Surgery of rheumatic mitral stenosis: comparison of different techniques.

OBJECTIVE: The breakthrough of percutaneous transvenous mitral commissurotomy (PTMC) has dramatically changed the indications for the surgical treatment of rheumatic mitral stenosis over the last decade. No recent studies comparing long-term results of PTMC, open mitral commissurotomy (OMC) and mitral valve replacement (MVR) with bileaflet prostheses are available in medical literature. METHODS AND RESULTS: Between January 1991 and December 1997, 313 patients with pure and isolated rheumatic stenosis were treated in our department. One hundred and eleven patients underwent PTMC, 82 OMC and 120 MVR. There was no statistical difference (p>0.05) between the mortality rates of the three groups of patients. No cases of hospital mortality were observed in the patients who underwent PTMC and OMC, whereas two patients (1.6%) died within 30 days after MVR. Seven year actuarial survival results are: 95.41+/-2(SE)% (PTMC), 98.05+/-1% (OMC) and 92.82+/-33% (MVR) (p=NS). Freedom from embolism was 98.78+/-1% in PTMC, 98.78+/-1% in OMC and 92.52+/-2% in MVR (p>0.05); freedom from reoperation was 88.43+/-8% in PTMC, 96.35%+/-2% in OMC and 97.72+/-1% in MVR (p>0.05). The mean NYHA class at the end of follow-up was lower in OMC (1.14+/-0.3) versus PTMC (1.39+/-0.6) and MVR (1.41+/-0.71) (p=0.001). CONCLUSIONS: Even though conservative techniques are the first option to consider in treating mitral valve stenosis, valve replacement with bileaflet prostheses no longer represents a limiting factor to survival and quality of life.

Reduction of coronary flow reserve non-invasively determined by transthoracic Doppler echocardiography as a predictor of left anterior descending coronary artery stenosis.

BACKGROUND: The aim of this study was to evaluate if a reduced coronary flow reserve determined by transthoracic echocardiography alone or combined with contrast agents may represent a predictive index of significant left anterior descending coronary artery (LAD) stenosis. METHODS: Thirty-four patients (mean age 59+/-9 years) undergoing coronary angiography for coronary artery disease were studied. Coronary stenosis was classified (according to visually determined percent narrowing) as severe (> 75%), moderate (40 to 75%) and mild (< 40%). Coronary blood flow velocities were recorded in each patient at baseline and after low-dose dipyridamole administration by use of a 3.5 MHz transducer with a machine equipped with second harmonic capability and nondirectional color Doppler software. Coronary flow reserve was defined as the ratio of hyperemic to basal diastolic peak velocity. RESULTS: Adequate Doppler recordings in the LAD were obtained by transthoracic echocardiography in 26/34 patients (76%); the infusion of Levovist allowed for the visualization of LAD flow in a further 7 patients, with an overall feasibility of 97%. Coronary flow reserve was significantly higher in the group of patients with mild coronary lesions (2.3+/-0.3) than in patients with moderate (1.68+/-0.29, p = 0.0004) or severe (1.49+/-0.39, p = 0.0005) LAD stenosis. CONCLUSIONS: By use of transthoracic echocardiography combined with contrast agents it is possible to visualize blood flow velocities in the LAD and to evaluate coronary flow reserve after dipyridamole infusion with a non-invasive approach. Combined with angiographic findings, this diagnostic approach could be useful in giving additional information to assess the functional significance of a stenotic coronary lesion.

Percutaneous mitral commissurotomy versus open mitral commissurotomy: a comparative study.

OBJECTIVE: Although many studies in medical literature are comparing percutaneous trans-septal mitral commissurotomy (PTMC) and open mitral commissurotomy (OMC), very few long-term comparative follow-ups are available. METHODS: Between January 1991 and December 1997, 193 patients with isolated mitral stenosis were assigned either to PTMC (111 cases) or to OMC (82 cases). PTMC was performed in all cases with Inoue Ballon, OMC was performed with standard techniques. Categorical values were compared by chi square analysis, whereas continuous data were compared by Mann-Whitney test. Univariate survival and event free analysis (Kaplan-Meier+/-SE and log rank) were performed. Recurrent stenosis was classified any mitral valve area (MVA) less than 1.2 cm2 and whenever post-op. echo showed a loss more than 50% of the initial gain. Data were reported as mean+/-SD. Data concerning late echocardiographic assessment were studied with linear and logistic regression analysis. RESULTS: The two groups were homogenous as far preoperative variables as sex, mean age, MVA, echo score and incidence of left atrial thrombosis were concerned. Mean NYHA was preoperatively higher in OMC (2.79+/-0.58) versus PTMC (2.42+/-0.5) (P = 0.001). There was no hospital mortality in both groups. Incidence of hospital complications was similar (4/ 111 after PTMC and 1/82 after OMC; P = 0.3). Seven year survival: 95.41+/-0.02 (PTMC) and 98.05+/-0.01 (OMC) (P = 0.3) and freedom from late complications did not show statistical differences: Embolism 98.78+/-0.01 in PTMC and 98.78+0.01 in OMC (P = 0.8); Recurrent stenosis 71.89+/-0.13 in PTMC versus 82.89+/-0.08 in OMC (P = 0.2); Reoperation 88.43+/-0.08 in PTMC versus 96.25+/-0.02 in OMC (P = 0.4). A larger MVA was found in patients undergone to OMC (2.05+/-0.35) versus PTMC (1.81+/-0.33) (P = 0.001). Furthermore mean NYHA was lower in OMC (1.14+/-0.3) versus PTMC (1.39+/-0.7) (P = 0.001). CONCLUSIONS: Both techniques achieve with a low operative risk and low incidence of complications a good palliation of rheumatic mitral stenosis. Incidence of complications in the follow-up is similar. OMC allows a larger mitral valve area, a better functional recovery and a lower incidence of late mitral regurgitation.

Product and equipment strategies for surgical services.

One of the most difficult challenges a materiel manager can face is designing different programs to support materiel management services for specific clinical and ancillary areas of the hospital. While basic service standards for purchasing and logistic services should exist for all areas, a large part of how effective a materiel management department is depends on customized program development. In other words, fine tuning services to major customers, such as the Operating Room, can mean the difference between simply filling supply orders and aggressive cost control. This article reviews how Baystate Medical Center (Springfield, MA) maintains consistency in standardizing materiel services while adapting to the requirements of individual departments, such as Surgical Services.

Mutations within the Ddc promoter alter its neuron-specific pattern of expression.

The Drosophila dopa decarboxylase gene (Ddc) is expressed in a reproducible set of approximately 150 neurons, and in a subset of the glia of the third instar larva's central nervous system (CNS). Expression in this pattern requires a cell type-specific neuronal enhancer/glial repressor region located 1000 bp from the transcriptional start site, and specific sequences within the promoter. We have used mutagenesis in vitro and P-element-mediated transformation to examine the role of the promoter, particularly its major CNS activator sequence (element I), in the generation of the wildtype expression pattern. Immunohistological analysis of these transgenic strains demonstrates that particular deletion mutations shift the site of transgene expression to a set of neurons which do not express Ddc at detectable levels in wild-type larvae. Transgene expression in these strains may be driven by a previously undetected activator sequence. Our data also suggest that glial expression may be driven by the same activator sequences that drive expression in the hypoderm.

Interference of prenatal and postnatal exposure to Ca2+-antagonist agents on rat functional development of vascular system.

Exposure to drugs during pregnancy can alter functional development of the vascular system. The present investigation was carried out in order to evaluate the effects of prenatal and postnatal exposure to Ca2+-antagonist (diltiazem, verapamil, and nimodipine) drugs on the development of rat vasomotor reactivity. Studies were carried out on pregnant female albino rats exposed from the first day of pregnancy until weaning to diltiazem and verapamil (6 and 24 mg/kg in their drinking water ad libitum) and nimodipine (3 and 12 mg/kg in their food ad libitum). After weaning, pups were exposed until the 60th day of age to the same treatment as their mothers were. Afterwards, pups from the 60th to 90th day of age were fed with a normal diet. In 30-, 60-, and 90-day-old conscious and anaesthetized pups, we evaluated the following: 1) systolic arterial blood pressure; 2) vasomotor responses elicited by various agents: L-noradrenaline (0.1, 1, and 5 micrograms/kg IV), L-isoprenaline (0.01, 0.1, and 1 micrograms/kg IV), and acetylcholine (0.01, 0.1, and 1 micrograms/kg IV) and by sinus-carotid baroreceptor stimulation; and 3) catecholamine, acetylcholinesterase, and adenosinase plasma levels. Prenatal and postnatal exposure to Ca2+-antagonist drugs significantly (P less than .05) decreased the pressor response to sinus-carotid baroreceptor stimulation and to L-noradrenaline and increased the hypotensive responses to L-isoprenaline and acetylcholine. Moreover, this type of treatment, although it induced a significant (P less than .05) decrease of catecholamine plasma levels, did not modify the acetylcholinesterase and adenosinase plasma levels in 30- and 60-day-old rats. On the 90th day of age, the evaluated parameters were not different from those of control rats. Our results showed that exposure to Ca2+ antagonists during pregnancy and the postnatal period may alter the functional development of rat vasomotor reactivity.

[Controlled clinical evaluation of 2 hypnotic triazole benzodiazepines, estazolam and triazolam, used the night before surgical interventions].

In order to study the efficacy of estazolam versus triazolam in preoperative patients, two groups of 49 patients each were treated with 2 mg estazolam and 0.5 mg triazolam in a single dose, the night before surgery. This controlled study was designed to evaluate sleep patterns, emotional status on awakening and side effects, by the administration of a questionnaire either in the morning of the day before (baseline) or on the morning of surgery. The results show better sleep patterns after estazolam excepting the duration of sleep that was the same for the two drugs. The better emotional status upon awakening in the morning before surgery and the lower incidence of side effects in the patients treated with estazolam, lead to the conclusion that estazolam is clearly better than triazolam in the treatment of preoperative patients.

[Interesting changes in blood fibrinolysis following administration of acetylsalicylic acid in vascular diseases of the lower limbs].

After reference to the results of investigations reported in literature with regard to the effects of ingestion of acetylsalicylic acid on the blood biochemism in normal and pathological conditions, the Authors illustrate the results of their research on patients with chronic obstructive arteriopathy or with post-phlebitic syndrome of the lower limbs. An accurate evaluation was made of the variations induced in some important blood coagulation parameters by the said drug, which was found to cause a clear increase in fibrinolytic activity and which is moreover endowed with marked antithrombophilic therapeutic capacity.
