Elevated serum levels of unsaturated folate binding protein: clinical correlates in a general hospital population.

We measured the capactiy of unsaturated folate binding protein to bind 3H-pteroylglutamic acid (UFBC) in serums from 489 general hospital patients. Of the patients 20% had mild-to-moderate elevations in UFBC (250 to 500 pg/ml), but only 6% had marked elevations (greater than 650 pg/ml), which correlated with metastatic cancer, active granulomatous disease, and especially with liver disease. The poor correlation of serum UFBC with folate status of the patient suggests that in many instances the elevated serum UFBC is neither the consequence nor the cause of folate deficiency. Although the role of folate binding protein remains unknown, it may be primarily an intracellular protein that is released into the serum as an index of activity of liver cells, granulocytes, and perhaps certain cancer cells.

Concurrent primary pneumococcemia, disseminated intravascular coagulation, and sickle cell anemia.

Fulminant pneumococcal infections are rare in teen-agers with sickle cell anemia. A 16-year-old black male with sickle cell anemia was treated as an outpatient for cryptogenic pain crisis, which delayed antibiotic therapy for primary pneumococcal septicemia for seven hours. This patient did not appear ill upon initial presentation but rapidly developed disseminated intravascular coagulation and died.

Clinical correlates of bone marrow plasmacytosis.

In light of the variable clinical expression and bone marrow plasmacytosis of multiple myeloma, we studied prospectively the prevalence and clinical correlates of bone marrow plasmacytosis in 133 anemic medical inpatients. Seventeen patients (13%) had 5% or more, and four patients (3%) had 10% or more marrow plasma cells. Only two patients (with plasmacytosis of 7.4% and 46.2%, respectively) had multiple myeloma. Lung infections (pneumonia, tuberculosis, abscess) accounted for 41% of the cases of plasmacytosis. We found that plasmacytosis correlated with the severity of pneumonia and that active pulmonary tuberculosis was usually associated with notable plasmac. ytosis. Cancer and liver disease were not important causes of plasmacytosis in this study. There was a correlation between plasmacytosis and serum hyperglobulinemia. A correlation between advancing age of the patients and increasing prevalence of plasmacytosis was attributed in part to an increasing prevalence of serious illness with age. However, some elderly patients with apparently benign disorders had unexplained plasmacytosis.

Extranodal lymphoma with an unusual constellation of features.

Hypercalcemia, lytic bone lesions, and hematogenous dissemination formed an unusual constellation of presenting features in a patient with extranodal non-Hodgkin's lymphoma. The patient's clinical course was short, with death resulting nine months after diagnosis, and this combination of features may indicate a poor prognosis.

Glutamate dehydrogenase specific activity and cephalosporin C synthesis in the M8650 series of Cephalosporium acremonium mutants.

Conditions for the accurate measure of glutamic dehydrogenase (GDH) from Cephalosporium acremonium were determined. K(m) values for alpha-ketoglutarate and ammonium ion were 7 and 15 mM, respectively. The half-saturation for reduced nicotinamide adenine dinucleotide phosphate was 5 muM. Reduced nicotinamide adenine dinucleotide did not serve as a cofactor for the enzyme. The specific activity of GDH was measured in six mutants of C. acremonium which varied in their ability to synthesize cephalosporin C. The mutants represented two separately derived lines, A and B. The four mutants in line B were characterized by a derepression of the GDH upon entry into stationary phase. The two mutants in line A were characterized by repressed levels of GDH during the same period. Both lines exhibited high GDH activity early in their fermentations, but activity decreased during the period of active cell growth. Cytochrome c concentrations followed the same pattern as total soluble intracellular protein. Line A mutants were low in cephalosporin C productivity and line B encompassed low, intermediate, and high productivity mutants. The relative frequency of yield improvements in line A and B indicate that the altered regulation pattern for GDH in line B may have removed a nitrogen limitation for cephalosporin C synthesis.
