Immunohistologic analysis of the cholesteatoma matrix in children.

The immunohistological characteristics of retraction pockets, cholesteatoma matrix and granulomatous tissue were compared in 14 samples from pediatric cholesteatoma. The junction between epidermis and the middle ear mucosa appeared as the most inflammatory area, displaying the characteristics of delayed type hypersensitivity. CD1 + Langerhans cells were observed in all epidermic areas, but expressed class II molecules only in the vicinity of polymorphonuclear infiltrates. Numerous mast cells and IgA producing cells were also observed, suggesting that defenses from the mucosal immune system are summoned and contribute to the pathogenesis of cholesteatoma.

Fixed-drug eruption presenting as an acute paronychia.

A case of fixed-drug eruption due to cefalexine presented with acute paronychia. Twenty days after a further challenge with a single dose of the drug, a biopsy of the affected skin showed intense expression for ICAM-1.

[New concepts of the course of cholesteatoma from the immunohistological study of 96 samples].

The immunohistological characteristics of retraction pockets, cholesteatoma matrix and granulomatous tissue have been compared. Langerhans cells, epidemic folds and sub-epithelial tissue were labelled with specific markers (HLA II, adhesion receptors, KI 67). The junction between epidermis and sub-epithelial tissue of middle ear mucosa appeared as the most inflammatory area, displaying characteristics of delayed type hypersensitivity phenomenon. CD1+ Langerhans cells appear to express class II molecules only in the vicinity of polymorphonuclear infiltrates. Epidemic proliferation seems to able place mainly in the deepest recesses of the epidermis.

Acute Disease

[External ear and abnormality of the urinary tract: a comparative study of 2 series (100 and 150 cases)].

Pinnal anomalies were observed in 46% of 150 children with urinary tract malformation. This ear feature was only recorded in 16 cases among 100 control children. The observation of abnormalities of the auricle (flattened structure of the pinna, deformed shape of the helix) should thus prompt to investigate a possible urinary tract malformation (vesico-renal reflux, mega-ureter). The identification of this disorder should occur as early as possible in order to perform corrective surgery before irreversible kidney damage. Any urinary disorder associated to atypical morphology of the external ear should lead to abdominal echography. Urinary infections, sometimes mistaken for upper respiratory tract infections, will sometimes provide a diagnostic lead, confirmed by cyto-bacteriological examination of urine. These observations further confirm the concomitant development of the external ear and urinary tract during embryogenesis.

Posture in basketball players.

Static and dynamic posturography was recorded in eleven professional national basketball players (complete team). This sport requires balance and skill and is practiced by tall subjects (average height: 197 cm) with a history of 10 to 15 tibio-tarsal sprains. In static tests, with eyes open or closed, the way was similar between controls and players, while the area was increased for the latter. Area increment could be related to the history of ankle trauma reported by these subjects. In toes up dynamic tests, the latency of the first response (short latency response-myotatic reflex) was shorter in players. Data provided by national charts were used to compare posturography performance and player's skill. The best player who also ranks high in the national chart, has distinctive results in static posturography, with almost no difference when the test is performed with closed eyes.

Lingual fibrosarcoma induced by 7, 12-dimethylbenzanthracene in the rat.

A single injection of 5 mg of 7, 12-dimethylbenzanthracene (DMBA) resulted in a high incidence of lingual fibrosarcoma in 67 Wistar rats. No metastases were found. We studied the influence of age and sex on the incidence, latency of tumors, loss of weight and the spontaneous survival of animals. The method described offers an experimental model for the study of cachexia resulting from upper digestive and respiratory tract cancer.

9,10-Dimethyl-1,2-benzanthracene

[Skull vault bone graft in augmentation septorhinoplasty].

We describe our technique of augmentation rhinoplasty with bone grafting. Degloving is used as the surgical access in collapsed noses, and midcolumellar incision allows further buildup of the nasal contour. A V cutting is made on the dorsum to receive the graft. A split skull graft is taken with a drill and an osteotome. The sculpted bone implant is maintained in position with a miniaturized screw. We compare various bone grafts and types of surgical access.

Bone Transplantation

[Use of the Galea flap in ORL].

We use the Galeal pericranial flap with the temporal vessels. Its high vascular content and its low esthetic and functional after effects are very interesting. The dissection between the outside of the flap and the subcutaneous tissue is difficult. The Galeal epicranial flap can cover wounds of orobuccopharynx, otopoiesis and loss of facial substance. We use it for arranging open technic and also for sealing the skull base.

[Value of anthropometric techniques in pediatric otology].

The recurrent and severe infections of the ENT region during childhood are frequently related to cranio-facial malformations or/and deficiency of the immune system. The cranio-facial abnormalities are at risk to be complicated by transmission deafness either primary or secondary through recurrent middle ear infections. In our pediatric out-patient clinic, most of the patients suffering severe recurrent ENT problems show variable malformations: abnormal implantation or shape of the external ear, a microretrognathism, cervical or facial branchial fistulae, high or ogival palate with anomalies of the dental occlusion or a bifid uvula. All these abnormalities share their origins in a pathological development of the first branchial arch. These developmental anomalies may directly lead to deafness (especially due to an abnormal middle car ossicular development since they are derived in part from the first arch). They may also favor secondary pathologies (middle ear otitis, abnormal soft palate). Moreover the development of the immune system is also dependent of a normal function of the endoblastic epithelium of the pharyngeal pouches which is a part of the branchial system. Immune dysfunctions may therefore accentuate the severity of the ENT Infections.

[Elevated levels of plasma 4-ene steroids in a case of congenital deficiency of 3 beta-hydroxysteroid dehydrogenase].

Plasma hormones have been evaluated from birth to six months in a boy with male pseudohermaphroditism due to congenital 3 beta-hydroxysteroid dehydrogenase (3 beta-HSD) deficiency. At birth, plasma 17-hydroxyprogesterone (17-OHP) was slightly elevated while cortisol was within the normal range. Both 17-OHP and testosterone (T) were increased the fifth day. At the age of 2 months, only the baseline level of 17-hydroxypregnenolone (17-OH5P) was slightly elevated and the diagnosis of 3 beta-HSD deficiency was established in view of the excessive response of 17-OH5P and dehydroepiandrosterone (DHA) to Synacthen administration. There was an important increase of T and dihydrotestosterone in response to chorionic gonadotropin (hCG) administration while no significant variation of the other studied steroids was observed. These results suggest that the determination of plasma 17-OH5P seems to be the best tool to diagnose congenital 3 beta-HSD deficiency. The increase of 17-OHP is the result of 17-OH5P conversion in peripheral tissues. Though T response to hCG is rather normal, a partial testicular deficiency of 3 beta-HSD cannot be excluded. The ambiguous genitalia observed in this boy with normal plasma T levels might suggest that T production might have been deficient at the time when urethral-fold fusion occurred.

17-alpha-Hydroxypregnenolone

[Static and dynamic posturography. Application to a population of young athletes].

The authors used a vertical force platform fitted with 4 pressure gauges (Societe Toennies). The static regime provides the statokinesigram, the lateral and sagittal stabilograms and their Fourier transformations. Information is collected with the eyes open and closed, thereby allowing the Romberg quotient to be determined. In the dynamic regime, the platform unexpectedly tilts to a toes-up position (4 degrees at a rate of 50 degrees/s). The activity of the tibialis anterior and soleus muscles is recorded by integrated electromyography. The early soleal response, provoked by the myostatic reflex, is followed by later responses due to the vestibulo-spinal reflex. These various parameters were recorded in young athletes and the obtained tracings are commented on.
