Fractal stream chemistry and its implications for contaminant transport in catchments

The time it takes for rainfall to travel through a catchment and reach the stream is a fundamental hydraulic parameter that controls the retention of soluble contaminants and thus the downstream consequences of pollution episodes. Catchments with short flushing times will deliver brief, intense contaminant pulses to downstream waters, whereas catchments with longer flushing times will deliver less intense but more sustained contaminant fluxes. Here we analyse detailed time series of chloride, a natural tracer, in both rainfall and runoff from headwater catchments at Plynlimon, Wales. We show that, although the chloride concentrations in rainfall have a white noise spectrum, the chloride concentrations in streamflow exhibit fractal 1/f scaling over three orders of magnitude. The fractal fluctuations in tracer concentrations indicate that these catchments do not have characteristic flushing times. Instead, their travel times follow an approximate power-law distribution implying that they will retain a long chemical memory of past inputs. Contaminants will initially be flushed rapidly, but then low-level contamination will be delivered to streams for a surprisingly long time.

Loaded hydroxylapatite-coated implants and uncoated titanium-threaded implants in distracted dog alveolar ridges.

AIMS: The aim of this study was to determine the response of alveolar bone after it was augmented vertically with distraction osteogenesis, implanted with hydroxylapatite (HA)-coated implants and noncoated titanium-threaded implants, and subsequently loaded for 1 year. METHODS: Eight dogs each had 4 implants placed horizontally into an edentulous mandibular quadrant. After integration, a distraction osteogenesis device was fabricated in the laboratory. An osteotomy was made to allow the crest of the alveolar ridge to be distracted vertically. After 10 mm of vertical distraction, the distraction devices were stabilized with light cured resin. After bone fill of the distraction gap was radiographically confirmed in all dogs at 10 weeks, 2 implants were placed into the ridges. Four dogs had threaded titanium implants placed, and 4 dogs had threaded HA-coated implants placed, with 1 implant in the distracted bone and 1 implant in adjacent nondistracted bone, for both groups. After 4 months for implant integration, bridges were fabricated and secured to the implants with screws. Crestal bone levels were evaluated by radiographs through 1 year of function. Animals were killed after 1 year of loading for histologic evaluation. RESULTS: The vertical ridge augmentation averaged 8.8 +/- 1.0 mm after 10 weeks of healing after distraction. Analysis of variance indicated a significantly greater change from baseline for HA-coated implants and for distracted bone sites. Histologic examination showed that bone had formed between the distracted segments creating an augmented ridge. The average thickness of the labial cortex in the distraction gap was significantly thinner than the lingual cortex in distracted bone or the lingual and labial nondistracted cortical bone. The presence of a dental implant did not significantly affect cortical bone thickness. Serial sections showed that implants remained integrated and functional without soft tissue inflammation. CONCLUSION: Dental implants placed into alveolar ridges augmented with the technique of distraction osteogenesis were functional for the length of this study.

Flumazenil in drug overdose: randomized, placebo-controlled study to assess cost effectiveness.

OBJECTIVE: To investigate cost effectiveness of administration of flumazenil to patients presenting with suspected acute drug overdose. DESIGN: Double-blind, prospective, placebo-controlled randomized study. SETTING: University teaching hospital. PATIENTS: Forty-three adults presenting with suspected drug overdose and having a Glasgow Coma Scale (GCS) score of <13. Patients with known benzodiazepine/tricyclic ingestion were excluded. INTERVENTIONS: Intravenous administration of flumazenil (up to 2 mg) or placebo. MEASUREMENTS AND MAIN RESULTS: Individual patient costs were assessed and data aggregated for each treatment group. Major diagnostic and therapeutic interventions were recorded and between group comparisons performed. Clinical response to study drug administration was assessed by obtaining pre- and post-drug GCS scores and observation of the patient for at least 180 mins for signs of resedation. Aggregate cost or number of major diagnostic and therapeutic interventions were not different between groups. Patients randomized to the flumazenil group showed a marked increase in GCS score (7.4 to 11.8) compared with those in the placebo group (8.2 to 8.6). CONCLUSION: Use of flumazenil in intentional drug overdose of unknown etiology is not cost effective.

Posttraumatic chylous ascites in a child: recognition and management of an unusual condition.

Chylous ascites is an extremely rare and often unrecognized complication of abdominal trauma in children. The management of this condition has traditionally been nonsurgical, but the success rate with nonoperative treatment is not always satisfactory. A case of posttraumatic chylous ascites in an abused toddler is presented, with emphasis on the diagnosis and treatment of this rare disorder.

Gender differences in corpus callosum size in first-episode schizophrenics.

Previous studies indicate differences between schizophrenics and normals in thickness and overall size of the corpus callosum, particularly in female subjects. The present study compares the area of the corpus callosum as measured by magnetic resonance imaging (MRI) in men and women experiencing first-episode cases of schizophrenia. The corpus callosum area is also correlated with measures of neuropsychological function. The results of this study suggest that women who are first-episode schizophrenic patients have a smaller total corpus callosum area than female controls, with no difference noted for men. In normal controls, a larger corpus callosum was associated with better cognitive function, whereas in schizophrenics, no such relationship emerged.

Comparison and significance of annual hydrochemical budgets in three small granitic catchments with contrasting vegetation (Mont-Lozere, France).

Chemical budgets are presented for three small granitic catchments in Southeastern France, with contrasting vegetation type: beech coppice; spruce forest; and grassland. The results show a small net loss of cations, and a large accumulation of sulphur in the soil, which acts as an additional proton sink. Significantly higher weathering rates are observed for the conifer catchment. Clearfelling of the spruce forest, late in the monitoring programme, increased net cation and nitrate losses. The importance of dry atmospheric deposition in the input-output budget, particularly for forests, is highlighted.

Measurement of gastric emptying rate in humans. Simplified scanning method.

Simultaneous measurements of the gastric emptying rate of the solid and liquid phase of a dual-isotope-labeled test meal were made using a gamma camera and a simple scintillation detector, similar to that used in a hand-held probe. A simple scanning apparatus, similar to that used in a hand-held scintillation probe, was compared with simultaneous measurements made by a gamma camera in 16 healthy males. A dual-labeled test meal was utilized to measure liquid and solid emptying simultaneously. Anterior and posterior scans were taken at intervals up to 120 min using both a gamma camera and the scintillation probe. Good relative agreement between the methods was obtained both for solid-phase (correlation range 0.92-0.99, mean 0.97) and for liquid-phase data (correlation range 0.93-0.99, mean 0.97). For solid emptying data regression line slopes varied from 0.75 to 1.03 (mean 0.84). Liquid emptying data indicated that slopes ranged from 0.71 to 1.06 (mean 0.87). These results suggested that an estimate of the gamma measurement could be obtained by multiplying the scintillation measurement by a factor of 0.84 for the solid phase and 0.87 for the liquid phase. Correlation between repeat studies was 0.97 and 0.96 for solids and liquids, respectively. The application of a hand-held probe technique provides a noninvasive and inexpensive method for accurately assessing solid- and liquid-phase gastric emptying from the human stomach that correlates well with the use of a gamma camera, within the range of gastric emptying rate in the normal individuals in this study.

A rational approach to the assessment of aluminium solubility controls in freshwaters.

The problem of assessing aluminium equilibrium solubility controls in freshwaters using thermodynamic techniques is placed in a statistical hypothesis testing framework. As an aid in interpreting field information, artificial data are generated which correspond to ideal equilibrium conditions, but which incorporate realistic analytical and temperature related uncertainties. These artificial data are then subjected to thermodynamic interpretations. The technique is useful in answering "what if" questions by showing the scatter to be expected in real data under true equilibrium conditions. Aluminium hydroxide is used as the example in this report. Reasons for discarding the conventional dependent axis plot (pAl3+ vs pH) in favour of a plot with independent or nearly independent axes are emphasized. An improved method is suggested allowing for temperature variations in the equilibrium constants when constructing stability diagrams. Also, the need for caution when regressing variables which both contain errors is noted.

Fenestration of tenosynovial capillaries.

A proportion of superficial capillaries in joint synovium are known to be fenestrated. The specialized functions of these fenestrated capillaries are unknown. We report the finding of fenestrae in tenosynovial capillaries from rabbit peroneal tendon sheaths. Fenestrations occupied 0.66% of the endothelial surface. Fenestrated capillaries appear to be a general feature of synovium, rather than a specific feature of joint tissue.
