[Clinical features of respiratory tuberculosis and diagnosis delay].

OBJECTIVE: To make a descriptive analysis of the clinical characteristics of patients with pulmonary tuberculosis and find the time of diagnostic delay. DESIGN: Retrospective study. SETTING: Autonomous Community of Navarre. PATIENTS: 317 cases of pulmonary tuberculosis diagnosed between January 1 1993 and June 30 1996. RESULTS: The most commonly observed symptom was coughing (76.8%), followed by expectoration (63.8%) and high temperature (60.3%). The high percentages of high temperature, asthenia, anorexia and weight loss in HIV-infected patients stood out. The mean period of diagnostic delay was 36.9 to 58.5 days, with a median of 15 days. For 76.8% of patients the delay in diagnosis was under a month. CONCLUSIONS: Early diagnosis is essential in tuberculosis control, especially in cases transmitted by bacilli, since these are highly contagious for months.

[Infant mortality and sudden infant death in Navarra between 1985 and 1996].

OBJECTIVE: The objective of this study was to obtain an accurate register of the deaths of children less than one year of age in order to discover the real infant mortality (IM) rate in Navara and its trend during the period between 1985 and 1996, a well as to investigate the deaths by sudden infant death (SID) in the same period and their relationship to the prone position during sleep. PATIENTS AND METHODS: Infant mortality was determined by detailed examination of all certified deaths. Research into possible nonregistered deaths was also carried-out. The sources of information included the archives and registers of the pediatric and obstetric wards and pathology departments of all hospitals. Deaths codified as 798.0 (CIE9 infinity R) were included in the study as death by SID. To determine sleeping posture. In the second semester of 1996, a survey was carried out on a sample of 285 children of under 6 months of age. The survey's model is the same as that published previously. RESULTS: In the period of 1985 to 1992, IM was 12.33 per thousand live births. The SID rate was 1.30/1,000 live births. The prevalence of the prone position is unknown. In the period between 1993 and 1996, IM decreased to 5.84/1,000 live births and this decrease was proportional in both the neonatal and postneonatal components. The rate of SIM decreased to 0.44/1,000 live births. At the beginning of 1993, 86% of the children slept in the prone position, in 1994, 38% and in 1996, only 23%. CONCLUSIONS: Trustworthy data in Navarra shows a decrease in IM of more than 50% from 1993 on. The SID rate also decreased by 65%, coinciding with a massive drop in the use of the prone position during sleep.

[Evaluation of program coverage of the hepatitis B vaccination in a school population. Navarra 1992-1993].

OBJECTIVE: To evaluate the coverage of the programme of control and coverage of Hepatitis B among children doing their 7th year of EGB (basic) in Navarra. DESIGN: Descriptive study. SETTING: School centres in Navarra. PARTICIPANTS: 7th-year EGB students in the 1992-3 school year. INTERVENTION: Vaccination of the susceptible population with three doses of anti-HB vaccine on the 0-1-6 months pattern. MEASUREMENTS AND MAIN RESULTS: Nominal data of children vaccinated were obtained from 171 schools out of a total of 185 (92.43%). For the rest of the centres, it is known only that vaccinations took place within the School Health Programme. The susceptible population was 6,955 children, of whom 97.8% received the first dose, 97% the second and 95.6% the third. The most common cause of non-vaccination in these children was lack of parental authorization, which reached 1.8%. CONCLUSIONS: A School Health programme, which is well-established and closely linked to the Primary Care network, together with a broad diffusion of information about the adolescent Vaccination Programme were key to its widespread introduction and the excellent coverage attained.

[Inequality before death: a comparative study between Navarra communities in the population segment from 25 to 74 years old].

OBJECTIVE: To identify differences in mortality between communities in function of certain socio-economic factors. Firstly, between the urban and rural population of Navarra; and secondly, among different neighbourhoods in Pamplona categorised according to their socioeconomic status. DESIGN: Descriptive study. As the instrument of comparison, the standard mortality rates (SMR) have been used with the direct method. The relationship between mortality and socio-economic status in the neighbourhoods of Pamplona has been analysed by means of lineal regression. SITE. Institute of Public Health of Navarra. PATIENTS OR OTHERS PARTICIPANTS: Those people recorded on the Municipal Censuses of Navarra in 1986, who were between 25 and 74 years old. 8,086 of these died during our period of study, which covered the five years between 1985 and 1989. MAIN MEASUREMENTS AND RESULTS: The SMRs for all causes in men (741 x 10(6)) and women (324 x 10(6)) who live in rural communities are significantly lower (p less than 0.05) than those in urban communities (863 and 390). The specific cause of death which marks the most difference between the two types of community are tumours (p less than 0.05). In the capital (Pamplona), masculine mortality was found to increase in line with the socio-economic status of the neighbourhood lived in (p = 0.017). CONCLUSIONS: There is greater mortality in urban than in rural communities; and in neighbourhoods with low socio-economic status as against those with a high status. However, in this second case, the greater mortality only applies to men.

[The validity of infant mortality statistics in Navarra].

In order to explore the validity of official infant mortality statistics, the number of infant deaths registered in the cohort of live births and resident in Navarra (Spain) from 1985 to 1989, according to data supplied by the Department of Statistics of Navarra, has been compared with that obtained in a retrospective search into the information of the hospitals. Among 24,383 live births of the cohort, 291 were certified as infant deaths. An over-register of 13 cases (4.5%) and an under-register of 26 cases (8.9%) was detected. By excluding those live births with less than six months of gestation, as done by the National Institute of Statistics, the validity is altered, because apart from the errors mentioned, one must add those resulting from the incorrect filling-in of the "number of weeks pregnant"= variable. When correcting these deficiencies, the final statistics (Infant Mortality Risk = 11.17 per 1000 live births) did not differ much from the official figures (IMR = 11.48 per 1000 live births).

[Approximation to the prevalence of heroin users in Navarre in 1990].

Intensive multi-agency case finding is a methodology developed to make an estimation of the number and sociodemographic characteristics of known heroin users in a given community. It basically consists of collecting information from services and/or sanitary, social and penal institutions which in the course of their activities contact, or are contacted by persons who are described as heroin users. With the aim of estimating the magnitude of heroin use in Navarra, information about heroin users was requested from public treatment centers for drug-dependences, therapeutic communities, psychiatric emergencies, general hospitals, hepatitis case registry, HIV+ case registry, prisons, forensic and various social services. The study covered the whole Autonomous Community (512,512 residents) during the year of 1990. Overall 1231 persons were counted as heroin users representing a prevalence rate of 2.4 cases/1000 residents. They were mainly male (3 to 1), and a 72% of cases were aged between 20 and 29. The demographic distribution was mainly urban (3.4 to 1) and there were large differences among the different urban zones.

[Magnitude of human immunodeficiency virus (HIV) infection. Navarre, 1992].

The information available from public laboratories in Navarra (Spain) on tests for detecting anti-HIV antibodies enabled to find out the number of HIV seropositive subjects from 1985 to June 1992, as well as their demographic characteristics and risk habits. 1205 subjects were found to be HIV seropositive, out of a population of 512,512 inhabitants which means an accumulated incidence of a 2.35 cases per thousand inhabitants. Higher rates were observed in young males (13.57 per thousand), and in urban areas (3.7 per thousand). The most frequent risk habit among HIV seropositive is the use drugs by intravenous injection (93.7%) followed by sexual transmission (3.5%). This methodology provides the minimum number of HIV seropositive cases in Navarra, since it does not include those not diagnosed, on diagnosed in private centres in Navarra and/or in other autonomous regions.

[Nosocomial and community outbreak of epidemic keratoconjunctivitis in Navarra in 1996].

BACKGROUND: Epidemic keratoconjunctivitis is an acute, generally bilateral infection, that appears sporadically or in outbreaks from a source of common infection which may be a visit to the ophthalmologist or a swimming pool. Keratitis may evolve to form sub-epithelial injuries in the cornea lasting as much as several months. METHODS: Description of an outbreak of epidemic keratoconjunctivitis that took place in Navarra (Spain) between April and August 1996 and which affected at least 266 people. RESULTS: The epidemiological research highlights the existence of two ophthalmologist consulting rooms as the transmission mechanism for the infection in 46 of the 116 patients researched. The spread of the infection in the environment of the patients was recorded in 119 cases, 62 of which are considered secondary cases. The aetiological diagnosis of the outbreak was carried out 11 days after the start of the microbiological research, isolating adenovirus in conjunctival samples of 8 of the 19 patients researched. CONCLUSIONS: The preventive measures taken to control the outbreak and their effectiveness was highlighted by the gradual reduction in cases.
