Pilot studies of 2 and 1 course carboplatin as adjuvant for stage I seminoma: should it be tested in a randomized trial against radiotherapy?

PURPOSE: Underpinned by increased confidence in cure of metastatic seminoma by chemotherapy during the past 12 years, three management strategies for Stage I seminoma have been evaluated by six collaborating centers within the Anglian Germ Cell Tumor Group. This paper evaluates the efficacy of surveillance, prophylactic radiotherapy and adjuvant chemotherapy, and discusses these differing management approaches. METHODS AND MATERIALS: Patients were recruited into the study between 1982 and 1992. There was no randomization between treatment groups. Seventy-nine patients received prophylactic radiotherapy (median follow-up = 51 months), 67 patients had surveillance alone (median follow-up = 61 months) and 78 patients were treated with adjuvant single agent platinum (median follow-up = 44 months). Fifty-three of these patients received two courses of platinum (median follow-up = 51 months) and 25 patients received one course (median follow-up = 29 months, range 22-72 months). RESULTS: There were 18 (27%) recurrences on surveillance, five (6%) after radiotherapy, one (1%) after two courses of adjuvant single agent platinum and none after one course of carboplatin. There was one death from testis cancer after radiotherapy and none after adjuvant chemotherapy treatments. Two patients died with drug resistant disease after relapse on surveillance. There was one death from a myocardial infarction after prophylactic radiotherapy and one death from suicide in the surveillance group. A retrospective quality of life questionnaire reviewing the incidence of early and late toxicity revealed no major differences though they suggest that those treated with one course adjuvant carboplatin had somewhat less sickness and an earlier return to work. CONCLUSION: Single agent carboplatin appears well tolerated and is an effective adjuvant treatment for Stage I seminoma. A multicenter randomized trial of the different treatment modalities is required to further evaluate its use.

Synchronous bilateral middle ear carcinoma.

A case of bilateral middle ear carcinoma is presented. The patient was diagnosed as having a squamous carcinoma of the right middle ear 3 months after an identical diagnosis had been made on the left. He had a relatively short history of previous ear symptoms. We believe that these tumours arose synchronously, and discuss possible aetiological factors.

A randomized study of misonidazole and radiotherapy for grade 3 and 4 cerebral astrocytoma.

The results are reported of a small clinical trial carried out to assess the potential value of the hypoxic cell radiosensitizer misonidazole in the radiation treatment of Grade 3 and 4 supratentorial astrocytomas. A total of 55 patients were randomly allocated to one of 3 treatment groups. No significant differences were seen between the median survivals of patients in the 2 control radiation groups and that of the third group in which oral misonidazole at a dose of 3 g/m2 preceded each of 4 weekly radiation doses. Possible reasons why no improvement was seen are discussed in detail.

Monitoring salivary misonidazole in man: a possible alternative to plasma monitoring.

Concentrations of misonidazole and its O-demethylated metabolite Ro 05-9963 in the plasma and saliva of 10 patients with malignant disease have been determined. A good linear correlation was established between plasma and saliva misonidazole concentration, and salivary sampling was found to be suitable for the estimation of a number of pharmacokinetic parameters. Data are also presented for serial tumour cencentrations of misonidazole and Ro 05-9963 in 3 of the 10 patients. Monitoring of salivary misonidazole concentration appears to be a useful alternative to plasma monitoring, particularly for those patients in whom plasma sampling is unsuitable or impossible.

Clinical studies with misonidazole.

The preliminary details of a randomized clinical trial of misonidazole for the radiotherapy of Grades 3 and 4 cerebral astrocytoma are described. Plasma concentrations of misonidazole and its O-demethylated metabolite, determined by high-pressure liquid chromatography analysis, are reported for 8 patients with astrocytoma, 1 with carcinoma of the bronchus and 1 with carcinoma of the breast. In the latter case, tumour concentrations are also presented.

Scheduled and unscheduled combination chemotherapy in the treatment of squamous cell lung cancer (cyclophosphamide, adriamycin, vincristine and bleomycin, with and without cis-platinum).

Fifty four patients with squamous cell lung cancer, treated with Cyclophosphamide (750 mg/m2), Adriamycin (50 mg/m2), Vincristine (1.5 mg/m2) and Bleomycin (30 mg) (CAVB) in a 2 day scheduled regime, showed an overall median survival time (MST) of 32 weeks (28 weeks for non-responders and 46.5 weeks for responders--a non-significant difference). The response rate was 24%, the median duration of response being 23 weeks. Bone marrow toxicity was not a major problem. A further 21 patients received the same drugs but in a single bolus (unscheduled regime). Comparison of their results with the 25 patients randomised to the scheduled regime showed no significant difference in MST or toxicity. The addition of Cis-platinum (50 mg/m2) to the unscheduled CAVB regime (CAVBP) in a further 22 patients did not result in an improvement either in survival or response, but added to the toxicity.
