Antibodies to hn-RNP protein A1 in systemic lupus erythematosus: clinical association with Raynaud's phenomenon and esophageal dysmotility.

Antibodies to recombinant hn-RNP protein A1 were found by ELISA in sera from 26 out of 67 unselected patients with systemic lupus erythematosus. A higher number of anti-A1 positive patients had Raynaud's phenomenon (50% vs 7%) and esophageal dysmotility (42% vs 5%) than the anti-A1 negative patients. All 8 patients with both Raynaud's phenomenon and esophageal dysmotility had a positive anti-A1 assay. No association was found with other clinical findings, nor with disease activity and treatment regimes. Anti-A1 antibodies did not correlate with anti-RNP and anti-Sm antibodies, which were present in 30% and 12% of the anti-A1 positive cases and in 22% and 7% of the anti-A1 negative cases, respectively. Our results indicate that antibodies to hn-RNP protein A1 may be associated with a subset of SLE patients with clinical features overlapping those of progressive systemic sclerosis and quite distinct from the group identified by anti-RNP antibodies.

[Respiratory complications in 44 patients with acquired immunodeficiency syndrome].

Observation was carried out of 44 patients affected with pulmonary pathology during the course of AIDS, each of whom presented a severe respiratory insufficiency, admitted to the Clinic of Infectious Diseases between May 1987 and March 1989. In the patients suffering from their first respiratory infection, a lower mortality rate was observed (11/28, 39.2%) compared with the patients suffering from a second or successive infection (10/16, 62.5%). In sixteen cases, the etiological agent was Pneumocystis carinii while in 14 subjects it was impossible to perform bronchoscopy due to particular conditions of the respiratory apparatus and diagnosis was made according to CDC clinical criteria. Several parameters were furthermore evaluated (age, duration of the symptoms prior to admittance, LDH, PaO2, WBC) as potential prognostic indices; at the conclusion of the study, no statistically significant differences were found, however, between the group of survivors and the deceased. For the specific anti PCP therapy, a great variety of drugs were administered; among them, first choice was given to cotrimoxazole. In particularly critical patients, methylprednisolone was added. In 21 patients, a mechanical respiratory aid (C-PAP) was applied with favourable results in 16 of them.

[Neonatal hypothyroidism screening. 9 years' experience at a peripheral hospital].

Since 1979 in our Hospital 2500 newborns have been screened for C.I. by T4 and, afterwards, by TSH test. One case of C.I. was detected with an apparently higher frequency than in Lombardia and Italy. Even in a peripheral hospital, the screening for C.I. is useful for patients and the society and it has to be extended all over the country.
