Preaortic paraganglioma mimicking a hypervascular tumor of the pancreas.

Paragangliomas are rare tumors that arise from neuroepithelial cells. They are most frequently located in the para-aortic region and they may be confused with other retroperitoneal tumors, especially pancreatic tumors. We present a case of a secreting preaortic paraganglioma in a young patient which was mimicking a hypervascular tumor of the pancreas, and that was completely resected 5 years after the failure of a first attempt to remove the tumor.

[Diagnostic and therapeutic strategy in carcinoma of the extrahepatic bile ducts].

The preoperative diagnostic approach in 25 patients with bile duct carcinoma was studied. Investigation procedures included gray-scale ultrasonography with guided fine needle biopsy of focal lesions, percutaneous transhepatic or retrograde cholangiography, transhepatic biliary drainage with brush and forceps biopsy of biliary stenosis as well as selective angiography for feasibility of surgical resection. Diagnosis was ensured in 23 patients by cytologic or histologic tissue specimens. Twenty-one lesions were judged unresectable on the basis of bile duct or vascular tumor extension and patient status. These patients were treated either by iridium 192 wire radiation therapy (6 cases) or by nonsurgical biliary drainage. Four patients with a potentially resectable tumor underwent surgery. This preoperative diagnostic approach can be recommended for selecting appropriate therapy in patients with bile duct carcinoma.

[Retention jaundice caused by central hepatic hepatoma. Treatment with Kron's extra-anatomic biliary prosthesis].

A 31 year-old patient presented with a retention jaundice from a centrally located hepatoma invading the hilum. Because of the patient's age and the severity of the pruritus and jaundice, palliative treatment was performed by percutaneous catheterization of the intrahepatic biliary tracts to drain the right and left hepatic canals. As the hilar stenosis could not be overcome, the external drainage was transformed into internal drainage by implantation of a Kron's biliary prosthesis linking the intrahepatic biliary tracts, following segment III hepatotomy, to the duodenal lumen, with burying of the prosthesis in the gastric antrum region. Jaundice was reduced until death of the patients from metastases 6 months later. This case demonstrates that the use of Kron's biliary prosthesis to perform a biliodigestive shunt from intrahepatic biliary pathways is a valid palliative procedure in patients with limited life expectancies.
