The efficacy and safety of ursodeoxycholic acid in the treatment of intrahepatic cholestasis of pregnancy was investigated in an open pilot study. Five patients received 1 gm/day of ursodeoxycholic acid during 20 days and another three patients received two identical periods of treatment separated by a 14-day interval free of the drug. Pruritus and serum levels of total bile salts and glutamic-pyruvic transaminase improved significantly during treatment with ursodeoxycholic acid. In the three patients who received two periods of treatment with ursodeoxycholic acid, pruritus and the laboratory alterations relapsed in the second week after the drug was discontinued, but they improved again when ursodeoxycholic acid was readministered. No adverse reactions were detected in the mothers or in their babies. All newborns were thriving normally during a follow-up period that lasted 5 mo after delivery. It is concluded that UDCA appears to be safe when administered in late pregnancy; its promising efficacy in the treatment of intrahepatic cholestasis of pregnancy should now be confirmed in controlled clinical trials.
A study was made of serum concentrations of cholesterol, phospholipids, triglycerides, cholesterol bound to high-density lipoproteins (HDL), HDL1-cholesterol, HDL2-cholesterol and cholesterol bound to low-density lipoproteins (LDL) in 16 dogs naturally infected with Leishmania infantum (ZMON-1) taken from an endemic focus. Results were compared with those of a control group of ten healthy dogs. Statistically significant increases in cholesterol, triglyceride and LDL-cholesterol levels were observed. There was, however, a statistically significant decrease in HDL-cholesterol level, mainly at the expense of the HDL2-cholesterol subfraction. Cholesterol transport is therefore shown to undergo changes which may be attributed to the consumptive evolution of the disease, immunocomplex deposits in cells, hepatic disorders and interactions between the parasite and the normal cholesterol metabolism of the host.

Epomediol is a terpenoid that prevents and reverses cholestasis induced by ethinylestradiol in the rat, apparently by improving liver cell membrane fluidity. Assuming that the pathogenesis of intrahepatic cholestasis of pregnancy (ICP) is related with increased estrogen levels, we studied the effects of epomediol in this disease. Patients hospitalized due to ICP received epomediol 900 mg/day (n = 7), or 1,200 mg/day (n = 4) orally, during 15 days. Biochemical parameters of liver dysfunction (serum bilirubin, bile salts, aminotransferase, alkaline phosphatases) were not modified during nor after epomediol administration. The severity of pruritus was significantly reduced in comparison to pretreatment status, with both doses of epomediol. A greater amelioration of pruritus was observed in patients treated with epomediol 1,200 mg/day than in patients who received 900 mg/day (to 20.7 +/- 6.2, as percent of pre-treatment severity score, versus 48.8 +/- 7.5 respectively; p < 0.05). After epomediol administration was stopped, pruritus relapsed in 6 patients; 3 of them had received the higher drug dose. After delivery, pruritus vanished and liver function tests returned to normal, in all patients. No adverse effects attributable to the drug were observed in the mothers or in their babies. The beneficial effect of epomediol on pruritus in patients with ICP appeared greater in this study than that observed recently in similar patients who received a placebo.

Broncho-alveolar lavages from 13 control subjects have been studied for the following parameters : phospholipids, free fatty acids, total last parameter, through the depolarisation of Diphenylhexatriene, is independent of the recovered volume. An identical study has been performed for two bronchiolo-alveolar cancers. A treatment with hydrocortisone-containing aerosol has been associated with an increase in phospholipid amount and a drastic decrease in microviscosity. This observation suggests that the synthesis of tensio-active phospholipids is dependant of corticoids by an enzymatic induction. The effect of therapeutics given in aerosol can be monitored by the biochemical and physical modification in broncho-alveolar fluids thus obtained.
This study concern 61 asbestos workers, exposed to very dusty conditions, but exposure is stopped for 4 years. Flow-volume curve shows a decrease in flow, at low-volume, particularly in old subjects, with a great dust index. In 13 subjects (21%), with normal chest radiographs, we observe a decrease in flow. This is interpreted as an airway obstruction, and it is an early effect of asbestos exposure. This obstruction concerns the small airways because the Vmax is density independent in the last portion of the VC. Flow-volume curve breathing helium oxygen mixture is not more sensitive, than breathing air. So, this test seems sufficient, to monitor the exposed workers, in epidemiological studies, and prevent repeated chest X-ray examinations which may be dangerous.
The authors having reported all the possible denominations of syndromes of respiratory distress in the adult mention again the main etiologies, stressing on the last ones. They recall afterwards the different clinical, radiological and biological stages of this syndrome. The hemodynamic profile is defined by normal catheterism data and the presence of a true anatomical shunt. On the other hand several hemodynamic variations are possible. The differential diagnosis is essentially cardiac edema. Finally the authors state that treatment is done only by symptomatic therapies: dehydration and assisted ventilation. They do not make use of membrane oxygenator. Assisted ventilation is essential and is mainly based on a good indication and the optimal use of permanent positive pressure.
Morphology--by growth phases-- was studied in the N. asteroides strain isolated from cows suffering from mastitis in Cuba. During the growth N. asteroides culture underwent a number of morphological cyclic changes; definite morphology was typical for each growth phase. There was a similarity between the structure of the surface N. asteroides and mycobacteria; cyclic changes in the structure of the membranes in nocardia were also shown.
On the basis of diagnostic criteria (history of exposure, airway and general symptoms, detection of antibodies, disturbance of pulmonary function, chest X-ray, allergen inhalation test) a definition of extrinsic allergic alveolitis is proposed. The air is better comparability of case reports, therapy recommendations a. o.
The authors report their experience of 100 transbronchial pulmonary biopsies done during fibroscopy. The technique is easy; patients with respiratory insufficiency or bullous lesions are excluded. Biopsy is done blindly, without any brillance amplifier. A hundred biopsies were performed (34 in limited opacities, 66 in diffuse images). Pulmonary parenchyma was brought out in 77% of cases and a histological diagnosis was made in 52% of cases. Two pneumothorax and 5 minor hemoptysis occured as sequellae of these biopsies. This method of study is interesting mainly for alveolar or interstitial diffuse pneumopathies. In such cases, it represents a real improvement over the bronchial biopsy, often avoiding the need for a surgical biopsy.
The authors report 3 observations of cervico-facial lipomatosis with mediastinal localization. In the 3 cases the rather unusual localization of Launois and Bensaude's disease produced respiratory troubles which worsened a chronic respiratory insufficiency due to another cause. Once a tracheotomy had to be done because of tracheo-malacia. Another time there was a pharynged localization. This disease etiology always remains obscure, usually appearing in alcoholic addicts of about 50. Mediastinal localizations have been unfrequently described but must be searched for systematically. The evolution is chronic, without specific treatment. If there is already a chronic respiratory insufficiency in relation with a chronic obstructive bronchopneumopathy or a cardiac insufficiency, then the mediastinal localization becomes an aggravating factor.
Two series of work are reported. The first concerns the pharmacocinetic study of the product after it was administered through different ways and the corresponding evolution of PaCO2. The second study concerns the effect on pulmonary arterial pressure, blood gases and some parameters of ventilatory mechanics, of a perfusion of 200 mg of this product over a two hours period.
Starting again a Rifampicin treatment gave rise to an acute renal insufficiency, reversible by hemodialysis. This incident resulted from an immunopathological conflict revealed by the presence of anti-Rifampicin antibodies. Renal needle biopsy did not show any immune deposits. The mechanism of these incidents is discussed. Authors draw attention to the necessity of avoiding discontinuous Rifampicin treatment.
