Nasal dermoid sinus cysts and the role of open rhinoplasty.

All suspected congenital abnormalities of the nose require further evaluation. The nasal dermoid sinus cyst (NDSC) is one of the many midline nasal masses that often pose diagnostic and treatment dilemmas for the plastic and reconstructive surgeon. NDSCs are distinct from other facial dermoids in their potential for involving deeper contiguous structures, and intracranial extension. Accurate diagnosis and effective treatment are essential to avoid craniofacial skeletal deformation, cyst rupture, and infection that could cause cutaneous, ocular, or intracranial complications. A comprehensive discussion of the embryogenesis, pathogenesis, diagnosis, and surgical management of the NDSC is presented to delineate the role of open rhinoplasty in optimizing the management of this congenital nasal deformity.

Rare craniofacial anomaly: Tessier no. 2 cleft.

Four cases of facial cleft that fit the anatomic description of the rare Tessier no. 2 cleft, with two patients having the no. 12 cleft extending to the cranium as no. 2 clefts, are presented. In all patients, clinical expressions of the anomaly were different. Thus, diverse surgical procedures were used in all cases. These cases and review of the literature help to define the soft-tissue and bony course of these clefts, and also emphasize the role of three-dimensional computed tomography scan imaging to show the bony cleft route. The diagnosis and treatment plan of the no. 2 cleft as well as its cranial counterpart are discussed in this report.

A rare craniofacial cleft: bilateral Tessier no. 5 cleft accompanied by no. 1 and no. 6 clefts.

A case of facial cleft that fits the anatomic description of the rare bilateral Tessier no. 5 cleft accompanied by unilateral no. 1 and no. 6 clefts is presented, and brings the total number of reported cases to 21. This case and a review of the literature help to define the soft-tissue and bony course of these clefts, and also emphasize the role of three-dimensional computed tomography to show the bony cleft route. The diagnosis and treatment plan of the no. 5 cleft as well as other rare clefts such as no. 1 and no. 6 clefts are discussed.

Complications in craniofacial surgery: the Turkish experience.

The records of 71 patients who underwent 75 consecutive craniofacial procedures involving a transcranial component have been analyzed retrospectively to assess the incidence and type of complications encountered during the 14-year period of 1985 to 1998 at Ege University Hospital, Department of Plastic and Reconstructive Surgery. The results were compared with those of similar reports from other craniofacial centers. Operations were classified into six groups according to their type to evaluate effectively their complication rates. Complications were also graded in terms of their severity. Serious complications were seen in 12 patients (16.8%), whereas life-threatening complications were seen in 6 patients (8, 4). The mortality rate was 2.7%. It is determined that complication incidence was notably higher in the group that underwent monobloc frontofacial advancement. A decrease in the rate of serious complications occurred with increased experience with the surgery. The results of our study indicate that although craniofacial surgery carries an inherent risk for significant complications, the risk can be minimized and the rate of mortality and major complications kept to an acceptable level by a careful and experienced craniofacial team.

Isolated cremaster flap: A modified model in intravital microscopy.

Inspection of the microcirculation in living tissues has been accomplished using various models. The rat cremaster muscle has been used for this purpose for more than 20 years. In our study, for the first time, the pedicle of the cremaster muscle has been catheterised indirectly in order to perfuse and drain the muscle and to obtain blood samples. The assessment of the effects of a variety of perfusants on the microcirculation after reperfusion injury can be carried out by using this model.

Aplasia cutis congenita with epidermolysis bullosa: a case report.

A child with epidermolysis bullosa and aplasia cutis congenita is presented. This is a very rare condition that represents an unusual manifestation of epidermolysis bullosa. Management of aplasia cutis congenita is controversial and may be conservative, surgical, or a combination of the two. In this patient, surgical treatment with split-thickness skin grafts failed, and conservative treatment using silver-sulfadiazine cream dressings was instituted.

Modified McIndoe procedure for vaginal agenesis.

Between 1982 and 1997, 29 patients with congenital absence of the vagina underwent modified McIndoe vaginoplasty at the Division of Plastic and Reconstructive Surgery, Ege University Medical School, Izmir, Turkey. As a modification, an X incision was utilized instead of a straight-line horizontal or sagittal incision. During the first postoperative week, a perforated Pyrex rigid mold was used. This was replaced with an unperforated mold at the end of the first week. These patients' medical records were reviewed retrospectively. Complications encountered included infection, total lack of skin graft take, stress urinary incontinence, partial graft loss, and vaginal stricture. All complications were treated except the stress urinary incontinence, and the final results were satisfactory.

Anatomic variations of the infraorbital foramen.

The aims of this study were to locate the infraorbital foramen and to determine the frequency and location of any accessory foramen, which may be troublesome during anesthetization of this region. In 45 cadavers, the infraorbital foramen was dissected according to classic principles for location. The line between the angulus oculi medialis and the angulus oculi lateralis was divided into three equal pieces, and a second line was drawn downward, perpendicular to the point uniting the internal and medial thirds. The position of the infraorbital foramen was determined in relation to that line and the infraorbital margin. For the right and left sides, the infraorbital foramen was found to be on that line in 75.6% and 68.9% of the specimens, respectively. The infraorbital foramen was 10.9 mm and 8.3 mm under the infraorbital margin in men and women, respectively. Regarding accessory foramen, 119 crania and 229 maxilla were observed (a total of 467 infraorbital foramen), and it was found that single accessory foramen were present in 11.5% of specimens and double accessory foramen were present in 1.28% of specimens. In 79.6% of specimens with single accessory foramen, the accessory foramen was superior and medial to the main opening. These results are helpful in decreasing anesthetic complications.

A new heart failure model in rat by an end-to-side femoral vessel anastomosis.

OBJECTIVE: The aim of this study was to develop a new shunting procedure for producing heart failure in the rat incorporating microvascular techniques and avoiding an abdominal operation. METHOD: We performed an end-to-side anastomosis between the femoral vein and the femoral artery just proximal to their trifurcation into the saphenous, epigastric, and distal femoral vessels. RESULTS: Of the 15 rats which underwent this procedure, six died within 48 h. The nine surviving animals were sacrificed and examined six weeks following surgery. All nine had developed cardiac hypertrophy and cardiac failure. CONCLUSION: This model provides a relatively simple and reproducible means of creating high output heart failure and cardiac hypertrophy in the rat without necessitating abdominal surgery.

Arterialization of the venous system in a rat lower limb model.

Creation of alternative vascular conduits has been one of the most challenging subjects in the history of vascular surgery. In the past, afferent arteriovenous fistulas have been used by vascular surgeons to rescue ischaemic extremities. We have undertaken a comprehensive investigation to characterize arterialization of the venous system at different levels of the rat lower limb. In Group 1 (n = 28), we performed an end-to-end anastomosis between the proximal saphenous artery and the distal saphenous vein (Superficial venous system), and ligated all other branches of the femoral artery. In Group 2 (n = 28), we performed an end-to-end anastomosis between the proximal saphenous artery and the distal femoral vein (Deep venous system). In Group 3 (n = 24), the control group, the femoral artery was ligated proximal to its trifurcation into the saphenous, epigastric, and distal femoral vessels. Group 4 (n = 10), the normal group, underwent no surgery. Animals were sacrificed at the first, third, and eighth postoperative weeks. Arteriographic, India ink injection, latex injection, and histological studies were performed on all groups. Successful arterialization of veins, significant neovascularization, and less ischaemic injury of muscles were noted in the arterialized vein groups (Groups 1 and 2) when compared with the ligated group (Group 3). This model appears to depict successfully arterialization of the lower limb venous system in a small animal model.

New simplified radio-opaque injection technique for visualization of rat arteries.

We have developed a new and simplified injection technique based on a mixture of latex and lead oxide. This injection technique is unique in its possession of both color and radio-opacity. We have developed and refined this methodology in rats, and have demonstrated it to possess superior qualities of injectability. Depiction of small vessels is highly detailed both radiographically and grossly.

Effect of vein grafting on the survival of microvascularly transplanted muscle flaps.

Since vein grafting is often required during transplantation of free muscle flaps but is associated with a higher failure rate than those flaps transplanted with primary anastomoses, we sought to compare primary repair with the use of vein grafting in an experimental setting. We transplanted the gracilis muscle to the contralateral side in 98 rats using four different methods of vessel repair. In the Control group (n = 28), both femoral vessels were anastomosed primarily. In Experimental Group 1 (n = 25), the femoral artery was anastomosed with an epigastric vein graft and the femoral vein was anastomosed primarily. In Experimental Group 2 (n = 25), the femoral vein was anastomosed with a femoral vein graft and the femoral artery was anastomosed primarily. In Experimental Group 3 (n = 20), both femoral vessels were anastomosed with vein grafts. The Control and Experimental Groups 1-3 survival rates were 89.3, 76.0, 84.0, and 70.0%, respectively; none of the experimental group survival rates was significantly different from that of the control (P < 0.5). This study demonstrates that the use of size-matched, interpositional vein grafts in the arterial or venous pedicle of the rat gracilis muscle flap during transplantation did not significantly decrease flap survival as compared to primary arterial or venous anastomoses. If the observed failure rate persisted with expansion of the study groups to 60-100 animals each, the failure rate of flaps with vein grafts would be significantly lower and comparable to failure rates reported in some clinical series. The large numbers necessary to significantly show this decrease make this model impractical for further studies.

Marjolin's ulcer of the scalp: report of 5 cases and review of the literature.

The formation of neoplastic changes in the scar tissue of chronically ulcerating wounds is a well-known process. This condition is most commonly seen after the postburn scars, but it may be seen after many kinds of scars. The term "Marjolin's ulcer" is used to describe this type of carcinoma. Although many different cell types can be seen in these lesions, the most commonly seen is squamous cell carcinoma. Squamous cell carcinomas resulting from the Marjolin's ulcer have a much greater tendency to metastasize than squamous cell carcinomas resulting from the other causes. Confusion continues about the precise pathophysiology of this lesion and the clinical behavior of this neoplasm, and the mortality and morbidity rates are also conflicting. As would be expected, there is a wide variety of suggested treatment protocols for this disease. This article, through case reports and review of the literature, offers criteria for the treatment of the Marjolin's ulcers that arise on the scalp, which is an uncommon site.
