Oral manifestations of langerhans cell histiocytosis in a pediatric population: a clinical and histological study of 8 patients.

Langerhans Cell Histiocytosis is a rare and protean disease of unknown etiology, clinically characterized by single or multiple osteolytic and/or soft tissue lesions. Oral involvement may predate systemic manifestations or mimic infectious/inflammatory disorders. In the present series, oral involvement characterized eight cases of pediatric Langerhans Cell Histiocytosis and oral symptoms represented a very early sign of this disease in four patients. A careful clinical investigation and a proper histologic analysis were required for achieving a definite diagnosis and for confirming clinical suspicion of disease relapse. Immunohistochemistry, that nowadays represents a reliable and accessible ancillary histological technique, allowed the definite diagnosis in all cases of our series.

The use of silicon elastomer in maxillofacial rehabilitation as a substitute for or in conjunction with resins.

Maxillofacial prosthetic rehabilitation aims to restore anatomic function when serious tissue defects are present, as a result of congenital factors, trauma, or surgery. The aims are both aesthetic and functional, and results are obtained using devices that contain, reconstruct, and fill. This study introduces a technique using methods and materials that are acceptable to the patient in that they satisfy chewing, phonetic, and aesthetic functions, while respecting bone structure and restoring function to the soft tissues without causing trauma to them. Our techniques do not use innovative materials, but ones that are structurally different and that, until now, have not been connected with attachment systems. Acrylic resin, acetylic resin, and silicon are used, each in such a way as to take maximum advantage of each material's characteristics and thus give the best results.

Autogenous calvarial bone graft harvest: intraoperational complications.

In the field of maxillofacial reconstructive surgery, the authors take into consideration the autogenous calvarial bone graft harvest technique for the correction of maxilla bone defects and examine some salient aspects such as technical problems and possible surgical complications.

Surgical buccinator muscle myotomy in dentoskeletal Class II alterations.

Recent studies on autoptic findings have underlined the complexity of perioral muscles; the discovery of a fourth band of buccinator muscle and the effects it can have on teeth, alveolar bone, and oral mucosa during growth has stressed the importance of muscular influence on the genesis of and therapy for basal class II dentofacial alterations. Frederick proposed a surgical marginal myotomy to elongate the buccinator bundle together with inferior vestibuloplasty to unload the jaw from the overcontraction of the muscle and to reduce the inferior lip hypotone. In our investigation, we have applied this technique on a group of 50 patients with basal class II defect selected according to our protocol before functional or fixed orthodontics, and we have recorded and compared cephalometric measurements and clinical facial profile changes 12 (T1) and 24 (T2) months after the end of therapy to a control group with the same defect treated only with orthodontic functional devices. We obtained improvement of cephalometric parameters in 80% of patients who underwent surgery and orthodontics, and in only 50% of patients who underwent only orthodontics. Results suggest that neutralizing buccinator and mental muscles pressure on the jaw during growth can help the clinician dealing with II basal malocclusion therapy and relapse.

[Rokitamycin in odontostomatology. Controlled study of doses].

BACKGROUND: Rokitamycin is a semisynthetic macrolide with a lactonic ring with 16 atoms, showing anti-bacterial action at concentrations near to MIC. A controlled clinical study is carried out, in parallel groups, whose aim was to assess the therapeutic action and safety of two dosage schemes of rokitamycin, in the short term treatment (5 days) of acute infective processes of odontostomatological origin. METHODS: Twenty patients (14 males, 6 females) were recruited for the trial, suffering from alveolitis, abscess, phlegmon, sialadenitis and suppurating cysts. The patients were divided in a randomized fashion into two groups: 10 patients were treated orally with 800 mg/day and 10 with 1200 mg/day. Rokitamycin was supplied in 400 mg tablets. RESULTS: The 5 days of treatment with rokitamycin determined the complete resolution of the infective process, with eradication of the germs originally isolated, all belonging to the Streptococcus genus. Clinical efficacy was evident by the third day of treatment, with a prompt improvement of symptoms (functional limitation, pain, tumefaction) and the return of body temperature to within normal limits, in a totally superimposable fashion between the two groups. Safety was excellent with both doses, with no side effects observed. CONCLUSIONS: The results of the study show that treatment with rokitamycin in the short term, at the usual dosage of 800 mg/day, is a valid therapeutic scheme in infective processes in odontostomatology.

[Aneurysm of the sinus of Valsalva and late rupture of the interventricular septum due to infective endocarditis. Is the two-dimensional echocardiography of endocarditic extension into the interventricular septum possible?].

In a 54 year old woman affected by infective endocarditis, with bicuspid aortic valve and aneurysm of sinus of Valsalva protruding into the right atrium, two-dimensional echocardiography helped to identify altered myocardial echo patterns of the interventricular septum at basal level. Because of the proximity with valvular lesions due to endocarditis, we could interpret these appearances as produced by invasion of the infective process into the septum. The autopsy report of septum rupture in the same area further supports our interpretation of the echocardiographic finding.

Implant surface and osseointegration.

Oral rehabilitation by means of osteointegrated implants is gaining more and more importance, in particular due to the improved characteristics of the fixture: among these, the superficial microscopical morphology plays a role of primary importance, since it can influence the evolution of the osteointegrative process. Therefore, this led to many surface treatments which can improve the quality and the speed of formation of the bony tissue. The aim of this paper is a review of the literature on this topic in order to find out which processes of characterization of the surface implant are now available and which results can be obtained.

[Results of triennial screening of a mountain community school population in USSL n. 42].

The paper reports data regarding dental caries in deciduous and permanent teeth in a sample population of 1015 pupils aged between 5 and 15, resident in the mountain community of the Chisone and Germanasca Valleys. The DMF index was significantly high, especially if one takes into account that the partial findings referring to '89 are virtually unchanged in comparison to '87.

[Odontogenic tumors. The importance of a correct histopathological diagnosis].

The troubles in the exact diagnosis of odontogenic tumors spring from the poor knowledge of the histological type of the lesion, from mutations in the ectodermal cells of the tumors and from the uncommonness of the pathology. Wrong diagnosis in this field of stomatology often lead to unnecessary surgical resection and could be avoid with better cooperation between surgeon and pathologist.
