholesterol levels, smoking and diabetes. After the effect of age was controlled, flow measurements remained statistically stable. Radiation dosage did not appear to influence carotid flow parameters. Perivascular reaction had the highest peak immediately postirradiation but regressed with time. This study suggests that therapeutic radiation of the neck at this dose level may not have important effects on the maxillofacial region blood supply for approximately eight months postoperatively; however, these patients should be closely evaluated for symptoms or signs of carotid artery lesions on a long-term basis.
Ectopic lingual thyroid is a rare developmental anomaly, presenting in the adult with obstructive symptoms during respiration, deglutition and speech. The main therapeutic issue after exact diagnosis is the restoration of thyroid function, and surgical intervention, when necessary, has to deal with problems related to the site and function of the ectopic gland. A case of a 25-year-old female with dyspnoea and dysphagia due to a 3 x 4 cm ectopic lingual thyroid, excised by an original peroral approach via a mandibular midline osteotomy, is reported. Our surgical technique is described and the diagnostic and therapeutic problems are reviewed.
A modified technique of anterior maxillary osteotomy is described, based mainly on the Wassmund (buccal aspect) and the Wunderer (palatal aspect) methods. Experience of 148 cases operated on during 15 years is presented.
Restoration of normal motion and function in patients with temporomandibular joint ankylosis, has been a difficult goal to achieve. According to the experience of the authors early mobilization and physiotherapy, without lining the joint with intermediate materials, is effective for treatment of TMJ ankylosis.
We report the experience of the clinical, histologic and radiologic features of a giant cell granuloma of the maxilla of severe growth. The removal of the lesion led to a large defect of the maxilla with oroantral fistula, which was immediately repaired by the use of the temporalis muscle flap. A four year follow-up period revealed no evidence of recurrence as well as excellent aesthetic and functional results. Further diagnostic and therapeutic problems are discussed in relation with recent literature.
A rare case of hemifacial microsomia of the mandible whose etiological entity is delineated from the bad posture of the foetus in the womb during the foetal period, is described. In this case, the combination of orthodontic and maxillofacial treatment, gave satisfactory results as regards the aesthetic problem of the person, the occlusal disturbances and the function, of the masseter and the temporalis-muscles. These functions were checked before, during and after the treatment by means of electromyograms, and a more balanced activity of these muscles could be detected after the combined treatment.
A tubular adenoid cystic carcinoma of the soft palate was studied histochemically using tannic acid-phosphomolybdic acid-Azophloxine (TPAzf), tannic acid-phosphomolybdic acid-Amidoblack (TPA) and tannic acid-phosphomolybdic acid-Acid Blue techniques. At the periphery of some tumor nests a few spindle cells were noted stained intensely red, blue-green and dark blue with TPAzf, TPA and TPAcB respectively. The apical portion or the whole cytoplasm of the cuboidal, eosinophilic cells that lined the central lumina of most neoplastic nests showed a similar tinctorial behavior. On the basis of their location and staining properties, the spindle cells were recognized as myoepithelial. On the other hand, the tinctorial behavior of eosinophilic cells was attributed to the dye binding by a conspicuous terminal bar-terminal web system or by tonofibrils accumulated in their cytoplasm as a result of squamous metaplasia. An inverse relation between the presence of myoepithelial cells at the periphery of tumor nests and the increasing thickness of terminal bar-terminal web system or the appearance of numerous tonofibrils in the eosinophilic cells was also noted.
Melanoma most often develops in the skin; usually at the site of a preexisting nevus. It is quite rare in the oral cavity and the maxilla is the most common location there. It appears that males between 60 and 70 years old are affected more often than females. The etiology is unknown. However the melachromatic nevus and the color of the skin are considered predisposing factors. Based on clinical and histologic criteria it is classified in three categories. Unfortunately the frequency of the occurrence of each category into the mouth separately, is inversely proportional to the prognosis. The 5 year survival rate of intraoral melanoma does not exceed 5-9%. The treatment of melanoma is surgical and comprises radical excision of the lesion and radical neck dissection. Radiotherapy and chemotherapy do not seem to contribute to the treatment. We present our experience of two patients with melanoma of the maxilla. In one case submandibular lymphadenopathy had already been established and a radical neck dissection was performed. In the other case subtotal maxillectomy was performed with intraoral approach.
The fracture of the styloid process is relatively rare. Most times the fracture is not obvious due to the lack of severe injuries, and remains without diagnosis and symptoms. It may be the result of other causes than that of injury in the head and neck region. The presenting case is of interest because of the referred symptoms.
Fractures of the facial skeleton require immediate reduction, in order to avoid functional and esthetic disorders. But in cases where the patient had already a facial deformity such as prognathism, a combination with sagittal splitting osteotomy operation is indicated, for reduction of the fracture and the deformity as well, in one stage. We present two such cases, with satisfactory results.
A case of typical mandibular asymmetry, probably caused by the development of a primordial cyst in the right ramus of the mandible, is presented. The one stage operation performed, which enabled the enucleation of the cyst and the simultaneous correction of the asymmetry, is described and the case discussed.
Myxoma is a benign tumor of the jaws of mesenchymal origin. A case of such a lesion of the mandible is reported, which was treated by peripheral ostectomy through an oblique-buccal incision, with satisfactory functional and aesthetic results.
We present the methodology of the emergency care in maxillofacial injuries, mainly the preservation of vital functions and the control of the general condition of the patient. Definitive care will follow the emergency care measures.
Solitary plasmacytoma of the bone is believed to be manifestation of the spectrum of diseases presented by the term plasma cell neoplasms and the lesion rarely involves the jaws. A case of solitary plasmacytoma of the mandible is presented, the diagnostic problem is posed and the method of treatment by peripheral osteotomy and radiotherapy discussed.
