Renal function impairment caused by intravenous urography. A prospective study.

Renal function was evaluated before and after intravenous urography (IVU) in 124 randomly selected patients. In cases with renal insufficiency (RI) (serum creatinine level, greater than or equal to 2 mg/dL), the incidence of renal impairment was higher (11 of 20 patients, 55%) than in the group without RI (16 of 104 patients, 15%). In the latter group, high blood pressure (BP) was associated with a higher frequency of renal impairment (28.6% vs 10.5%). Advanced age, mild proteinuria, and a single functioning kidney were not risk factors. The IVU preparation contributed to renal function impairment in ten cases, while in the other 17 cases, the iodinated contrast material was the only factor apparently involved. Renal function returned to its previous level in a mean period of 12 days. One patient suffered progressive and irreversible renal failure, and two others had a slight, persistent impairment of renal function. It was concluded that the incidence of renal function impairment is high, but cases are usually mild and reversible. The most important predisposing factors are RI and high BP.

[Cryptococcosis not associated with AIDS. Clinical study of seven patients].

BACKGROUND: The aim of the present was to study the clinical characteristics, the diagnostic methods, response to treatment and the evolution of cryptococcosis not associated with AIDS. METHODS: Seven cases studied in the Hospital Ramon y Cajal of Madrid, Spain over a 19-year period (1980-1998). Diagnosis was determined by LCR culture, blood or other clinical samples. RESULTS: Four patients were males with a mean age of 45.1 years (range: 23-69 years). All except one case presented a predisposing disease and three received corticoids and/or immunosuppressive drugs. Six patients presented meningitis and one pneumonia. The meningitis showed a subacute course with pleocytosis, hypoglycorrhachia and hyperproteinorrhachia in most of the patients. Chinese ink staining, culture and the cryptococcic antigen of LCR were positive in all the cases of meningitis evaluated. All the patients were initially treated with amphotericin and fluorocytosin. This schedule was substituted in two cases by amphotericin lipid formulas because of adverse effects and by fluconazol as an elective choice in one case. The evaluation was favorable in all the patients. One patient presented meningeal and cutaneous recurrence. Three patients died, although none did so during the acute episode or as a consequence of the infection. CONCLUSIONS: Cryptococcosis not associated with AIDS is rare in Sapin. It usually evolves with subacute hypoglucorrhachic meningitis and the conventional diagnostic techniques (Chinese ink staining, cryptococcic antigen and culture) provide adequate profitability. Response to treatment with amphotericin and fluorocytosis is usually favorable, although there are therapeutic alternatives which are adequate for selected cases.

[Serologic diagnosis of Lyme disease. A pending problem].

We analyze the experience in serologic diagnosis of Lyme's borreliosis. From a total of 551 patients studied from 1987 to 1989, we further evaluate 80 cases with erythema chronicum migrans or a clinical diagnosis of Lyme's disease and positive serological tests. The techniques used were IFI, ELISA1 (Whittaker Bioproducts) and ELISA2 (MarDx Diagnostics). Serological tests results were evaluated in relation to clinical data. Five cases were excluded because of no-specific symptoms. There were 20 false-positive results, mainly due to other infections (HIV infection, tuberculosis, Mediterranean spotted fever and syphilis). Fifty-five patients were considered clinically of having Lyme's disease. IFI test was positive in 81.8% of all the 55 cases, ELISA2 in 58.4% of 53 cases tested and ELISA1 in 23% of 43 cases tested. Correlation between IFI and ELISA2 positive test was seen in 45% of cases. Specificity of all tests was higher than 97%. The study shows that sensitivity for all three techniques used was not optimal, and also there are some differences in their results. However, specificity was adequate.
