Components of an effective transitional residential program for homeless mentally ill clients.

UNLABELLED: Review of clinical records of 228 former clients of a Transitional Residential PROGRAM for severely, persistently mental ill homeless persons was conducted to examine PROGRAM results for completers and noncompleters. Of the 228 clients, 110 (48.3%) completed the PROGRAM: they became psychiatrically stabilized, found secure housing, and began receiving disability pensions. Completers participated in significantly more activities than noncompleters. Psychiatric diagnosis was unrelated to successful PROGRAM completion. Individuals who left against advice had significantly fewer previous hospitalizations.

The embryo freezing programme at Groote Schuur Hospital. First results.

'Surplus' embryos from 28 patients on the in vitro fertilisation programme at Groote Schuur Hospital were frozen for transfer in later cycles. Depending on the stage of the embryos, either propanediol or glycerol was used as a cryoprotectant. In 20 patients embryos survived the thawing procedure and were transferred. Five pregnancies resulted, 2 twin and 3 singleton. The pregnancy rate per embryo transfer in this group of patients in thus 25%. The pregnancy rate per freezing procedure is 19.2%.

Pregnancy without ovarian function. A case report.

A 38-year-old nulliparous woman suffering from premature menopause was stimulated with oestradiol valerate in order to create an artificial endometrial cycle. Three oocytes were donated by a woman on an in vitro fertilisation cycle and were inseminated with the patient's husband's sperm. These were then transferred into the patient's uterus after the endometrium had been primed with progesterone to change it to the secretory phase. Pregnancy resulted and proceeded well. Gestation was terminated at the 34th week by caesarean section and 2 healthy boys and 1 girl were delivered.

Amplification of rhinovirus specific nucleic acids from clinical samples using the polymerase chain reaction.

We describe a novel method for the detection of human rhinoviruses in clinical samples, using the polymerase chain reaction. Two synthetic oligonucleotide primers were produced that bind in the 5' noncoding region of all rhinovirus serotypes tested, about 350 nucleotides apart, and were used to prime polymerase chain reaction amplification of the intervening stretch of DNA. The product of this reaction, which can be clearly visualized by gel electrophoresis, is a discrete 380 bp band, the occurrence of which is diagnostic of the presence of a rhinovirus in the clinical sample analysed. The technique, which is rapid, sensitive, and reliable, has been used successfully for all the different rhinovirus serotypes tested to date in our laboratory. However, the sensitivity of detection is greatly dependent on the inclusion of both tRNA and vanadyl complexes during the viral RNA extraction process. Using this technique, under optimal conditions, we were able to detect virus in clinical samples with titres as low as TCID50 10(2.5).

The complete nucleotide sequence of coxsackievirus A21.

We have determined the complete nucleotide sequence of coxsackievirus A21 (CAV-21), the first member of this enterovirus subgroup to be analysed in molecular detail. The sequence, which is 7401 nucleotides long, encodes an open reading frame of 2206 codons, preceded by a 5' non-coding region of 711 nucleotides and followed by a 3' non-coding region of 72 nucleotides plus a poly(A) tract. The most striking feature is the remarkable homology to the poliovirus (greater than 90% at the amino acid level) in the 3' part of the genome. The rest of the genome is much less homologous, suggesting that CAV-21 is a recombinant virus. Rhinovirus-like characteristics, including the length of the 5' non-coding region and a slight --U/--A imbalance in codon usage, may be related to the fact that CAV-21, like rhinoviruses, infects the upper respiratory tract. However, the sequence sheds little light on the molecular basis of the shared receptor specificity.

The nucleotide sequence of human rhinovirus 1B: molecular relationships within the rhinovirus genus.

We have determined the complete nucleotide sequence of human rhinovirus 1B and made comparisons with other rhinoviruses. Extensive homology was found with serotypes 2 and 89 but the similarity to serotype 14 was considerably less. Rhinovirus-specific characteristics have been noted, in particular the length of the 5' non-coding region and the pattern of codon usage, and these may be sufficient to define the rhinoviruses as a distinct genus rather than being considered as members of the enteroviruses as has been suggested previously.

The clinical features of the Cohen syndrome: further case reports.

We report the clinical features of six patients with the Cohen syndrome. The characteristic features include mental retardation, truncal obesity, prominent incisors, and tapering digits. Pelviureteric obstruction and epilepsy are reported as possible new features of this syndrome.

Diagnostic discrepancy in personal accounts of patients with 'schizophrenia'.

Five published accounts of patients with "schizophrenia" were reviewed in the light of the DSM-III diagnostic criteria. None of the accounts unequivocally met the DSM-III criteria for schizophrenia, but all did meet the criteria for some other psychiatric condition, especially affective disorders. The survey results suggest that popular accounts of "schizophrenia" present a confusing picture of schizophrenia. Inasmuch as these accounts often are invoked to support "cures," the reading pubic is doubly misled, as when a naturally remitting condition such as depression is mistakenly called schizophrenia. The psychiatric profession has a responsibility to clarify diagnostic misconceptions put forth in popular literature, and DSM-III might provide more objective guidelines for this much-needed clarification.

Circadian organization of thirteen liver and six brain enzymes of the mouse.

The activities of 13 liver and 6 brain enzymes were studied in 7-12 week old CD2F1 male mice that had been fed ad libitum and standardized either to 12 hours of light (0600-1800) alternating with 12 hours of darkness (1800-0600) (LD12:12); or to a reversed light-dark cycle (darkness 0600-1800; light 1800-0600) (DL12:12). Three separate studies were performed on two different days; in each experiment, subgroups of 14 animals were sacrificed at 3-hour intervals. Livers were assayed for: isocitrate dehydrogenase, glutamate dehydrogenase, lactate dehydrogenase, alcohol dehydrogenase, glutathione reductase, glyoxylate reductase, L-alanine aminotransferase, glutamate oxalacetate transaminase, pyruvate decarboxylase, fructose-1-phosphate aldolase, fructose diphosphate aldolase, fructose 1,6-diphosphatase, and fatty acid synthetase. Brains were assayed for phosphoglucose isomerase, adenosine triphosphatase, creatine phosphokinase, pyruvate kinase, adenylate kinase, and malate dehydrogenase. All 19 enzymes demonstrated a prominent circadian rhythm in at least one experiment. Moreover, each rhythmic variable showed a statistically significant fit to a 24-hour cosine (sine) curve by the method of least squares. In general, peak activities of the liver enzymes analyzed were associated with the beginning of the dark cycle and initiation of the animal's activity, while the group of brain enzymes had peak activities which occurred at the beginning of the animals' rest span and were near the beginning of the light cycle. The phasing of each of the rhythms could be reversed within a two-week span after reversing the environmental light-dark cycle 180 degrees.

Depression in family practice: long-term prognosis and somatic complaints.

Somatic pain, functional, and anxiety complaints of 154 depressed patients were followed during the course of their initial depression and were found to parallel the depression: these complaints increased in number just prior to diagnosis of depression and decreased to normal levels after one year's treatment of the depression. Persistence of these types of somatic symptoms after one year's treatment predicted eventual chronicity of the depression. Older patients were also more likely to develop chronic depressions, and there was some indication that those individuals who had an initial remission of a depression followed by a second depression which then became chronic had longer first depressions.
