Argyrophilic nucleolar organiser regions (AgNORs) in breast lesions.

Nucleolar organiser regions (NORs) demonstrated by argyrophilia of NOR-proteins are indicator of cellular proliferative activity. The NORs can be identified in the nuclei as brown or black dots with silver colloidal staining technique in formalin fixed paraffin sections and in cytology smears. Seventy-five cases including 45 tissue sections and 30 fine needle aspiration cytology (FNAC) smears of benign and malignant lesions of breast have been studied to evaluate the significance and practical application of AgNOR count per nucleus. Out of 45 tissue sections 15 belonged to fibrocystic disease, 10 fibro-adenomas and 20 carcinomas and of the 30 FNAC smears, 10 were fibrocystic disease, 8 fibro-adenomas and 12 carcinomas. In fibrocystic disease the mean AgNOR count was 1.60 (FNAC group-0.75, tissue section-1.61). In fibro-adenomas it was 1.61 (FNAC-1.63, tissue section-1.59). The mean count in carcinoma was 12.10 (FNAC-12.08, tissue section-12.10). The difference in AgNOR count in fibrocystic disease and fibro-adenoma was not significant, but that between benign breast lesion and carcinoma was significant. No difference was observed between FNAC and tissue section groups in benign or malignant lesions. The simple staining technique can be used as an additional criterion to differentiate the benign and malignant lesions of breast.

Clinicopathological study of clinically undiagnosed cases of kala-azar with special reference to grading of parasites.

Clinicopathological study of 34 clinically undiagnosed cases of kala-azar has been undertaken with a view to establish the diagnosis and correlate the quantitative grading of Leishmania donovani in bone marrow/splenic aspirates with clinical features and duration of disease. The ages of the patients were between 5 and 45 years. The duration of illnesses ranged from 2-24 months with 47% having 2 months' duration. Splenomegaly was less than 7 cm in 70.5% cases. Anaemia, leucopenia and mild thrombocytopenia were present in all the cases. Aldehyde test was positive in 47% cases. Bone marrow aspiration revealed the parasites in 82.3% cases. L donovani was also observed in 66.6% cases of splenic aspiration including 6 cases where bone marrow failed to reveal the parasite. Grading of the parasites in bone marrow and splenic aspirates revealed majority of the cases (73.5%) were in 3+ and 4+ grades. No correlation between splenic size and duration of illness or parasite grading was detected. Neither any positive correlation between parasite grading and duration of illness was observed.

Fine needle aspiration cytology of cervical lymphadenopathy with special reference to tuberculosis.

One hundred eighty cases of cervical lymphadenopathy have been studied by fine needle aspiration cytological examination followed by histopathologic examination of the excised lymph nodes. The diagnostic accuracy was 84.4% for tuberculous lymphadenitis by fine needle aspiration cytological examination. Observation of caseous necrosis (84.2%) and epithelioid cells (73.6%) were the most characteristic diagnostic features in the aspirated smears. Acid-fast bacilli were observed in 45.6% cases. Metastatic carcinoma also yielded a high diagnostic accuracy ie, 89%. Fine needle aspiration cytology has been found to be safe, quick, inexpensive with high diagnostic accuracy in cervical lymphadenopathy.

Rapid diagnosis of typhoid fever by co-agglutination in an Indian hospital.

Detection of Salmonella typhi infection by a co-agglutination assay for specific O, H and Vi antigens and by blood culture were compared for 110 patients with suspected typhoid fever. Blood cultures were positive for S. typhi in 25.5% of patients. Co-agglutination tests with patients' serum and with blood culture supernates gave positive results in 70.9% and 67.3% of cases respectively. S. typhi antigens Hd and O9 were detected in patients' serum by co-agglutination in 96.4% of blood culture-positive, and 62.2% of blood culture-negative patients. Co-agglutination results were uniformly negative with serum samples from a control group of 50 healthy individuals, 20 patients with febrile non-typhoid infectious disease and 20 patients with non-infectious febrile disease. Of the 25 patients with suspected typhoid fever who had not received prior antibiotic treatment, 88% yielded positive blood cultures and 96% gave positive results in serum co-agglutination tests. By contrast, of the 85 patients who had received prior antibiotics, only 7% yielded positive blood cultures, but 63.5% gave positive results in serum co-agglutination tests. Co-agglutination tests with serum offer a simple, rapid, sensitive, specific and economical method for the early diagnosis of typhoid fever.

Malignant lymphoma--a 15-year study report.

Analysis of incidence of different types of malignancies during 15 years showed occurrence of malignant lymphoma in 192 cases (4.1%). There were 82 cases (42.7%) of non-Hodgkin's lymphoma and 110 cases (57.3%) of Hodgkin's disease. Lymphocytic lymphoma, the major type observed among the non-Hodgkin's lymphoma with 58 cases (30.21%), showed a preponderance of well differentiated type (40 cases). Other cases in this group comprised mixed cellularity (2.08%), histiocytic (3.64%) and unclassified one (6.77%). Analysis of the Hodgkin's disease cases showed majority in it belonged to the mixed cellularity type (44.5%) followed by the lymphocytic predominant type (27.3%), lymphocytic depletion type (18.2%) and nodular sclerosing type (10%). The incidence of 14 cases of extranodal lymphomas was 7.3% among the lymphomas with maximum occurrence in the gastrointestinal tract (42.8%).

Rare variant of acute promyelocytic leukaemia.

Acute promyelocytic leukaemia (APL) is a peculiar sub-type of acute myeloblastic leukaemia characterised by presence of atypical promyelocytes in bone marrow and peripheral blood and common occurrence of haemorrhagic episodes associated with disseminated intravascular coagulation. Two morphological forms of APL are recognised--typical hypergranular and microgranular or M3 variant. This microgranular form of APL is rare but has got some peculiar features and often can be diagnosed by peripheral blood smear examination alone without bone marrow examination. Three cases of microgranular form of APL observed during a period of 6 months are reported here.

Similar activities of nerve growth factor and its homologue proinsulin in intracellular hydrogen peroxide production and metabolism in adipocytes. Transmembrane signalling relative to insulin-mimicking cellular effects.

Generation of hydrogen peroxide in adipocyte plasma membrane and its intracellular metabolism and regulatory role have been shown by Mukherjee and co-workers to be a major effector system for insulin [Fedn Proc. 35, 1694 (1976); Archs Biochem. Biophys. 184, 69 (1977); Biochem. Pharmac. 27, 2589 (1978); Fedn Proc. 37, 1689 (1978); and Biochem. Pharmac. 29, 1239 (1980)]. The possible involvement of this mechanism in the action of structurally similar polypeptides having some insulin-like metabolic effects was investigated. The beta-subunit of nerve growth factor (2.5 S NGF, mol. wt 13,500) which has a striking structural homology with proinsulin and has been reported to exert certain insulin-like metabolic effects in its own target tissues (e.g. growing neurites and sympathetic ganglia), and the insulin-derived polypeptides, desalanine-insulin and desoctapeptide-insulin, as well as proinsulin, were examined for their effects on rat adipocytes, employing the technique of formate oxidation. Both NGF and proinsulin caused increased [14C]formate oxidation, showing similar intrinsic activities, up to a maximum of 140-160% of the basal rate; insulin increased the rate to 190-210% of the basal rate. The relative potencies of the hormones toward H2O2 formation and stimulation of the pentose phosphate pathway activity were: insulin (EC50: 2.5 x 10(-11) M), desalanine-insulin (EC50: 2.5 x 10(-10) M), proinsulin (EC50: 8 x 10(-9) M), and NGF (EC50: 10(-9) M). The biologically inactive derivative, desoctapeptide-insulin, did not stimulate glucose oxidation, although it caused a small increase in formate oxidation, with an EC50 of 5 x 10(-7) M, indicating a suboptimal level of H2O2 formation in the elevation of the hexose monophosphate shunt activity. 3-Amino-1,2,4-triazole (50 mM), which irreversibly decomposes the peroxidatic compound II of the catalase: H2O2 complex, inhibited formate oxidation to a greater extent in the hormone-treated cells than in the control cells, whereas sodium azide, an inhibitor of the hemoprotein, catalase, completely inhibited it. The abilities of the polypeptides to stimulate H2O2 formation correlated with their abilities to promote lipogenesis from [U-14C]-D-glucose, as expected of insulin. The cellular GSH/GSSG ratio increased concomitantly with the stimulation of glucose oxidation via the shunt, indicating a tight coupling between these processes. The results confirm that the hydrogen peroxide production is a common basis of the metabolic actions of growth-promoting polypeptide hormones or mitogens beyond their respective receptors.

Proteoglycans, proteases, chemotaxis, and aggregation of inflammatory, cells.

A trypsin-like protease activity present in rabbit alveolar macrophages was shown to be activated by hyaluronic acid (HA) at 10(-9) M, but not by other proteoglycans. This activation of proteases by HA was accompanied by aggregation of alveolar macrophages and by the release of chemotactic activity. This chemotactic activity was destroyed by proteases and was less active for polymorphonuclear leukocytes than for alveolar macrophages. These effects of HA appear to be sepcific for alveolar macrophages, because no similar effects of HA on human polymorphonuclear leukocytes or lymphocytes could be observed. These observations suggest that HA, which is secreted into airways, plays a specific role in the macrophage-mediated chronic inflammatory processes in the distal lung.

Motility of rabbit alveolar cells: role of unsaturated fatty acids.

A mechanism for the specific accumulation of macrophages in alveoli or other biologic cavities following injury is presented. The data herein indicate that unsaturated fatty acids, ie, linoleic and linolenic acids, which accumulate in rat pleura following injection of carrageenan or during incubation of rabbit alveolar macrophages (AMs), strongly activate migration in vitro of AMs but not of human polymorphonuclear leukocytes (PMNLs). Other anionic lipids, ie, phosphatidylglycerol, as well as various nonspecific proteins, such as gelatin, or albumin were also shown to be potent activators of migration of AMs and not of PMNLs. These observations suggest that the elaboration of unsaturated fatty acids, as well as of nonspecific proteins, is responsible for the specific accumulation of macrophages in injured body spaces, such as alveoli or pleura.

Motility of human polymorphonuclear leukocytes. Roles of hydroxy fatty acids, other lipids, and cations.

Rapid ionic fluxes across local areas of leukocyte plasma membranes result in local swelling, ie, ruffling or blebbing. These fluxes are stimulated either locally or generally over the cell membranes by hydrophobic chemotactic peptides. This swelling is associated with migration of the cells in the direction of the swollen area of the cell. Hydroxy fatty acids, other lipids, and Ca++ ionophores activate a Ca++-dependent migratory activity of the cell, whereas acidic peptides activate a monovalent cation-dependent migratory activity. These two processes are therefore additive. The swelling due to the peptides results in an increase in urea space but no change in inulin space, whereas, swelling due to permeant cations and anions causes a massive increase in both spaces. Migration appears to result from the pushing of cellular contents, nucleus and cytoplasm, into the more swollen area of the cell by unopposed contraction of the unswollen area of the cell.
