[Demonstration by lymphocyte transformation test of the allergic genesis in a case of acute hepatitis].

After accidental re-exposure to metamizole a 50-year-old patient developed severe jaundice within five hours (total bilirubin 43.9 mg/dl) with a rise of GOT to 147, of GPT to 222, of gamma-GT to 380 and of alkaline phosphatase to 497 U/l. The allergic genesis of this reaction was demonstrated in the lymphocyte transformation test which, in the presence of the causative substance and its metabolites, was performed five times during a period of 232 days. The first three tests were positive with stimulation indices up to 2.3. Sensitization to the metabolite 4-aminoantipyrine was demonstrated: this is known to be the first of four metabolites formed within 1-1 1/2 hours and constituting 40% of the four metabolites in the blood.

[Arthroses of the radial ray of the wrist joint with special reference to the correlation between trauma and arthrosis].

Incidence of arthrosis in the region of the hand-joint and carpal bones has proved to be substantially higher than generally assumed. This was established by X-ray analysis in more than 80 per cent of all cases investigated. The changes thus recorded were found to be related to sex and age. Studies were conducted into the relationship between arthrosis and previous trauma.

[Pheochromocytoma of the urinary bladder].

Pheochromocytoma is a very rare tumor of the urinary bladder. In this report two pheochromocytomas are shown with symptoms of a typical vesical tumor and without typical pheochromocytoma symptoms.

[Long-term results of compression-osteosynthesis with three AO-screws for spongiosa in fractures of the femur-neck (author's transl)].

Between 1969 and 1978 in 42 patients fractures of the femur-neck were treated with three AO-screws for spongiosa (screws with a short thread). Late results in these patients are reported. The interval between operation and follow-up was in average 90 months. In no case pseudarthrosis had developed; the rate of necrosis of the femur head was 23.8%. The method is suitable in selected cases of medial fractures of the femur-neck, type I and II, and in lateral fractures, preferably in younger patients. The results in fractures of type III according to Pauwels show, that in advanced age these fractures should be treated with a partial or total prosthesis, because the rate of necrosis of the femur-head cannot be lowered.

[Simultaneous bilateral rupture of the quadriceps (author's transl)].

Simultaneous bilateral rupture of the quadriceps has several causative pathogenetic factors. On the one hand there is diabetes with macro- and microangiopathy, on the other hand acute and chronic inflammatory processes such as spotted fever and leptospirosis, dysentery and hepatitis are worth mentioning. It is also interesting that the history contains statements on an Osgood-Schlatter disease in the left knee and a suspected meniscus lesion during the war years. The daily bending of the knee by this relatively old patient must be seen as a chronic lesion of over-strain.

[Incarcerated Chilaiditi disease. A rare indication for surgery].

Surgery was performed on a rare case of incarcerated Chilaiditi's syndrome, a syndrome that has been known for 10 years. The patient was in the state of ileus with volvulus of the stomach and hepatodiaphragmatic interposition of the stomach, transverse colon, and omentum majus. The patient was at high risk--77 years old and in bad condition due to hyperemesis and abdominal pain. Preoperative intensive care was necessary. During the operation the intestinal structures were repositioned, the diaphragmatic hernia was resected, and the muscular parts were sewn tight. The right position of the liver was reconstructed by sewing the ligaments. Colon resection was inevitable because the colon loop could not be separated and because there was lack of intraabdominal space. For indication of preventive and elective surgery, we propose that the diagnostics be enhanced when Chilaiditi's syndrome is found by chance and is still asymptomatic. These techniques may include a complete radiological examination with contrast medium, intestinal endoscopy, and computer tomography of the thoracoabdominal region.

[Causes of injury in hang-gliding].

It is reported about 36 hang-gliding accidents within a period of 2 1/2 years. Injuries of the upper and lower extremities predominate. The localization of the injuries is most striking; their origin is discussed. Most of the accidents are caused by three risk factors: pilot error, equipment failure, and weather conditions. Prevention of accidents is discussed.
