Prospective study of Streptococcus milleri hepatic abscess.

Thirty-seven cases of microbiologically demonstrated pyogenic hepatic abscess were observed in a prospective study over a seven-year period. Biliary disease was the most common source of liver abscess (42%). Streptococcus milleri was the most common cause of hepatic abscess, accounting for 51% of the cases. Hepatic abscess is due to Streptococcus milleri clinically distinct from other forms of pyogenic liver abscess due to its torpid nature and the longer duration of its symptoms [42 vs. 11 days]. Occult hepatic abscess should be suspected if the blood culture is positive for Streptococcus milleri, since 28% of bacteremia cases due to Streptococcus milleri stem from hepatic abscesses. It is important to distinguish Streptococcus milleri from other members of the viridans streptococci group, which are frequently isolated as contaminants, but only exceptionally cause hepatic abscess. Unlike other pyogenic hepatic abscesses, those caused by Streptococcus milleri are frequently monomicrobial (79%). In the present study, empirical therapy of pyogenic hepatic abscess always included a drug that is effective against Streptococcus milleri.

Increasing uterine receptivity by decreasing estradiol levels during the preimplantation period in high responders with the use of a follicle-stimulating hormone step-down regimen.

OBJECTIVE: To analyze the effect on uterine receptivity of a decrease in E2 levels during the preimplantation period with the use of a step-down regimen in high responders undergoing IVF. DESIGN: Prospective controlled clinical study. SETTING: The Instituto Valenciano de Infertilidad. PATIENT(S): High responders in whom at least one previous IVF attempt failed in which 3-4 good-quality embryos were transferred and E2 levels were >3,000 pg/mL on the day of hCG administration. INTERVENTION(S): Gonadotropins were administered according to two different protocols. Blood samples were collected and IVF was performed. MAIN OUTCOME MEASURE(S): Serum E2 levels and reproductive outcome of IVF. RESULT(S): Estradiol levels on the day of hCG administration and throughout the preimplantation period and the number of oocytes collected were significantly lower with the use of the step-down regimen than during the previous failed cycle in which the standard protocol was used. The fertilization rate was similar and the number of good-quality embryos transferred was comparable. However, the implantation and pregnancy rates were significantly improved in patients who underwent the step-down regimen compared with those who received the standard protocol. CONCLUSION(S): With the use of a step-down regimen with FSH in high responders, our clinical results demonstrate that uterine receptivity can be improved when E2 levels are decreased during the preimplantation period.

Cytokines and embryo implantation.

The implantation process is currently considered the most relevant limiting factor for successful pregnancy. It is evident that molecular interactions at the embryo-maternal interface at the time of implantation are crucial in order to understand the mechanisms of embryonic implantation. The principal aim of this study is to demonstrate the existence of specific communication pathways between the human embryo and endometrium. The molecular interactions appears to be initiated by the endometrium in the presence of an implanting blastocyst and is mediated through embryonic cytokines (specifically the IL-1 system) and the target is the endometrial epithelial beta3 integrin subunit. If the role of beta3 is accepted as a marker of uterine receptivity these results may indicate that the normal hormonally regulated human endometrium could be the trigger for molecular events preparing the blastocyst to communicate effectively and regulate endometrial adhesion molecules in order to implant.

Predominant recognition of species-specific determinants of the GroES homologues from Mycobacterium leprae and M. tuberculosis.

The Mycobacterium leprae and M. tuberculosis 10,000 MW heat-shock protein homologues of GroES have previously been identified as major immunogens for human T cells. We used synthetic peptides to characterize the determinants recognized by murine T cells. The findings suggest that, despite 90% sequence identity between these two proteins, T cells recognize prominently the species-specific determinants localized within amino acid residues 21-40 and 49-72. Analysis of the molecular determinants of species-specificity for the M. leprae GroES sequence 25-40, using T-cell hybridomas and major histocompatibility complex (MHC)-binding assays, led to the identification of epitope cores and critical residues. Interestingly, closely overlapping epitope cores were found to be restricted by either H-2Ad (24-34) or H-2Ed (28-34). Furthermore, the site recognized by the M. leprae-specific monoclonal antibodies ML06 and ML10 was also localized in the overlapping sequences 25-31 and 25-29. In conclusion, we demonstrated that immunodominant species-specific T- and B-cell epitopes can be found in a mycobacterial heat-shock protein despite its highly conserved amino acid sequence. This finding suggests the feasibility of identifying a sufficient number of M. leprae-specific determinants for a composite T-cell immunodiagnostic reagent for tuberculoid leprosy.

Increased chromosome abnormalities in human preimplantation embryos after in-vitro fertilization in patients with recurrent miscarriage.

Recurrent miscarriage is a pathological condition induced by maternal and embryonic causes. This paper describes a prospective study to determine the real incidence of aneuploidy for autosomes 13, 16, 18, 21, 22, and gonosomes in preimplantation human embryos obtained from patients with recurrent pregnancy loss after ovarian stimulation in an IVF-ET programme. Our results indicate that aneuploidy for the chromosomes analysed are abnormally higher in embryos obtained after IVF from recurrent abortion patients (58%) compared to non-recurrent abortion patients undergoing IVF. Furthermore, monosomies are six times more frequent than trisomies (47:8) in preimplantation embryos from recurrent abortion patients. Based on the present study, preimplantation genetic diagnosis (PGD) of embryos obtained from patients with recurrent miscarriage could prove advantageous in diagnosing abnormal embryos and selecting normal embryos for transfer.

Peptide-specific T cell response to Mycobacterium tuberculosis: clinical spectrum, compartmentalization, and effect of chemotherapy.

The T cell repertoire of 59 patients with untreated tuberculosis was compared with that of 46 bacille Calmette-Guérin-vaccinated controls by assaying the proliferative responses to six permissively recognized peptides from the 16-, 19-, and 38-kDa molecules of Mycobacterium tuberculosis. A trend from higher to lower reactivity following this order: vaccinated controls > lymph node disease > localized extrapulmonary > pulmonary > pleural was seen for 4 of the peptides (P < .03). The decreased response of blood lymphocytes from patients with pleural tuberculosis was partially accounted for by sequestration of peptide-responsive cells within the pleural fluid. Chemotherapy "reversed" the depressed proliferative responses of patients with pulmonary and pleural tuberculosis depending on the peptide origin, being greatest for peptides of 16 kDa, transient for those of 19 kDa, and least for those of 38 kDa. These data demonstrate antigen specificity in the decreased responsiveness of patients with tuberculosis.

Intracytoplasmic sperm injection as a routine indication in low responder patients.

The aim of this study was to determine if a low response to gonadotrophin stimulation could be considered as an indication for intracytoplasmic sperm injection (ICSI). This prospective study included a total of 96 non-male infertile couples with six or fewer retrieved oocytes, who underwent 104 in-vitro fertilization (IVF) cycles between January 1996 and April 1997. They were randomly divided into two groups for fertilization, one by IVF and the other by ICSI. Groups were compared in terms of fertilization rates, fertilization failure, embryo quality, embryos transferred and reproductive outcome. ICSI provided similar fertilization rates per inseminated oocyte (77.7 versus 70.2%) and per obtained oocyte (56.5 versus 58.8%) as IVF. Furthermore, equal numbers (2.2 versus 2.5) and quality of embryos were obtained and comparable pregnancy (21.1 versus 17.3%) and implantation (14.0 versus 11.1%) rates. Neither the number of retrieved oocytes, nor patient age was relevant for the fertilization rates obtained with both techniques. The number of cases with complete fertilization failure was similar in both procedures. We conclude that the technique of fertilization is not related to the reproductive outcome of low responders, and the routine use of ICSI is not indicated.

Molecular interactions between embryo and uterus in the adhesion phase of human implantation.

Molecular interactions at the embryo-maternal interface at the time of implantation is an exciting field demanding a wide effort in order to understand the crucial process of embryonic implantation. The objective of the present work is to demonstrate the existence of a specific communication pathway (at the molecular level) between embryo and endometrium in the adhesion phase of human embryonic implantation. This pathway of molecular interactions is apparently initiated by the endometrium in the presence of an implanting blastocyst. It is mediated through the embryonic interleukin 0(IL)-1-alpha + IL-1-beta, and the target is the endometrial epithelial beta-3 integrin subunit. If the relevance of beta-3 is accepted as a marker of uterine receptively, these observations may imply that the normal hormonally-regulated human endometrium is the trigger of molecular events preparing the blastocyst to efficiently communicate and regulate endometrial adhesion molecules in order to implant.

Alterations to the nitric oxide pathway in the spontaneously hypertensive rat.

OBJECTIVES: To examine the role of nitric oxide in the cardiovascular system in spontaneous hypertension. In particular, we wanted to know whether the production of nitric oxide in the cardiovascular system of the spontaneously hypertensive rat is different from that of the normotensive Wistar-Kyoto rat and whether nitric oxide is biologically effective in this system. DESIGN: We studied various aspects of the L-arginine-nitric oxide pathway in the cardiovascular system of spontaneously hypertensive rats and Wistar-Kyoto rats. METHODS: To address the first objective we analysed the expression of endothelial nitric oxide synthase in the heart by Western blotting and the activity of constitutive nitric oxide synthase in resistance microvessels obtained from the mesenterium, both from spontaneously hypertensive rats and Wistar-Kyoto rats aged 14-18 weeks. We also analysed the concentration of the oxidative product of nitric oxide, nitrate, in plasma from these rats. To address the second objective, that is, to assess the bioactivity of nitric oxide, we studied the accumulation in tissue of cyclic guanosine 3',5'-monophosphate (GMP), as well as the acute and chronic effects of withdrawing the nitric oxide vasodilatory tone with the inhibitor of nitric oxide synthesis NG-nitro-L-arginine methyl ester on Wistar-Kyoto rats and spontaneously hypertensive rats. RESULTS: We found that the expression of endothelial nitric oxide synthase in the heart, the activity of constitutive nitric oxide synthase in resistance microvessels and the concentration of nitrate in plasma were all significantly higher in the spontaneously hypertensive rats. In contrast, neither cyclic GMP levels nor the effects of NG-nitro-L-arginine methyl ester were greater in the spontaneously hypertensive rat than they were in the Wistar-Kyoto rat. CONCLUSIONS: The nitric oxide pathway is upregulated in the resistance circulation and the heart of the spontaneously hypertensive rat by a mechanism involving induction of the constitutive nitric oxide synthase and overproduction of nitric oxide. However, nitric oxide is not sufficiently bioactive to stimulate the formation of cyclic GMP and to maintain an adequate nitric oxide-dependent vasodilatory tone.

[Incidence and risk factors in fractures of the proximal femur due to osteoporosis].

Every year more than one million fractures of the proximal femur occur in the world, especially in older persons. Given the continuous aging experienced by populations, such fractures will become more frequent from year to year and will constitute a growing public health problem. The largest increase is expected to occur in countries of Latin America around the year 2050. Since nearly 70% of all atraumatic fractures in persons over 45 are due to osteoporosis, a case-control study was conducted in the city of Mar del Plata, Argentina, for the purpose of investigating the incidence of and the risk factors associated with proximal femur fractures due to osteoporosis. Between 1 August 1992 and 31 July 1993, a record was kept of all fractures of the proximal femur due to osteoporosis in persons over 50 years of age that visited any of the city's 30 public and private health centers. A total of 246 cases was recorded. The incidence rate per 100,000 inhabitants in the above-50 population was 259 among women and 92 among men, for a ratio of 2.8:1. The incidence was consistently higher in the older age groups, especially in persons over 75. Factors associated with a statistically significant increased risk of fracture of the proximal femur were: a history of neurologic disorders, psychotherapeutic drug use, alcohol consumption, previous fractures, cardiovascular disease, and a decreased intake of milk products. There were no observed differences between cases and controls with respect to age at menopause, weight, height, previous activity, smoking habits, or sun exposure, nor were such differences detected in terms of the percentage of women who had undergone oophorectomy.

Bayesian inference of variance components for litter size in Rasa Aragonesa sheep.

Variance components were estimated for litter size in Rasa Aragonesa sheep, a meat breed from northern Spain, to determine whether selective breeding for litter size is a reasonable strategy to improve reproductive performance. We assumed an animal mixed effect threshold model with a binary response variable. Marginal estimates of the genetic parameters were obtained in the underlying scale using Bayesian inference, implemented via the Gibbs sampling procedure and a data augmentation approach. Posterior marginal means of heritability and repeatability were .077 and .141, respectively. Moreover, the 95% highest marginal posterior density region of heritability ranged from .051 to .101. Therefore, we conclude that litter size is a trait that could be selected for in breeding programs. The effect of the loss of pedigree information, a common feature of sheep production, on the estimation of the genetic parameters was also studied using simulation. The results indicate that the lack of pedigree information has little effect on our estimates of heritability.

Bayesian analysis of Wood's lactation curve for Spanish dairy cows.

A Bayesian procedure is presented to analyze test day performances in the scope of Wood's lactation function: atb (exp (-ct)), where a, b, and c = parameters, and t = time-dependent variable. The procedure has been applied to 148,557 test day controls corresponding to records for 15,349 first lactations of Spanish dairy cows. The procedure uses all available information and reduces the presence of atypical lactations. Moreover, the posterior marginal distribution of (co)variance components, breeding values, and systematic effects for the underlying variables of Wood's function are provided. The posterior means of the heritabilities were 0.43, 0.17, 0.40, and 0.29 for the parameters a (initial milk yield), b (ascent to peak), c (descent from peak), and total milk yield, respectively. Selection for total milk yield implies a great effect on the initial milk yield (a) but only minor effects on ascent to peak (b) and descent from peak (c).

Rising incidence of non-Hodgkin's lymphoma in Spain: analysis of period of diagnosis and cohort effects.

Incidence trends in non-Hodgkin's lymphomas for the period 1973-1991 were studied using data from Spain's Zaragoza and Navarre registries. The overall rate of increase was 5.8% per year. Age-period-cohort models were used, also including sex and registry as explanatory variables. In these models, the variable period was categorized according to the observed changes in diagnostic availability during the length of the study. Non-Hodgkin's lymphomas increased with age, and rates were 31% lower in women. Incidence was almost 50% higher in Navarre, but differences in registry procedures might account for this discrepancy. The rise affects all adult age groups and seems to be the result of changes related not only to the period of diagnosis, mainly ascribable to improvements in diagnostic and coding practices, but also to the birth cohort. According to the model, the risk increased with on-coming generations at a rate of 1.5% per year. The AIDS epidemic in Spain is unable to explain this trend, although it may well exert a stronger influence in the future. Additional knowledge is required to understand the observed rise and to prevent the growing numbers of cases that are expected in years to come.

Detection of Legionella pneumophila in wastewater by nested polymerase chain reaction.

The study of Legionella in treated wastewater acquires special importance when this water is used in irrigation by spray, as Legionella is transmitted via the inhalation of aerosols and may consequently represent a health risk. In this study, we applied polymerase chain reaction (PCR) amplification as an alternative method to plate culture for detecting L. pneumophila in twelve heavily biocontaminated samples from a wastewater treatment plant. Moreover, we studied the efficiency of rapid gel filtration methods and filtration through chelating ion exchange resin in the elimination of PCR inhibitors from wastewater samples. When Legionella was investigated by PCR without any previous treatment, no amplification occurred, and when we used chromatographic methods to eliminate PCR inhibitors, nine out of twelve samples became positive. These results indicate the abundant presence of Legionella in wastewater, and although the methods used to eliminate PCR inhibitors are effective in the preparation of clean samples, the possible presence of different metal-organic matter compounds, which are not eliminated, may produce false-negative results.

Childhood cancer incidence in Zaragoza and Navarre (Spain): 1973-1987.

Patterns and trends in childhood cancer incidence for Navarre and Zaragoza registries were studied over the 15-year period, 1973-1987. Overall cancer rates and rates for 10 specific types of cancer were analysed using a log-linear Poisson model or, alternatively, a gamma-Poisson model whenever overdispersion was present, with age, sex, registry and period being used as predictor variables. Childhood cancer was 30% more frequent in boys than in girls, and, except for lymphomas and bone tumours, incidence decreased remarkably with age. Adjusted rates were high in comparison with other European countries, particularly in the case of non-Hodgkin's lymphomas. Cancer rates proved somewhat higher in Navarre, but this difference attained statistical significance solely in the case of central nervous system tumours (rate ratio = 1.75; 95% confidence interval 1.21-2.54). A significant rise in overall incidence was observed (11% 5-yearly increase) due mainly to the upward trend in central nervous system tumours. While the rise in these tumours coincides with the period which witnessed the spread of computerised tomography in Spain, the trend nevertheless held steady over the last 5-year period, when access to this diagnostic technique had already become generalised nationwide.

Heart disease education and prevention program targeting immigrant Latinos: using focus group responses to develop effective interventions.

Although research has provided considerable knowledge concerning the positive effects of behavioral change on morbidity and mortality from heart disease and related risk factors, some segments of the population have not benefited equitably from this information. In April 1995, the National Heart, Lung, and Blood Institute (NHLBI) conducted seven focus groups to determine knowledge and attitudes about heart disease and associated risk factors, identify media usage and preferences, and assess publications usage and preferences among Spanish-speaking Latino immigrants residing in the Washington, D.C., metropolitan area. This information was gathered to assist in the development of key messages and strategies for the NHLBI Latino Community Cardiovascular Disease Prevention and Outreach Initiative, Salud para su Corazón--a heart disease prevention and education campaign. Findings from these focus groups indicate that Latinos may not benefit from heart disease prevention messages developed for the general population because of language and cultural differences. The researchers concluded that health education and disease prevention programs targeting the Latino community should develop educational materials and interventions that address language preferences and cultural values. Furthermore, to be effective, these programs should show people how to make positive behavioral changes based on their current circumstances, while remaining sensitive to the fact that Latino immigrants face major life adjustments and many are still greatly influenced by their country of origin.
