This study was undertaken to better understand the metabolic fate of dietary isoflavones in humans. Twelve volunteers were challenged with soya flour and urinary diphenol levels were then determined by gas chromatography (GC) and gas chromatography-mass spectrometry (GC-MS). The presence of previously described urinary diphenols was confirmed, i.e. the isoflavones, daidzein and genistein; the isoflavonoid metabolites, equol, dihydrodaidzein (Int-O-D), O-desmethyl-angolensin (O-Dma); the lignan, enterolactone. Diphenols detected for the first time were the isoflavone, glycitein and five novel isoflavonoid metabolites which are tentatively identified as 6'-hydroxy-O-desmethylangolensin (6' OH-O-Dma), dihydrogenistein (Int-O-G), dehydro-O-desmethylangolensin (dehydro-O-Dma) and two isomers of tetrahydrodaidzein. Urinary excretion rates of the three isoflavones (daidzein, genistein, glycitein) over a 3-day period following soya challenge showed moderate variation (4x, 6x and 12x, respectively) between the 12 individuals suggesting some individual variabilities in ability to deconjugate and to absorb dietary isoflavones. However, urinary excretion rates of each of three major isoflavonoid metabolites (equol, O-Dma, 6' OH-O-Dma) showed more marked variation (922x, 17x, 15x, respectively); while some of this variability may reflect varying individual ability to ferment dietary isoflavones per se, an inverse relationship was found between urinary levels of equol and both O-Dma and 6' OH-O-Dma suggesting individual variability in the preferred metabolic pathways of dietary isoflavones.
The DNA-binding factors encoded by the E2A gene, which include the basic helix-loop-helix proteins E12 and E47, are thought to regulate Ig gene expression. In the rat these factors are products of the Pan locus and are referred to as Pan-2 and Pan-1, respectively. To determine when during B cell differentiation Pan (E2A) DNA-binding proteins are first expressed, cells from a newly developed rat long term lymphoid bone marrow culture system were analyzed molecularly and phenotypically using a newly defined anti-Pan mAb. The data indicate that Pan-expressing cells appear in B cell progenitors before the appearance of the CD45R B lineage-associated Ag or microH chain protein. The results also indicate that Pan proteins are present in cells that had undergone D-JH rearrangements. Finally, although Pan (E2A) proteins are present in myeloid precursors, expression is extinguished as differentiation into mature myeloid cells occurs.
OBJECTIVE: To assess the benefits of gastric tube feeding in an elderly community hospital population. SUBJECTS: One hundred consecutive patients who required feeding gastrostomies from July 1984 through June 1987. SETTING: Durham (NC) Regional Hospital, a 380-bed community hospital. METHODS: Patients were evaluated using a quality of life scale (QL scale) adapted from Spitzer's QL Index. The evaluation was based on hospital records at the time of tube placement and interviews with patients or family members at follow-up between June 1991 and March 1992. Subjective evaluation of the benefits of gastric tube feeding were obtained in interviews with patients or their families at follow-up. RESULTS: Overall there was no significant change in the objective evaluation of quality of life at follow-up. Men, patients over 76 years of age, and patients with chronic illnesses such as multiple strokes or dementia showed the poorest response on the QL scale. Subjective evaluation by patients or their family members was positively correlated with objective evaluation on the QL scale. Family members of patients who showed the poorest response on the QL scale were more likely than other family members to respond no to the question, "Would you want this done to you if you were in his/her situation?" CONCLUSIONS: Our QL scale provides a good indication of patients' and family members' subjective evaluation of the benefits of gastrostomy tube feeding after 4 to 8 years. Thus, the scale should be helpful to physicians who must consult with patients and their families and make decisions about the use of this procedure. The significant discrepancy between family members' evaluations of the benefit of the procedure to the patient and their refusal of the procedure for themselves if they were in the patient's situation confirms the need for advance directives and the importance of conscientious implementation of the Patient Self Determination Act of 1990.
Seventeen rats with lesions involving the sensorimotor forepaw and hindpaw regions were tested for the ability to run along a narrow bridge without slipping and to hold on to a small-diameter dowel suspended above the ground. Nine of the animals received the calcium channel blocker, nimodipine, once per day for the first 2 weeks after surgery, while the remaining eight rats received vehicle alone. Repeated measurements over the first 3 postoperative weeks showed deficits among the rats with lesions on both tasks. The animals treated with nimodipine, however, showed much better recovery, especially after the initial testing sessions.
This research investigated the potential for contamination of fabric by aldicarb (Temik 15) pesticide. Fabric soiled with synthetic perspiration or synthetic sebum was contaminated with Temik granules for various time periods and extracted with methanol. Extracts were analyzed with high performance liquid chromatography. The aldicarb transferred to both soiled and unsoiled fabric; however, the sebum soil and longer contact time increased the transfer of aldicarb to fabric. The dissipation of aldicarb from fabric was affected by the soil type and the formulation of the Temik granule. It is apparent a potential for dermal exposure to granular pesticide exists when the pesticide is trapped in pockets or folds of clothing.
The elucidation of the enzymatic processing mechanism associated with the formation of antigenic peptide fragments that combine with MHC class II molecules is fundamental to our understanding of the immune system. We have investigated a structurally well defined protein, recombinant human growth hormone (rhGH), as an antigen, and present data supporting the hypothesis that the enzyme cathepsin B can produce peptide fragments bearing T cell epitopes associated with lymphocyte proliferative response to hGH in Balb/c (H-2dhaplotype) mice. Minimal T cell epitopes are not generated; rather the cathepsin cleavage sites flank the three antigenic peptide regions, amino acid residues 31-41, 81-100, and 166-181.
The number of physicians who provide ambulatory care for patients with skin diseases is rapidly increasing. Current and proposed changes in health care financing may limit direct access to dermatologists. We provide recent data on volume of visits for cutaneous diagnoses, including the types of medical practitioners who provided these ambulatory services, and compare these data with earlier surveys that span 15 years. In 1989, dermatologists saw 41% of the 51.6 million persons with cutaneous diagnoses. The number of visits per dermatologist declined significantly from 1974 to 1989 (p < 0.05). Visits to dermatologists were significantly more likely to be reimbursed by Blue Cross/Blue Shield or self-pay than visits to other physicians for skin problems. From 1974 to 1989, the demand for dermatologic services from dermatologists has not increased as rapidly as the supply of dermatologists. Our data also suggest that restrictions on direct access to dermatologists or changes in reimbursement may substantially decrease the demand for services provided by dermatologists.
The purpose of this prospective trial was to determine the changes in position and size of the mandible in children treated with either the Fränkel function regulator or Harvold activator. Forty-two 10- to 13-year-old children with Class II, Division 1 malocclusions were matched in triads according to age and sex and randomly assigned to either control, Fränkel function regulator, or Harvold activator groups. There were no statistically significant differences between the groups at the beginning of the study. After 18 months, significant increases in gonial angle and articulare-pogonion length in the Harvold group were attributed to a change in the location of articulare because the condyles were positioned downward and forward at the end of treatment. The main effects of both appliances were to allow vertical development of the mandibular molars and increase the height of the face. The Harvold appliance also proclined the lower incisors and increased mandibular arch length. We could find no evidence to support the view that either appliance was capable of altering the size of the mandible.
A monoclonal antibody (Yae) was characterized and shown to specifically recognize E2A proteins in vivo, including the E2A-Pbx1 fusion gene products, p77E2A-Pbx1 and p85E2A-Pbx1. E2A proteins of a predominant molecular mass of 72 kDa, which comigrated with in vitro-produced rat E12 and and rat E47, were detected in human pro-B, pre-B, mature B, and plasma cell lines. The Yae antibody detected an E2A-containing microE2 enhancer element-binding complex (BCF-1) in pre-B- and mature B-cell lines in electrophoretic mobility shift assays which displayed a migration rate similar to that of in vitro-produced rat E12 and rat E47. A new E2A-containing microE2-binding species (P-E2A) was identified in plasma cells by using electrophoretic mobility shift assays. E2A proteins were detected in pro-B cells but were unable to bind the microE2 site. These observations suggest that the microE2 site is the target of stage-specific E2A regulatory complexes during B-cell development. Immunostaining analyses demonstrated the predominant nuclear localization of E2A proteins. Finally, we have identified an E2A form, designated I-E2A, which is unable to bind DNA. Our observations demonstrate novel in vivo mechanisms for the regulation of transcription by E2A proteins during B-cell development.
The effects of substitute adult male role-models were examined for 60 adolescents between the ages of 13 and 18 years. Significant mean differences were found on Depression, Hypochondriasis, and Suspiciousness scales of the MMPI and Guilt and Assaultiveness of the Buss-Durkee Hostility Inventory. Interpretation of scores showed that fatherless boys who had substitute male role-models were similar in personality to boys with fathers. Adolescent boys without fathers and from lower socioeconomic families were significantly more similar in personality to young offenders.
An analysis of the literature was the basis for a set of predictions regarding the Preschool Embedded Figures Test performance of a small, cross-sectional sample of 37 3- to 5-year-old children. The test scores were modestly reliable. Predicted age-related differences in scores for boys and girls were observed, including an interaction of age with gender; however, other predictions regarding those scores were not supported. Based on a small sample, it was tentatively concluded that the evidence for continued use of the Preschool Embedded Figures Test as a measure of field independence for young children was weak.

A three-center clinical evaluation of the use of ultrasonic instrumentation for cement removal during 90 revision total hip arthroplasties was undertaken. In conjunction with hand tools, cement removal was facilitated with ultrasonic tools. Superficial bone burns were seen in eight cases (9%). Bone perforations were created with ultrasonic tools in three cases (4%)--two followed perforations already created by hand tools in the proximal metaphyseal area. Only one perforation (1%) was made in the shaft of the femur. Ultrasonic instrumentation is a valuable adjunct for cement removal in revision total hip arthroplasty.

The context of agriculture in the Midwest, given many smaller, family run operations, presents multiple and complex challenges; occupational health intervention strategies used in other industries are not immediately applicable. 2. Public health nurses in Minnesota's Occupational Health Nurses in Agricultural Communities project (OHNAC) are applying the strategies of community wide health promotion through community organizing for farm health and safety promotion. 3. The theoretical basis for community wide health promotion encompasses theory from several disciplines, and a five stage model for community organization in health promotion has been developed and tested. 4. At the root of tis approach to farm safety is the effort to change long held beliefs and community norms, especially the belief that farm injuries and fatalities are a necessary part of agricultural work.

Previous studies have reported revascularization and reorganization of dental pulp chambers with periodontal tissues of monkeys following complete surgical transection through a portion of the apical roots. This study observed 128 teeth in four adult monkeys. Following surgical transection, the tissues were acquired by perfusion fixation, serially sectioned, and stained for cellular detail with hematoxylin and eosin. Collagen tissues were stained with Preece's trichrome and neural tissues with Rowles' silver cyanate for controlled impregnation. At 1 and 2 weeks the coronal tissues showed tissue disruption, necrosis, and degenerating nerves. The 3- and 4-week tissues that had been completely transected showed replacement healing of the pulp tissue with periodontal ligament connective tissue, but no nerves were present. At 6 weeks, no nerves were present in the coronal chambers of those teeth with complete vital transection. The 24-, 36-, and 52-week pulp chambers with complete transection failed to show nerve fibers in their reorganized connective tissues.
Thapsigargin is a potent inhibitor of the microsomal calcium-ATPase. We hypothesized that thapsigargin-induced liberation of calcium from sequestered intracellular sites could result in irreversible brain injury. To assess neurotoxicity, intracerebral injections of 0.1-100 nmol thapsigargin were performed in 7-day-old rats, and the severity of injury was assessed 5 days later by microscopy and measurements of lesioned areas. We found that thapsigargin elicited dose-dependent tissue injury in perinatal rodent brain.
