[Presence of hepatitis C virus in renal tissue in membranoproliferative glomerulonephritis and cryoglobulinemia].

Although hepatitis C virus infection has been documented in several extrahepatic diseases, the deposition of HCV RNA in glomerular structures has proved to be difficult to demonstrate. We report a patient with membranoproliferative glomerulonephritis, type III circulating cryoglobulins and hepatitis C virus infection with detection of HCV RNA in serum, cryoprecipitate and renal tissue using specific RT-PCR technique. These data confirm that HCV could have a direct role in renal damage.

[Irritable bowel syndrome in patients under age 55 with dyspeptic syndrome].

The presence of IBS was studied in 289 patients under 55 years of age with dyspeptic syndrome. The subjects were divided into two groups: group I (240 patients with DS) and group II (49 patients with DS and IBS). The mean duration of follow%shy;up was 10.8 years (130 months). IBS was diagnosed in 49 patients (17%). Females accounted for 64% and 77% of groups I and II, respectively (p < 0.05). There were no differences in mean age or the type of FDS. The patients in group II consulted more often and earlier (92% returned for consultation, and took 27 months in doing so) than those in group I (52% and 40 months)(p < 0.05). The number of diagnostic tests requested was greater in group II. Psychiatric pathology was in turn more common in group II than in group I (20 versus 7%)(p < 0.05).

Alpaca semen characteristics previous to a mating period.

Increasing the knowledge of the semen characteristics in the alpaca will contribute to understanding one of the many factors that affect the poor fertility rate in this species. Ten adult male alpacas, 2.6-10 years of age, average weight 64.7 +/- 4.7 kg were used. The animals were distributed randomly into two groups of five each and submitted alternatively to two semen collections, using an artificial vagina and sexually receptive females. For the first semen collection the animals had a sexual rest period of about 90 and 45 days before the second. Duration of semen collection, color and volume of ejaculate were recorded, and sperm concentration and morphology (light microscopy) were evaluated. Descriptive statistical analyses were used for each variable, considering all samples obtained (n= 19). An analysis of variance for animal groups and opportunity of collection were used for quantitative variables. Most frequent color was opalescent white (84.2%). There were no statistical differences among male groups or between semen collections. The average values and standard deviations for the quantitative variables were: 12.3 +/- 7.2 min for semen collection time, 1.8 +/- 0.8 ml for ejaculate volume, (17.6 +/- 26.1) x 10(6) sperm/ml for sperm concentration and 34.0 +/- 52.2 x 10(6) for total number of sperm per ejaculate. The percentage of normal spermatozoa was 51.0 +/- 12.4%. From the total abnormalities, that of mid piece segment (14.4%) was the most frequent. These results indicate that male alpaca have poor semen quality, when compared with other domestic species. Nevertheless, for the evaluation of male alpaca as breeders it would be necessary to create a protocol for the selection of them, where phenotypic, behavioral and seminogram aspects are considered. The values reported herein define the characteristics of the alpaca semen that could be considered as the initial base of the seminal analysis to select male alpacas before mating.

[Nasal polyposis: postoperative long term results (5 years) after endoscopic sinus surgery].

The purpose of this study is to evaluate the results after endoscopic sinus surgery in cases of nasal polyposis. We include 54 patients with diffuse nasal polyposis suffering from nasal obstruction. An endoscopic sinus surgery under general anesthesia with excision of the polyposis was performed. There were no complications. The patients were followed closely and treated with local and systemic steroids. After 5 years, the 75% of these patients were free of nasal symptoms and in the 67% the mucosa appeared normal. We also review the influence of some factors (ASA triad, anosmia, allergy...) in the recurrence of the disease.

Detection of epithelial tumour RNA in the plasma of colon cancer patients is associated with advanced stages and circulating tumour cells.

BACKGROUND: Although circulating tumour DNA has been detected in patients with different types of cancer, little is known of free RNA in cancer patients. AIMS: We investigated the presence of RNA from epithelial tumours in plasma from patients with colorectal carcinomas, and its correlation with tumour characteristics and circulating tumour cells. METHODS: beta-actin mRNA was analysed to assess the viability of plasma RNA in samples from 53 patients with colonic cancer and 25 controls. Subsequently, nested primers were used to detect the presence of cytokeratin 19 (CK19) and carcinoembryonic antigen (CEA) RNA in the same samples. Nine clinicopathological parameters were studied to correlate the molecular and clinical parameters. Additionally, we investigated for micrometastases in blood in 18 of these patients and in 10 of the controls samples. RESULTS: All samples had detectable quantities of beta-actin RNA. In the controls, one case (4%) was positive for CEA and five (20%) for CK19 RNA; of the 53 patients, 17 cases (32%) were positive for CEA and 39 (73.6%) for CK19 RNA. This was statistically significant (p=0.000001). Advanced stages (p=0.03) and soluble CEA status (p=0.03) were associated with the presence of CEA, CK19, or both RNAs in plasma. Lymph node metastases (p=0.06) and vascular invasion (p=0.07) were almost significant. On the basis of these results, we examined the possible presence of micrometastases in blood in several of these patients. The presence of plasma tumour RNA was found to be associated with circulating tumour cells in blood (p=0.04). CONCLUSIONS: Epithelial tumour RNA is detectable in plasma from colon cancer patients. This molecular event is associated with advanced stages and circulating tumour cells. Our results could offer new approaches in the diagnosis and monitoring of colon cancer.

Endotoxin and anti-endotoxin antibodies in the prognosis of acute pancreatitis.

AIMS: We investigate the behaviour of endotoxin in patients with acute pancreatitis and its relationship with the development of complications. EXPERIMENTAL DESIGN: Prospective study. PATIENTS: We assessed plasmatic endotoxin and anti-core endotoxin antibodies (EndoCab IgG and IgM) levels on first and third days from admission in patients with acute pancreatitis episodes, classifying them as mild or severe according to Atlanta's criteria. RESULTS: Nineteen patients were included, seven with severe pancretitis (36.8%) and twelve with mild pancreatitis (63.2%). Endotoxin levels were similar on first day in both mild and severe pancreatitis, and higher in the latter on third day (p > 0.05). Patients with severe pancretitis had lower EndoCab IgM levels on first and third days from admission (day 1: 18.3 vs 33.3 MU/ml, p < 0.01; day 3: 18.4 vs 33.4 MU/ml, p < 0.05). When analysed separately systemic and local complications, we observed, in the same days, a decrease of EndoCab IgM levels in patients who developed systemic complications (day 1: 18.3 vs 32.7 MU/ml, p = 0.01; day 3: 18.3 vs 35.1 MU/ml, p < 0.01). EndoCab IgG levels were also lower in severe acute pancreatitis in both determinations, but differences weren't significant. CONCLUSIONS: EndoCab levels decrease early in severe acute pancreatitis, mainly if systemic complications are present. This antibody depletion is greater for IgM than IgG, and seems to occur earlier than an increase in endotoxemia.

[Prognosis in dyspeptic patients under the age of 55].

BACKGROUND: We studied the evolution and diagnostic tests performed in patients with dyspepsia aged less than 55 years. PATIENTS AND METHOD: We included 289 patients under age 55 with clinically manifest dyspepsia who were referred for specialized digestive evaluation from the primary care setting. RESULTS: One half of patients consulted again in the course of the study for the same symptoms. In 131 patients (45%), an endoscopy was proposed at some stage during the study. Gastroduodenal pathology was identified in 37% of the patients who underwent an endoscopy. CONCLUSIONS: In these patients, the approach adopted by the gastroenterologist must comprise assessment of the symptoms with endoscopy.

Use of ultrafiltration on the analysis of low molecular weight complexing molecules. Analysis of iminodiacetic acid at constant ionic strength.

Low molecular weight complexing molecules may be indirectly analyzed by ultrafiltration with the aid of a water-soluble polymer. The theory concerned is developed and the analysis of iminodiacetic acid (IDAA) is performed by ultrafiltration of aqueous solutions containing poly(sodium-4-styrenesulfonate) (PSS), Cu(NO3)2, and IDAA in variable concentrations. The technique allows measuring IDAA in a range of concentrations between 10(-4) M and 10(-5) M when a 1.6 x 10(-4) M Cu2+ solution is ultrafiltered in the presence of PSS and IDAA.

Melatonin protects against renal oxidative stress after obstructive jaundice in rats.

The goals of this study were to analyze the renal oxidative status in experimental biliary obstruction and to evaluate the impact of melatonin on renal oxidative stress. Cholestasis was done by double ligature and section of the extra-hepatic biliary duct. Melatonin was injected i.p. (500 microg/kg/day). Malondialdehyde, reduced glutathione, catalase, superoxide dismutase, glutathione reductase, glutathione peroxidase and glutathione transferase were determined in the renal tissue. After biliary obstruction, an increase in malondialdehyde (P<0.0001) and a fall in reduced glutathione (P<0.0001) were seen. Moreover, the scavenger enzyme activity had significantly diminished. After melatonin administration, the malondialdehyde fell significantly (P<0.0001), whereas reduced glutathione showed an important increase (P<0.0001) compared with the ligated bile duct group. Experimental bile duct obstruction was associated to an increase of renal oxidative stress. Treatment with melatonin decreased the renal lipid peroxidation, and both the reduced glutathione as well as the scavenger enzyme activity recovered.

[Helicobacter pylori infections: antigen detection in stool samples].

BACKGROUND: The aim of this study is to evaluate a new diagnostic test to detect Helicobacter pylori antigen in stool samples (HpSA), and compare the results with those obtained by standard techniques (rapid urease test,culture, histological examination of gastric biopsy specimens,13C-urea breath test and serology), in a paediatric population with gastrointestinal symptomatology. PATIENTS AND METHODS: Sixty patients with dyspeptic symptoms (37 females and 23 males;mean age 10.9 years) attending the Gastroenterology Service were included in the study. Exclusion criterium was previous treatment with proton pump inhibitors, bismuth compounds or antibiotics. Rapid urease test, culture and histologic study of gastric biopsies,13C-urea breath test and serology, as well as HpSA, were performed to all patients. RESULTS: Forty seven patients were considered infected by H.pylori on the basis of bacterium isolation and 13C-urea breath test positivity. HpSAwas detected in 45 of the 47 H.pylori positive patients(95.7%). There were no HpSA false positive. CONCLUSION: Our results show that this new test is highly sensitive (95%) and specific(100%) for detection of H. pylori infection. It has some advantages over other non invasive techniques: it is easy to perform,requires no blood samples and its cost is lower than that of 13C-urea breath test.

Children's and adolescents' pain language in Spanish: translation of a measure.

The decision to include a Spanish-speaking sample in a study of pain in children with leukemia necessitated translation of the data collection tools. Therefore, the Adolescent Pediatric Pain Tool (APPT) was translated by using the standard translation, back-translation procedure. The Spanish word list consists of 66 descriptors in the sensory, affective, evaluative, and temporal domains. After initial pilot work with the translated APPT with well children in the Dominican Republic, additional pilot work was conducted in California. Two of these studies were completed with well children and adolescents, and one study that was part of a clinical research project focused on pain in children and adolescents with leukemia. Although content; construct, concurrent validity; and test-retest reliability were tested, further studies are necessary with larger populations of children and adolescents. This series of pilot studies supports the usefulness of the Spanish translation of the APPT word list, particularly for clinical or research situations in which multiple versions of a tool would counteract ease of use.

Serum albumin as indicator of clinical evolution in patients on parenteral nutrition. Multivariate study.

BACKGROUND AND AIMS: The aim of this study is to establish whether serum albumin concentration at the beginning of parenteral nutrition is related to morbidity and mortality. METHODS: In this cohort study spanning four years, a number of patients were classified into twelve groups, depending on their clinical status at the beginning of parenteral nutrition. Their serum albumin concentration and other clinical parameters were then measured and twelve multiple logistic regression models were thus generated in order to model the relationship between initial albumin concentration and risk of morbidity/mortality. RESULTS: 1953 (84%) of the 2321 patients studied were hypoalbuminemic. In six models, this condition was associated with a significant increase in the risk of nosocomial infection. However, no model could be associated to significant risk of renal failure, and only patients with previous hepatopathy were at risk for hepatic failure. In seven models, there was a significant increase in mortality. CONCLUSIONS: Serum albumin concentration at the beginning of parenteral nutrition is related to mortality and morbidity associated with nosocomial infection in some groups of the study.

Bochdalek's hernia in adults.

Bochdalek's hernia is a congenital hernia of the diaphragm, which is manifested in the early years of life. Its diagnosis is difficult and is based on barium studies. We present an adult patient with Bochdalek's hernia who exhibited a gastric volvulus. The patient had a history of intermittent abdominal pains. In this article, we analyze the diagnostic and therapeutic procedures, laying special emphasis on the importance of early diagnosis in the prevention of complications.

Squamous cell carcinoma of the oesophagus in a patient with Klippel-Trenaunay syndrome.

The Klippel-Trenaunay syndrome (KTS) is a rare disorder characterized by a triad of port-wine stains, varicose veins, and bony or soft tissue hypertrophy of one extremity. Digestive bleeding is the most commonly observed gastrointestinal manifestation. In rare cases, the syndrome is associated with malignancies. We report the case of a 38-year-old woman with KTS and dysphagia caused by an oesophageal squamous cell carcinoma.

Objective assessment of the sympatholytic action of the Nei-Kuan acupoint.

From animal experiments and clinical trials, we can think that acupuncture of Nei-Kuan (EH-6) is effective in the control of some vegetative functions. We have tried to determine the real reach of these assumptions by means of simple quantitative measurement methods, such as detection of cardiac frequency and systolic and diastolic blood pressure, and the sympathetic electrical response. The experiments were performed on 29 healthy volunteers. Basal values of these parameters were recorded, and also after orthodox Nei-Kuan acupuncture, non-acupunctural point puncture, supine lying by 15 min, Chü-Tze (EH-3) acupuncture and bipolar electrical stimulus of the median nerve at the Nei-Kuan level in the wrist. Such conditions were designed to evaluate non-specific puncture, repose and direct median nerve stimulation influences in the effects of Nei-Kuan acupuncture. We conclude that Nei-Kuan acupuncture strongly inhibits sympathetic tone, with reduction of cardiac frequency, systolic blood pressure and an important reduction of the amplitude of the sympathetic electrical response; the latency of the electrical response was also prolonged. Some weak effects on blood pressure and cardiac frequency were observed also, as non-specific effects of puncture and median nerve stimulation. From previous anatomical data, we hypothesize that the level of such actions is suprametameric, with strong implication of the diencephalon and cerebral cortex.

[Tolerance to and colon cleansing with 2 preparations. Polyethylene glycol or sodium phosphate].

OBJECTIVE: To compare colon cleansing and tolerance with two preparations: polyethylene glycol 4-liters (PEG) and sodium phosphate (FS). MATERIAL AND METHODS: Prospective randomized study of 100 consecutive patients who underwent endoscopy after taking PEG (Bohm lavage solution) or FS (Fosfosoda). All patients were evaluated for the presence of adverse effects, degree of discomfort of the preparations (1: mild, 2: moderate, 3: severe) and the degree of cleansing with the products (1: poor, 2: fair, 3: good, 4: excellent). RESULTS: Age, sex, depth of insertion and surgical history were similar for both groups. Discomfort in the FS group was mild in 37, moderate in 11 and severe in 2 (mean score 1.30 +/- 0.54); in the PEG group it was mild in 28, moderate in 15 and severe in 7 (mean score 1.58 +/- 0.73). Colon cleansing was poor or fair in 11 (22%) patients in the PEG group and in 8 (16%) patients in the FS group and was good or excellent in 39 (78%) in the PEG group in comparison with 42 (84%) in the FS group. Among the 50 patients who took the FS preparation, 12 had previously taken the PEG preparation and 11 of these preferred the FS preparation. Among the 50 patients who took the PEG preparation, 10 had previously taken preparations: 4 had used enemas (considering the present PEG to be worse) and 6 had taken the same PEG as that used in the present study (p < 0.005). One patient in the FS group and four in the PEG group did not complete the preparation because of vomiting. The adverse effects of both preparations were similar. CONCLUSIONS: The adverse effects and degree of colon cleansing were similar for FS and PEG. Tolerance and patient comfort were greater with FS than with PEG.
