Physiologic implications of adding small amounts of carbon dioxide to the gas mixture during inhalation of xenon.

In addition to being a physiologically active tracer of CBF, xenon (Xe) in subanesthetic concentrations produces a relatively mild lowering of carbon dioxide (CO2) in the blood and elevation of transcranial Doppler (TCD) velocity. The addition of small concentrations of CO2 (0.4-1.2%) to the inhaled mixture produced no measurable effect on end tidal (P(et)) CO2 or TCD velocity. Cerebral blood flow (CBF) alterations induced by Xe are minimized by allowing P(et)CO2 to fall, permitting quantitative measurement of CBF by the Xe/CT CBF method.

Effects of pinealectomy on wool growth and wool follicle density in merino sheep.

There is evidence to indicate that pinealectomy may enhance wool growth in the sheep. The aim of this study was to determine the effect of pinealectomy on wool growth and wool follicle density in Merino sheep. Castrated Merino rams (4 months old) were either pinealectomized (P), sham-pinealectomized (S) or not treated (C). Wool growth on mid-side patches was measured every 4 weeks and follicle density was monitored in skin biopsies collected before treatment and at regular intervals for 60 weeks. Venous blood samples were taken on each of these occasions for prolactin analysis. Melatonin concentrations were determined in venous blood collected pre- and posttreatment from samples taken over a 24-hr period during the winter solstice. Pre- and posttreatment plasma melatonin levels (mean +/- SEM) 65 +/- 17 and < 13 pg/ml for P, 86 +/- 21 and 69 +/- 20 pg/ml for S, and 94 +/- 41 and 122 +/- 37 pg/ml for C, respectively, indicated that the pineal glands had been successfully removed. Wool growth, total follicle density and liveweight (mean +/- SEM) did not differ between treatment groups. Measurements at week 60 were 3.9 +/- 0.3, 4.1 +/- 0.2, and 3.8 +/- 0.3 gm clean wool/100 cm2; 66 +/- 6, 69 +/- 7, and 64 +/- 3 follicles/mm2; and 50.4 +/- 1.3, 50.8 +/- 1.4, and 52.6 +/- 1.1 kg liveweight for groups P, S, and C, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

CBF measured by Xe-CT: approach to analysis and normal values.

Normal reference values and a practical approach to CBF analysis are needed for routine clinical analysis and interpretation of xenon-enhanced computed tomography (CT) CBF studies. We measured CBF in 67 normal individuals with the GE 9800 CT scanner adapted for CBF imaging with stable Xe. CBF values for vascular territories were systematically analyzed using the clustering of contiguous 2-cm circular regions of interest (ROIs) placed within the cortical mantle and basal ganglia. Mixed cortical flows averaged 51 +/- 10ml.100g-1.min-1. High and low flow compartments, sampled by placing 5-mm circular ROIs in regions containing the highest and lowest flow values in each hemisphere, averaged 84 +/- 14 and 20 +/- 5 ml.100 g-1.min-1, respectively. Mixed cortical flow values as well as values within the high flow compartment demonstrated significant decline with age; however, there were no significant age-related changes in the low flow compartment. The clustering of systematically placed cortical and subcortical ROIs has provided a normative data base for Xe-CT CBF and a flexible and uncomplicated method for the analysis of CBF maps generated by Xe-enhanced CT.

Effects of cimetidine and ranitidine on basal gastric pH, free and total acid content in pigs.

The effects of intramuscular administration of multiple doses of cimetidine and ranitidine on basal gastric pH, free and total acid content from young adult pigs were studied. Cimetidine (4.5 mg kg-1, four times a day, intramuscularly, for three days) significantly (P less than 0.05) raised the basal gastric pH above 3.5 with a simultaneous reduction in free acid content at two, three and 26 hours after the administration of the eighth dose. Ranitidine (0.75 mg kg-1, four times a day, intramuscularly, for three days) significantly (P less than 0.05) raised the basal gastric pH above 3.5 with a concomitant reduction in free acid content at three and 38 hours after the administration of the eight dose. Neither cimetidine nor ranitidine had any significant effects on total acid content. These results confirm that the pig is a basal acid secretor and that the pharmacodynamics and pharmacokinetics of cimetidine and ranitidine in pigs might be different from those in humans.

Clinical trials in hypertension: some general thoughts and some particular controversies.

A common fallacy in the analysis of clinical trials is described. Correlating the differences between pairs of random numbers and the first of them produces a correlation coefficient of 0.71 with a regression equation of y = -4.5 + X. Misuses of correlation and regression in hypertension and hypolipidaemic trials are described and advice given. The author also puts on record his objection to the pedantry that surrounds the use of the analysis by intentions-to-treat method. Patients who go into such an analysis may include those who never received a single dose of the treatment to which they were randomized, those who started it but were later switched to the other treatment though still analysed in their original group, patients who could be regarded as having died from obviously nontreatment causes by any sensible and reasonable doctor, and patients later found even to have suffered the wrong illness.

The clinical and immunologic responses of normal human volunteers to low dose hookworm (Necator americanus) infection.

Five normal human volunteers were exposed to approximately 50 infective larvae of Necator americanus and were observed for the development of clinical signs or symptoms and for changes in blood eosinophil levels, IgG antibody titers, total and parasite-specific IgE, and lymphocyte blastogenic responses for 6-10 weeks. Bronchoalveolar lavage was performed on four subjects prior to infection and at times when larval migration through the pulmonary tree was likely. Eggs were demonstrated in the stools of four volunteers who remained untreated for more than 6 weeks; one volunteer had to be treated at day 40 because of severe gastrointestinal symptoms. All others also complained of abdominal pain and flatulence between days 35-40. All volunteers developed marked blood eosinophilia which peaked between days 38-64 and ranged from 1,350-3,828 eosinophils/mm3. Small increases in total and parasite-specific IgE and IgG were noted in some volunteers. One volunteer showed a significant lymphocyte blastogenic response. With the exception of mucosal erythema, bronchoalveolar lavage results were unremarkable. Our data indicate that a single small inoculum of hookworm larvae is capable of producing significant transient gastrointestinal morbidity and marked blood eosinophilia but does not induce other prominent T cell- and B cell-dependent immune responses.

Assimilation of cholesterol by Lactobacillus acidophilus.

Considerable variation was found among strains of Lactobacillus acidophilus isolated from the fecal flora of pigs with regard to the ability to grow well in the presence of bile and to assimilate cholesterol from a laboratory growth medium. The uptake of cholesterol occurred only when the culture(s) was growing in the presence of bile under anaerobic conditions. Consumption of L. acidophilus RP32, which was selected for its ability to grow well in the presence of bile and to assimilate cholesterol from the laboratory medium, significantly inhibited increases in serum cholesterol levels of pigs (P less than 0.05) fed a high-cholesterol diet. Consumption of L. acidophilus P47, which was selected for its ability to grow in the presence of bile and lack of ability to remove cholesterol from the growth medium, failed to have a similar effect. This indicates that certain strains of L. acidophilus act directly on cholesterol in the gastrointestinal tract, and thus may be beneficial in reducing serum cholesterol levels.

Sensitivity and accuracy of the visual analogue scale: a psycho-physical classroom experiment.

1 Twenty-seven intelligent volunteers took part in a classroom experiment on two occasions to assess subjectively, ordinally related volumes of sound (offered in random sequence), using the visual analogue scale. 2 It was simple to use and largely acceptable. Big differences were significant by parametric tests but small ones were sometimes significant only with ordinal ones. Within-subject comparisons were more accurate and more sensitive than those between-subjects. The t-test was very robust. 3 Five out of 49 results were erroneously significant; they remained so no matter how the data were handled. It was concluded that this was due to a shift in either perception, cognition or scoring between the two sessions. 4 Arcsine transformations made little difference. The mechanism of these is fully discussed. A conversion to proportional scores resulted in very much improved sensitivity. 5 It is recommended that authors using the visual analogue scale: have valid reason in their own setting for using a transformation; present the distribution, or at least the medians and ranges of their raw scores; also use a simple but different measure so as to demonstrate internal consistency between them.

A novel adaptation of the information game for the development of a new drug.

Medical advisers in the pharmaceutical industry have an important function and place in the development and evaluation of new drugs. Although mainly involved in the human and clinical investigations, it is essential that they have the background knowledge and experience to evaluate the results of pre-clinical studies and to apply these safely and effectively to the clinical situation. It is rare nowadays for a medical adviser to obtain direct experience in all areas of drug research due to its complexity. Simulation exercises are a means of obtaining this experience without suffering the results of a wrong decision. An adaptation of the Information Game proved to be a useful and acceptable means of illustrating the decisions that need to be made in a drug development programme and at the same time provided experience in coping with the various problems that might arise in such a programme. The training objectives of the game should be clearly defined at the outset in order to concentrate on the parts of the game that really matter and cover the maximum number of teaching opportunities.

Prospective cerebral MR study of HIV seropositive and seronegative men: correlation of MR findings with neurologic, neuropsychologic, and cerebrospinal fluid analysis.

PURPOSE: As part of a longitudinal study of human immunodeficiency virus type 1 (HIV) infection, we attempted to identify early cerebral MR findings that might correlate to clinical evidence of central nervous system involvement. METHODS: We studied 65 seropositive and 40 seronegative homosexual males using cranial MR, neurologic, immunologic, and neuropsychologic examinations. RESULTS: The incidence of mildly enlarged ventricles, sulci, and punctate areas of abnormal signal in both groups was similar in both groups. Diffuse, poorly defined areas of abnormal white matter signal were difficult to consistently identify in seropositives. Enlarged adenoidal lymphoid tissue was found in 30 (46%) of seropositives and 2 (5%) of seronegatives (P = .0001). The incidence of sinus inflammatory change was similar in the two groups. CONCLUSION: MR of intracranial contents is substantially normal in a non-AIDS HIV(+) population.

Immune profile of women experiencing recurrent spontaneous abortions of unknown aetiology.

Immunological and immunogenetic investigations are described in a group of 28 women with a history of unexplained recurrent (greater than or equal to 3 consecutive) spontaneous abortions (RSA). Compared with female blood donor controls, recurrent aborters had significantly elevated mean serum complement C4 (p less than 0.05), but not C3, levels. Immunoglobulin levels and T4:T8 ratios showed a similar distribution to the controls. Mixed lymphocyte reactivities to partners' lymphocytes were low in some of the patients, but hyporesponsiveness correlated well with husband-wife HLA-DR compatibility. No individual HLA specificity was significantly increased in the RSA patients, but the combination A3, B7, DR2 was common (p less than 0.02), and general HLA antigen sharing with partners was significantly increased (p less than 0.004) relative to obstetrically normal couples. RSA women who shared two or more HLA antigens with spouses had higher mean serum immunoglobulin concentrations, which for IgM reached statistical significance (p less than 0.05).
