Healing large tympanic membrane perforations using hyaluronic acid, basic fibroblast growth factor, and epidermal growth factor.

Large tympanic membrane perforations usually require a surgical tympanoplasty for closure. Reducing surgical costs and risks has encouraged investigators to examine nonsurgical office procedures for healing these perforations. Growth accelerators are the most promising agents. We study here the closure of large acute perforations using weekly applications of 1 mg of 1% hyaluronic acid (HA), 0.4 microg basic fibroblast growth factor (bFGF), or 1.0 microg epidermal growth factor (EGF) directly to the tympanic membranes of the experimental ears. Control ears were treated with 0. 1 mL Vasocidin. Complete closure was obtained in 100% of the ears treated with HA and EGF and 85.7% of those treated with bFGF by day 21, compared with 63.6% of the controls by day 32. Moderate-to-severe ipsilateral and contralateral external canal hypertrophy was noted in 14.2% and 37.5% of the ears treated with bFGF and HA, respectively, but was not seen in ears treated with EGF or in the control group.

Evidence for characteristic vascular patterns in solid tumours: quantitative studies using corrosion casts.

The vascular architecture of four different tumour cell lines (CaX, CaNT, SaS, HEC-1B) transplanted subcutaneously in mice was examined by means of microvascular corrosion casting in order to determine whether there is a characteristic vascular pattern for different tumour types and whether it differs significantly from two normal tissues, muscle and gut. Three-dimensional reconstructed scanning electron microscope images were used for quantitative measurements. Vessel diameters, intervessel and interbranch distances showed large differences between tumour types, whereas the branching angles were similar. In all tumours, the variability of the vessel diameters was significantly higher than in normal tissue. The quantitative data provide strong evidence for a characteristic vascular network determined by the tumour cells themselves.

The inherent cellular sensitivity to 62.5 MeV(p----Be+) neutrons of human cells differing in photon sensitivity.

The inherent sensitivity of 20 human cell lines to the 62.5 MeV(p----Be+) clinical neutron beam at Clatterbridge, UK, has been assessed and compared to their sensitivity to 4 MeV photons. The survival curves of the cell lines following neutron irradiation were curvilinear, and the inherent neutron sensitivity varied by 4.5 fold (0.1 survival level) between the extreme values, in the cell lines studied. There was a strong correlation between the sensitivity of these human cells to photon and neutron irradiation. It was concluded that should these in vitro patterns occur in the clinic, the 4-fold variation in RBE and inherent sensitivity to neutrons could result in overall lower local control rates following fast neutron therapy than might be anticipated. It suggests the need for the development of predictive assays as a potential means of selecting tumours most appropriate for neutron therapy.

Lung radioprotection by WR-2721 at low X-ray doses per fraction.

The response of mouse lungs to single doses and ten fractionated irradiations has been tested using breathing rate and lethality as assays for damage. The radioprotective effect of 300 mg/kg WR-2721 has been determined for mice breathing air or 10% oxygen. The protection factor was assessed from dose response curves obtained at monthly intervals from 24 to 48 weeks. A low protection factor (1.2-1.4) was observed for single doses in air or 10% oxygen and also for ten fractions in air. Considerably more protection was seen with ten fractions in mice breathing the reduced oxygen concentration (protection factors of PF = 1.5-1.7). It is postulated that the low PF values normally reported for lung are due to the naturally high oxygen concentration in all cells in this tissue. A fraction of the cells becomes sufficiently hypoxic in 10% oxygen to be susceptible to WR-2721 radioprotection. This subpopulation can then be detected with small X-ray fractions (less than or equal to 5 Gy) but not with large single doses.

VIII nerve response to click stimuli in normal and pathological cochleas.

A flat 30--50 dB hearing loss was established in chinchillas following a 5 day exposure to an octave band of noise (354--708 Hz, 95 dB SPL). After exposure, single auditory nerve fiber recordings were obtained using click and tone burst stimuli. The thresholds of units from the noise-treated animals were elevated 30--70 dB and the tuning curves were abnormally broad. At the threshold for click stimulation, the fiber latencies were shorter in the noise-treated animals than those in normal animals. However, the latencies for the two groups were similar when stimulated at the same intensities. As indicated by the number of peaks in the PST histograms obtained with clicks, the units from the noise-treated animals showed considerably more damping in the neural response than those from normal units. The temporal spacing between the peaks in the histograms for units of similar CF was the same in the normal and noise-treated groups, although this cannot be taken to infer that an individual unit PST histogram would remain the same after noise exposure as before. These limited neural data therefore show changes in the same direction as those in the transient mechanical response of the basilar membrane reported by Robles et al.

The prevalence of middle ear disease among learning impaired children. Does a higher prevalence indicate an association?

The prevalence of middle ear disease among children who are educationally classified as learning disabled as compared to that found in normal children is discussed. A higher incidence of middle ear pathology was found among the learning impaired children. The potentially hazardous effects that middle ear disease can have upon academic and linguistic development are emphasized.
