In this micro-scale method for liquid-chromatographic measurement of cimetidine in 250-microL volumes of plasma or gastric fluid, a combination of organic- and aqueous-phase extractions, and protonation of the internal standard and cimetidine, enabled us to detect 2.0 ng of cimetidine on the column. We also used a radial compression module, which reduced the retention times for cimetidine and the internal standard to only 2.78 and 2.00 min. The speed and sensitivity of this method facilitates analysis of large numbers of samples.
Clinicians often find that the application of pharmacokinetic principles for the calculation of a drug regimen is bewildering and difficult. In this article it is shown that clinically useful calculations can be simply done at the bedside using a handheld calculator. The only requirement is an understanding of the terms that define the pharmacokinetic behavior of a drug. Appropriate manipulation of these terms will allow calculation of loading and maintenance doses, peak and average plasma concentrations, and other useful pharmacological data. The result will be improvement in drug therapy.

5-Fluoro-5'-O-nitro-2'-deoxyuridine (FdUMN), a neutral isostere of 5-fluoro-2'-deoxyuridine 5'-monophosphate, inhibited the growth of L1210 cultures. The inhibition of L1210 cultures by FdUMN was prevented by thymidine, but not by 2'-deoxyuridine. Like 5-fluoro-2'-deoxyuridine (FdUrd), FdUMN inhibited the incorporation of 2'-deoxyuridine into DNA, but the onset of this inhibition was not immediate, as was seen with FdUrd. FdUMN did not inhibit the activity of purified thymidylate synthetase from Lactobacillus casei and was a poor inhibitor of thymidylate synthetase activity in homogenates of L1210 ascites cells. However, after incubation with homogenates of these cells and subsequent addition of ATP, FdUMN inhibited this enzyme effectively. These results indicate that intracellular activation of FdUMN is required for its inhibition of thymidylate synthetase.
Twenty mildly retarded adults were matched for chronological age and sex with twenty non-retarded children using the Peabody Picture Vocabulary Test. All 60 subjects were given an 89-item Fear Survey Schedule with a three-point rating scale. Mildly retarded adults were more fearful in general and showed a more intense level of fear than their chronological age matched controls. However, they reported less overall fear than their mental age matched controls. Differences in type of feared objects or situations were reported between groups. The importance of understanding the degree and types of fear in promoting de-institutionalization in the retarded is stressed.
