Hepatitis B surface antigen, hepatocellular carcinoma and cirrhosis in south India--an autopsy study.

This study is aimed at finding the association of hepatocellular carcinoma and cirrhosis with hepatitis B surface antigen in a particular geographical area, Andhra Pradesh State in South India. In total, 206 cases of autopsy livers were studied for the presence of hepatitis B surface antigen by orcein staining. Of the 114 cases of cirrhosis 67.54% were positive for the antigen. There were 13 cases of macronodular, 55 cases of mixed and 46 cases of micronodular cirrhosis. The antigen positivity was 100%, 98.7% and 21.74% respectively. The difference in positivity between micronodular and the other two types of cirrhosis was statistically significant (P less than 0.01). Of the 58 cases of hepatocellular carcinoma, 50 were associated with cirrhosis. In 80% of these cases, hepatitis B surface antigen was demonstrated, whereas 75% of cases of hepatocellular carcinoma not associated with cirrhosis, were positive for hepatitis B surface antigen. The geographical importance of these findings was discussed.

Adenocarcinoma of the bladder in a boy.

The first case of adenocarcinoma of the bladder in a boy is described. Clinically, there were red friable granular areas over an exstrophic bladder. Multiple bladder biopsies revealed adenocarcinoma, which was treated with total cystoprostatectomy, ureterosigmoidostomy and reconstruction of the epispadias with a good result. In view of the early malignant change in an exstrophic bladder it is advantageous to perform multiple bladder biopsies, especially in older children before undertaking surgical treatment. These biopsies would help to detect the early malignant change and determine the plan of treatment.

Distribution of glands in the mucosa of the hard palate and its relation to carcinoma.

Carcinoma of the hard palate in women who practice reverse smoking is very common in Visakhapatnam. This carcinoma usually occurs in the middle of the posterior half of the hard palate and not in the soft palate or in the anterior half of hard palate. The volume of the glandular tissue in the mucosa of the hard palate is maximum to the right or left half of the midline in the posterior half in both fetuses and adults. The localization of the carcinoma of the hard palate in reverse smokers corresponds to the glandular area of the mucosa of the hard palate.

Primary malignant melanoma of the hard palate.

A case of malignant melanoma of the oral cavity is presented. The very high frequency of squamous cell carcinoma of the palate in Visakhapatnam, India, is compared to the rarity of malignant melanoma, even though pigmentation of the oral cavity is quite common in this geographical region.

Serum lipoprotein profile in infectious hepatitis.

There is absence of pre-beta and alpha-lipoproteins in the acute stage of infectious hepatitis. With improvement both clinically and biochemically there is appearance of both lipoproteins, with complete reversal to normal lipoprotein pattern in the end.
