Gestures and words in early development of children with Down syndrome.

This study investigated the development of language and communication in children with Down syndrome (DS). More specifically, the aim was to examine the relations among verbal comprehension, verbal production, and gesture production in the very early stages of development. Forty children (age range: 10-49 months) with DS and 40 children with normal development (age range: 8-17 months) participated in this study. Children with DS came from two Italian health centers. The communicative and linguistic development of children with DS was measured by administering the Italian version of the MacArthur Communicative Development Inventory. The children with DS were severely delayed when compared with normally developing children in reaching the developmental stages. In such children a dissociation emerged between verbal comprehension and production, in favor of comprehension, whereas a synchronous development was found between vocal lexical comprehension and gestural production. The individual differences previously reported in these children are also evident in all domains examined. There were no significant differences between children with DS and typically developing controls matched for lexical comprehension on verbal production. However the two groups differed significantly in gestural development, suggesting a "gesture advantage" in children with DS compared with controls matched for word comprehension. Some possible reasons for this dissociative profile are discussed.

[Bidirectional tachycardia. Physiopathogenetic considerations and therapeutic strategy].

The authors report two cases of bidirectional ventricular tachycardia in patients suffering from digitalis intoxications. Considered the severe prognosis of such arrhythmias, the importance of an early diagnosis for starting an exact therapeutic strategy is stressed. It is important to note the absolute contraindication at the use of cardioactive glucoside. The various pathogenetic hypotheses about this arrhythmias are reviewed.

[Atrial natriuretic factor and pulmonary function].

The aim of this review is to provide a critical and concise discussion of present knowledge on the role of atrial natriuretic factor (ANF) in physiological as well as pathological pulmonary conditions. The lung contributes only to a small extent to the production of circulating ANF; on the other hand, the lung represents the major degrading site of the protein. Plasmatic ANF concentration increment during lung disease may therefore be due to a reduction in ANF plasma removal enzyme rather than to increased ANF production. Lung tissue shows more ANF receptor sites than any other organ. The effect of ANF on bronchial and pulmonary artery muscle lining is particularly evident. In fact ANF administration in asthmatic patients leads to bronchodilation comparable to dilation induced by salbutamol. Furthermore, elevated levels of circulating ANF seem to influence fluid redistribution through alveolar-capillary membrane leading to protein mobilization through the alveolar space. On the contrary, in the cardiomyopathic hamster ANF induces relevant guanylate cyclase activation before the animal has developed hemodynamic changes. Guanylate-cyclase activation may protect the lung through counteracting pulmonary edema formation, as shown by fluid reduction in alveolar spaces following pneumotoxic agents administration. This effect seems independent of natriuretic and hypotensive ANF effects.

[Assessment of the ejection fraction in ischemic cardiopathy with two-dimensional echocardiography. Validity and limits of the method].

In this study the authors compare the Ejection Fraction (EF) as determined by the Single Plane Ellipse Echocardiographic method versus the Ventriculographic EF. The authors emphasize the validity of the two-dimensional echocardiography to assess left ventricular EF in order to improve the clinical and pharmacologic management of the patients with the cardiac ischaemic disease.

[Acute aortic dissection: diagnostic possiblities of mono- and two-dimensional echocardiography].

A case of an acute aortic dissection (AAD) has been reported in a 61-year-old woman, in whom, by the mono and two-dimensional echocardiography (M and 2D E) the authors have detected: 1) The disconnection between the intima and adventitia. 2) A flap of the intima in the aortic cavity. The authors emphasize the usefulness of the M and 2D E because it permits: 1) An early diagnosis of the AAD. 2) A reduction of the lapse between the angiographic examination and the surgical therapy. 3) A better prognosis.
