[Spondylolisthesis in athletes].

Isthmic spondylolysis is a fatigue fracture which mostly affects athletes. It is acquired between the ages of 5 and 15 years and is directly related to the type of sport practised. Ethnic and racial factors are also involved. It is of interest to identify the first symptoms of isthmic damage at the initial stage. The practice of sports must be interrupted to facilitate isthmic repair. Sporting activities can be resumed, but the initial therapeutic procedure (hyperlordosis) must be modified. Once spondylolysis has been constituted, few sports are contra-indicated in adolescents. Major displacements towards spondyloptosis are rare, but surveillance is necessary. Isthmic spondylolysis is usually well tolerated by adults.

Symptomatic spinal epidural lipomatosis induced by a long-term steroid treatment. Review of the literature and report of two additional cases.

Spinal epidural lipomatosis associated with Cushing's syndrome is an uncommon complication (11 reported cases). Two additional symptomatic cases with neurologic deficit are described. Steroid treatment was systemic in the first case and local with epidural injections in the second. The second case is unique because no similar observations have yet been reported. In most cases, a preoperative computed tomographic scan establishes the diagnosis by demonstrating dural compression by an adipose mass. Myelography is far less specific. In some cases, the exact diagnosis is made at the time of surgery. The treatment is primarily surgical, with laminectomy over the length of the compression and the removal of the compressing fat. Neurologic recovery is dependent on two factors: the level of the compression and the adequacy of decompression.

[Comparative study of fine needle aspiration and large caliber needle biopsy under echographic control for the diagnosis of abdominal tumors].

The aim of this study was to compare the results of fine needle aspiration (22 G) with large caliber needle biopsy (18 G) in patients suspected of having abdominal malignancy. From November 1988 to December 1989, 73 patients aged 32 to 78 years (mean 66 years), suspected sonographically of having hepatic malignancy (66 cases) or extrahepatic tumors (7 cases), underwent ultrasonography guided percutaneous fine needle aspiration (22 G) and biopsy with a large caliber needle (18 G). There were no complications. Overall sensitivity for diagnosis of malignancy was significantly better (p less than 0.01) with 18 G needle biopsies (91.2 percent) than with fine needle cytology (71.4 percent). Specificity was 100 percent. The distinction between primary and secondary malignant tumor was possible in 87.5 percent with the large caliber needle and in only 42.9 percent with the fine needle (p less than 0.001). Histological findings were adequate hepatic metastases in 86.5 percent with the 18 gauge needle and in 51.4 percent with the 22 gauge needle (p less than 0.001). In the 7 extra-hepatic tumors, the 18 G biopsy was always positive whereas fine needle cytology was positive in only 5 out of 7 cases. Correct diagnosis for benign diseases was possible in 7 out of 10 cases with the large caliber needle whereas the fine needle cytology was always negative. These results show the superiority of large caliber needle (18 G) compared to fine needle (22 G) guided punction for both diagnosis of malignancy and origin of the tumor. Complications did not occur more frequently than with fine caliber needles.

[Carcinoma in situ in localized Caroli's disease].

A new case of Caroli's disease associated with cholangiocarcinoma is reported. It is the first in situ carcinoma description. Intrahepatic biliary cysts are a predisposing condition, even if mechanism remains hypothetical.

[Restatement and conception of an experimental battery of measurements of displacements in the three planes of a vertebral unit].

The study of motions of a vertebral unit is difficult because of the great number of participating structures. Analysis in the three planes is required to obtain optimal visualization of the entire spectrum of criteria involved. Initially, we used comparators to achieve these measurements. However, the complexity and number of parameters quickly led us to switch to electronic sensors. At present, a completely automated test bench has been developed using an Orion data processing chain. Decoding and interpretating results nevertheless remains difficult in such tridimensional analyses.

The surgical treatment of post-traumatic vertebral deformities.

The authors report their experience in the treatment of malunited fractures of the spine. After eliminating reducible lesions, the methods of treatment used for fixed irreducible lesions are described. In the cervical spine, the approach is anterior, and includes corporectomy to decompress the medullary canal. It is often impossible to restore cervical lordosis but this is not essential. In the dorsal, dorsolumbar and lumbar spines, experience has demonstrated the inadequacy of isolated anterior and posterior surgical approaches, so a combined approach is proposed aimed at achieving good results, both neurological and mechanical. Reconstruction of the vertebral body is carried out by anterior grafting using bank bone from femoral heads, while fixation is achieved posteriorly by the use of Roy-Camille plates. Indications are carefully considered, taking into account that progression is always a possibility in patients with no neurological deficit. The results are analysed in 14 cervical lesions and 10 dorsolumbar lesions. This operative treatment frequently produced stabilisation as well as the regression of pyramidal signs, but recovery of medullary lesions was never observed. The relief of pain, however, was a constant finding.

[Fractures of the talus in children. Apropos of 23 cases].

The authors have reviewed 23 fractures of the talus in children. They propose a classification based on the radiological maturity of the bone and the specific type of fracture. Group I: Children younger than 6 years (6 cases). The fracture line separates the bone into two parts. The diagnosis was difficult, with displacement being underestimated in some cases. Associated lesions in the same limb were present in 5 cases. Five fractures of the neck of the talus were treated conservatively with excellent or fair results. One comminuted fracture was treated surgically with a poor result. In all cases treatment should aim to maintain a good tibio-tarsal joint. Group II: Children older than 6 years (17 cases). Eight cases were osteochondral fractures, two were partial fractures and seven were complete fractures. Two of these developed subsequent avascular necrosis. The relationship between osteochondral fractures and osteochondritis is discussed.

Plating of thoracic, thoracolumbar, and lumbar injuries with pedicle screw plates.

This article describes a technique of plating thoracic, thoracolumbar, and lumbar injuries with pedicle screw plates. The shape of the plates is premolded to the spine curvature. The strength of the pedicles where the screws are implanted explains the good results of reduction and stabilization. This technique allows the surgeon either to reduce and fix a spinal fracture or to perform large approaches and extensive procedures when necessary.

Internal fixation of the lumbar spine with pedicle screw plating.

The results and complications of pedicle screw plate (PSP) stabilizations were investigated in lumbar fresh fractures, malunions, lumbar metastases, primary tumors, lumbosacral fusions, and highgrade spondylolisthesis. The low incidence of neurologic complications of PSP is noteworthy, and a reason for recommending the method.

Transverse fracture of the upper sacrum. Suicidal jumper's fracture.

Thirteen patients with transverse fractures of the upper sacrum were studied to determine fracture anatomy, clinical presentation, and therapeutic approaches. The injury results from falls from a height and is usually associated with suicidal attempts by jumping. The position of the lumbar spine in lordosis or kyphosis at the time of impact determines which of three types of morbid anatomy will result. Because of associated polytrauma, fracture of the upper sacrum is often not recognized in the acute stage, and awareness of the possibility of such injury, especially in the presence of perineal neurologic deficit should result in securing good quality radiographic study, including tomograms. Surgical treatment is often required.

[Surgical treatment of spinal metastases by stabilization using posterior plates screwed into the vertebral pedicles].

Ninety spinal metastases were treated by plating using a posterior approach. Fifty were thoracic or thoraco-lumbar, 11 in the upper cervical spine, 14 in the lower cervical spine and fifteen in the lumbar spine. Surgical fixation was followed by radiotherapy, chemotherapy or hormone therapy, when indicated. In the upper cervical region a special plate was screwed into the occiput. In the lower cervical spine fixation was accompanied by anterior vertebrectomy at the same procedure. In the entire series good stability was obtained in nine out of ten cases. Mortality during the first post-operative months was about 10 to 15 p. 100 dependent on the level. Two thirds of the patients gained some benefit from the operation. Cases with paraplegia and loss of independence had less favourable results than others. The ideal indications are threatened collapse of vertebrae, recent neurological impairment of severe pain resistant to conservative treatment.

[Epiphyseal fixation with an external fixator. Biomechanical and clinical study].

Fifty one fractures of an epiphysis were treated by fixation using a new external fixator. The stability of the fixation was compared with that obtained by other types of fixator. The basis of the technique was the insertion of five pins into the epiphysis and of a series of pins in the diaphysis. Fixation between the epiphyseal and diaphyseal pins was achieved by three bars. The results obtained in 38 cases, half of them being at the proximal end of the tibia were analysed and were found to be encouraging. Only three had to be grafted secondarily. There was a low number of mal-unions. The main advantage of this method was to allow early mobilisation of the joint. The risk of sepsis did not give rise to concern provided that the rods do not cross the joint.
