[Efficacy and reversible nephrotoxicity of cefsulodin during treatment of septicemia from Pseudomonas aeruginosa infection of a pacemaker].

A case of P. aeruginosa septicaemia originating from a pace-maker electrode is described. Cure was obtained by surgical ablation of the contaminated electrode by a right thoracotomy and monotherapy with cefsulodin, an antibacterial agent. Nephrotoxicity was observed during treatment and regressed after its withdrawal. No other potentially nephrotoxic drug was used. The therapeutic indications of new beta-lactamase stable cephalosporins are reviewed.

[Gastric eosinophilic granuloma : value of endoscopic polypectomy. Apropos of 2 cases].

Two ulcerated, sessile, gastric eosinophilic granulomas, measuring 5 and 10 mm in diameter, were diagnosed by endoscopic polypectomy (accurate prior forceps biopsy had not established accurate diagnosis). The patients, aged 76 and 74 years, had anemia and epigastric pain respectively. None of the usual etiologies (parasitic, chemical, traumatic and ulcerous) were found. Survival, clinical symptoms and location were similar to data reported in the literature, which are briefly reviewed. The value of endoscopic polypectomy is emphasized. With this procedure, pedunculated forms can be treated and sessile forms diagnosed, thus avoiding a useless gastrectomy and its hazards in these elderly patients.

[The Duffy blood group in migrants of French-speaking Africa].

The Duffy blood types survey in 544 subjects coming from French speaking African countries, shows the high percentage of subjects having a Duffy a- b- blood type (94%). It ought to be emphasized that this predominantly Duffy a- b- population resides in areas in where there is according to the WHO data, a very low incidence of Plasmodium vivax invasion. Moreover, we have taken for another example North Vietnam, an area where there is a large Plasmodium vivax invasion and where we have found a small percentage of Duffy negative subjects (5.6%). In agreement with many authors reporting an association between Duffy negatively and vivax malarial refractoriness, this survey demonstrate that in French speaking African areas where there is a very low incidence of Plasmodium vivax invasion we have found, as expected, a high rate of Duffy negative subject.

[Radiological aspects of the duodenum and small intestine in severe strongyloidiasis. Apropos of 4 cases].

Four cases of severe strongyloidiasis in Martinique patients are reported. Clinical features were typical of the affection, and the outcome was fatal in two cases. Fibroscopy findings were negative but larvae were demonstrated in biopsy specimens in 3 out of the 4 cases. A barium meal follow-through examination showed the different radiological images of the infestation: non-specific parasitic duodenitis and pseudo-stenosis of D3 with duodenal dilatation, barium reflux into the main bile duct, and absence of jejunal folds, these images being very suggestive of strongyloidiasis.

[Early syphilitic hepatitis. Two cases].

Two patients are presented as cases of secondary syphilitic hepatitis. Clinical and biological signs coincided with usual description of this affection. Syphilitic serological tests were all very positive. These two patients were homosexual men. In one case, we have found treponema in hepatic biopsy, underelectron microscope only, which confirmed the diagnosis. Evolution of both patients was improved by anti-syphilitic penicillin the rapy. Thus, this is an important diagnosis, which should not missed, because of the good prognosis of this affection with specific treatment, and because of the increasing frequency of syphilis for 20 years.

[Anti-I anti-erythrocyte auto-immunization in malaria].

An anti-erythrocyte autoimmunization with anti-I specificity was shown in patients previously exposed to malaria (falciparum malaria in 98.7% of cases). An anti-I antibody was eluted from red cells in 14.6% of 41 patients with an acute P. falciparum infection. The frequency of the anti-I immunisation was found to be correlated to the rate of the antimalarial antibodies among the 152 patients without parasitaemia, coming from areas endemic for falciparum malaria: this frequency was 5.2% for a rate below 1/64, 9.7% for a rate between 1/64 and 1/256, and 34% for a rate above 1/1,024. Anti-I antibodies were more often noted in African patients coming from forest areas, considered as chronically infected with P. falciparum, and in other patients who have been in France for less than a month. No anti-I antibodies were found in the control group, composed of 52 Caucasian healthy subjects with no previous exposure to malaria. This anti-I immunisation was transitory and its disappearance was usually accompanied by a lowering of anti-malarial antibodies.

[Value of the string test in the diagnosis of duodenal parasitosis].

Diagnosis of small intestine parasitosis can be realized by string-test. This examination, easily performed, does not produce side-effects in ninety percent of cases. The main problems are: good string unrollment and duodenal passing over. When Necator ova or gardia cysts are found in the stools by the Junod's technic, string-test is always positive. In one case, this latter even showed Schistosoma mansoni ova. Among 18 patients with unexplained hypereosinophilia, in spite of a parasitologic check up involving three stools examinations, the string-test showed Necator ova. With regard to strongyloidiasis, when properly performed, string-test is positive in seventy percent of cases. Therefore, this technic seems very useful on the field or in laboratories which do not usually realize the Junod's technic.

[Human basophil degranulation test in the diagnosis of anguilluliasis. Preliminary results].

The human basophil degranulation test was applied to the diagnosis of strongyloidiasis. The preliminary results show an excellent response. 33 of the 35 cases give a definite positive result and in the 15 control cases, with no parasitosis or an other helminthiasis, the test was negative. If these results are confirmed by a more important study of sensitivity and specificity, the test would be of interest when the parasitosis is of difficult evidence and especially by patients needing immunodepressive therapy.
