Memantine for treatment of moderate to severe Alzheimer's disease.

Health Canada has issued a Notice of Compliance with conditions (NOC/c) for memantine in the treatment of moderate to severe Alzheimer's disease (AD). The evidence of relative benefit and harm from memantine in this population derives from two randomized controlled trials (RCT) of 24 to 28 weeks duration, in a total of 656 patients; and a post hoc subgroup analysis of 79 patients with severe AD from a third trial of 12 weeks. Memantine alone or in combination with donepezil demonstrates improvements in primary outcome scores of activities of daily living and cognition, but not of global performance. Memantine's rate of diffusion may be rapid, as it is the only drug available for severe AD and it has a potential for use in unapproved indications.

Comparison of literature searches on quality and costs for health technology assessment using the MEDLINE and EMBASE databases.

Biomedical databases are an important source of information for health technology assessment. However, there is considerable variation in the costs of accessing commercial databases. We sought to measure the quality, amount of overlap, and costs of information retrieved from two of the main database sources--MEDLINE and EMBASE. Librarians at two health technology assessment agencies ran a total of eight literature searches on various medical technologies, using both databases. All search results were independently reviewed by two researchers. The researchers were asked to identify relevant references and to rank each of these according to a level of evidence scale. The results were tabulated to show the number of references identified by each database, the number of relevant references ranked by level of evidence, and the number of these references that were unique to one or the other database. The cost of retrieving references from each source was also calculated. Each database contained relevant references not available in the other. Because of the longer time lag for indexing in MEDLINE, many of the references that originally appeared to be unique to EMBASE were subsequently available in MEDLINE as well. Since our study was conducted, MEDLINE has been made available worldwide, free of charge, via the Internet. Hence, the cost difference between the databases is now even greater. However, notwithstanding the costs, it appears that literature searches that rely on only one or the other database will inevitably miss pertinent information.

Fifteen years of mammography in cosmetic surgery of the breast.

In a series of 1149 cases of cosmetic surgery of the breast performed from 1973 to 1989, early diagnosis of breast cancer in 34 cases was possible by relying mainly on the use of mammography for the diagnosis. It is strongly recommended that a policy of mandatory preoperative mammography be implemented so that all patients can be protected from a lethal disease that has a far better prognosis when detected early.

Outcome of mammary capsulotomies.

We reviewed the outcome of capsulotomies, both open and closed, in 134 patients with fibrous capsular contractures surrounding mammary implants. The rate of occurrence of fibrous capsules is given, along with other percentages for each type of augmentation. The study shows that the first attempt at a capsulotomy, either closed or open, gives a better result than the following attempts. After the third attempt, successful capsulotomies are either very rare or nonexistent. We therefore advise that after two or at most three unsuccessful closed capsulotomies, one should opt for an open capsulotomy. After two or at most three unsuccessful open capsulotomies, one should change to another approach.

Reduction mammoplasty by the total dermoglandular pedicle.

The use of a total dermoglandular pedicle in 700 cases of reduction mammoplasty are described by this team of authors. They emphasize that their modification of the dermoglandular pedicle provides an extremely reliable blood supply to the areola and is quick and easy to perform. There is no anesthesia of the nipple postoperatively, and the shape of the breast is favorable both in the immediate postoperative period and over the long-term follow-up observation between the years 1979 and 1984.

Immediate flap reconstruction after excision of basal cell carcinoma of the face.

The authors review 175 basal cell carcinomas of the face, treated primarily by surgery. The overall cure rate was 97%. Among these cases, 53 were treated by excision and immediate local flap reconstruction. Analysis of results in the latter group encourages the use of this method for virgin, untreated tumors provided basic rules of surgical oncology are followed. Furthermore, it has a definite advantage cosmetically over more conservative methods. If radiotherapy has been the initial treatment, however, immediate reconstruction after excision of a recurrent tumor should be contemplated reluctance.
