Incidence of digestive cancers and occupational exposure to asbestos.

While the role of exposure to asbestos in the development of several cancers such as mesotheliomas and bronchopulmonary cancers is now well established, the possible relationship between digestive cancers, other than peritoneal mesotheliomas, and occupational exposure to asbestos is still controversial. The great majority of the studies are based on mortality data. The aim of the study was to analyse the relationship between digestive cancer incidence and occupational exposure to asbestos in a population of subjects for whom precise occupational exposure data and precise incidence data were available. The population consisted of salaried and retired workers from a company using asbestos to manufacture fireproof textiles and friction materials. There were 1454 men (79.9%) and 366 women (20.1%). A cumulative exposure index and a mean exposure concentration in fibres/ml for each subject were calculated with the aid of an in-house job-exposure matrix. The number of cases of digestive cancer observed was compared with the expected and Standardized Incidence Ratio (SIR) was estimated. Precise occupational exposure data allowed us to study the dose-response relationship between asbestos exposure and risk of digestive cancer using Cox model. Fifty-six digestive cancers occurred in the study population over the 18-year follow-up period for 48.4 expected (SIR = 1.16 [0.87-1.50]). Comparing with incidence in the county, SIR was not significant for any of the digestive localization, but for peritoneum. However, even after taking into account the potential confounders via the Cox model, there was a significant dose-response relationship between the occurrence of digestive cancers and the mean exposure concentration, even after exclusion of peritoneum cancers. Our study provides initial evidence suggesting a relationship between occupational exposure to asbestos and the risk of digestive cancer: first, it is a study of incidence although the risk evidenced is not significant; secondly, a dose-effect relationship is demonstrated in the whole population. However, these preliminary results require confirmation by more powerful studies focusing on larger series.

Asbestos bodies in the sputum of asbestos workers: correlation with occupational exposure.

A cross-sectional medical survey including collection of three consecutive sputum samples was carried out among 270 retired workers of a textile and friction materials factory, in order to investigate the relationship between asbestos body identification and asbestos exposure. The individual cumulative asbestos exposure, determined by means of a plant-specific job-exposure matrix based on asbestos air measurements in the workshops, proved to be heavy with a mean cumulative exposure of 217 fibres x mL(-1) x yr. Macrophages and asbestos bodies were identified in sputum samples by light microscopy. The lung origin of the sputum, suggested by the presence of macrophages and/or asbestos bodies, was confirmed in 82.6% of subjects, and 53% of these samples were positive for asbestos bodies. The prevalence of asbestos bodies was not related to sex, smoking status or latency. Conversely, multivariate analysis showed a positive relationship with cumulative exposure, duration and intensity of exposure to asbestos, as well as age and time since retirement. These findings suggest that sputum analysis for asbestos bodies may remain a relevant and noninvasive marker of heavy occupational exposure to asbestos, even years after retirement. Owing to the new perspectives in lung cancer screening, it might contribute to the identification of high-risk subjects.

Pseudomembranous aspergillus bronchitis in a double-lung transplanted patient: unusual radiographic and CT features.

Pseudomembranous aspergillus bronchitis is considered as an early form of invasive pulmonary aspergillosis, a well-known airway infection in immunocompromised patients. Radiologic features concerning invasive aspergillosis of the airways have been reported. However, we describe here an unusual feature of invasive aspergillus bronchitis, never reported to date, observed in a double-lung transplanted patient. Chest radiograph and CT revealed significant peribronchial thickening without any parenchymal involvement.

[Rolled atelectasis and asbestosis. Apropos of 26 patients].

Round atelectasis (AE) is a benign form of respiratory problem which develops due to fixing of the visceral pleura. This lesion for which the principal cause is exposure to asbestos may pose problems of differential diagnosis with bronchopulmonary cancer. In a cohort of 286 patients suffering from benign asbestos related pleural disease the diagnosis of round atelectasis was made on computerized tomography in 26 patients (31 AE) on the following criteria: rounded opacities of less than 7 cm in diameter situated at the periphery of the lung in contact with a thickened pleura with an acute angle linking the pleura and the opacity, a reduction of lung volume on the side of the atelectasis and the presence of a "comet tail sign". These patients were investigated to specify the circumstances of the occurrence, including their symptomatology, the changes in lung function, the topography of the round atelectasis and the associated radiological signs. Intense exposure to asbestos either continuous or discontinuous was found in 19 patients; 20 patients presented with some respiratory symptoms (dyspnoea 15/26, cough 11/26 and chest pain 9/26) but the reduction in lung function was moderate (7 had restrictive ventilatory trouble, 4 obstructive problems and a mixed problem in 1). The preferred localisation for round atelectasis was in the inferior lobes in the posterior basal lateral vertebral area (26/31) which may explain their being frequently missed on the standard radiograph (only one case of round atelectasis was visible on the straight chest radiograph in our study). An understanding of the pathology and the computered tomographic characteristics are now well defined and should enable an unnecessary diagnostic thoracotomy to be avoided which besides has no justification from the functional point of view.

Primary tumour staging of gastric and colorectal cancer.

Almost 15 years after its introduction, endosonography or endoscopic ultrasonography is an important technique in a wide range of gastrointestinal diseases. Other imaging techniques are CT, MRI and barium examination. There is a general consensus that the most important prognostic factor in gastric and colorectal carcinoma is the presence or absence of lymph node invasion, but malignant fixation of tumour through direct invasion of adjacent tissues also appears to be very important. Non-invasive preoperative assessment of tumour stage based on one or a combination of the above imaging modalities should allow appropriate treatment to be planned in each case.

Benign fibrous mesothelioma of the pleura: MR study and pathologic correlation.

Benign fibrous mesothelioma of the pleura is a rare tumor of mesodermal origin. We describe the MR findings in three pathologically proven cases. All three tumors were imaged by MR as well-circumscribed lesions with smooth margins in contact with the pleura, but without chest wall invasion. Their low signal intensity on T1- and T2-weighted sequences reflect their fibrous nature. In one case a pedicle connecting the tumor to the chest wall was visualized on a sagittal MR scan. In two cases gadolinium-enhanced T1-weighted gradient echo sequences revealed intense contrast uptake by the tumor correlated with the intratumoral hypervascularization noted by histologic examination. Although the number of cases presented is small, MR seems to be the most accurate imaging modality in the assessment of the diagnosis.

Sonographic diagnosis of solitary solid liver nodules in cancer patients.

The purpose of this study was to analyse the sonographic patterns of solitary solid liver lesions in cancer patients and to evaluate the diagnostic accuracy of ultrasound alone and in combination with other techniques (liver function tests, histology). A total of 422 solitary solid liver lesions (SSLL) were diagnosed by ultrasound in cancer patients; 197 lesions were benign and 225 malignant. The predominant aetiology for hypoechoic SSLL (128 cases) was metastasis (112 cases), whereas most hyperechoic SSLL (265 cases) were haemangiomas (155 cases) rather than a metastasis (86 cases). The 29 isoechoic SSLL included 27 metastases and 2 benign lesions. A halo was found to be highly predictive of malignancy (97%-100%). The positive predictive value for malignancy of an SSLL was very high when the results of liver function tests were abnormal (97%-100%). In our experience, histological proof is unnecessary in the majority of liver lesions in cancer patients.

Indications for hepatic ultrasonography in breast cancer staging and follow-up.

Retrospective analysis of the medical records of 6649 breast cancer patients seen over an 11-year period found 438 patients (6.6%) with liver metastases (LM) and 432 patients (6.5%) with benign liver lesions (BLL). Liver ultrasonography (LUS) and liver function tests had been performed for all patients. LM were the first manifestation of metastatic spread in 20.1% of the 438 patients; median survival was related to the presence (6.7 mo.) or absence (12.2 mo.) of extrahepatic metastases (EHM). Liver function tests were normal in 20.5% of the patients in whom LM were first diagnosed by LUS. Most LM were hypoechoic (70.9%) BLL corresponded to cysts, hemangiomas, calcifications, and focal fatty infiltration. LUS appears indicated for (i) pretherapy disease staging, and in particular for detection of BLL, and (ii) follow-up of patients without EHM for early diagnosis of LM.

Tolerability and utility of mangafodipir trisodium injection (MnDPDP) at the dose of 5 mumol/kg body weight in detecting focal liver tumors: results of a phase III trial using an infusion technique.

PURPOSE: To evaluate the tolerability of mangafodipir trisodium (MnDPDP) and its utility for enhancing the ability of magnetic resonance (MR) imaging to detect focal hepatic lesions compared with non-enhanced MR and contrast-enhanced computed tomography (CT). MATERIALS AND METHODS: 119 patients with focal hepatic lesions were examined by MR and by contrast-enhanced CT. MR was performed before and after the infusion of 5 mumol/kg MnDPDP, at a concentration of 10 mumol/ml. Histologic confirmation was obtained in 79 patients. RESULTS: There were no severe adverse events. Five patients reported mild adverse events related to the infusion. MnDPDP-enhanced SE T1 and GE T1 sequences revealed more focal lesions than the same sequences before contrast infusion in, respectively 22.6 and 36.1% of the cases, and fewer focal lesions in, respectively 5.9 and 1.7% of the cases. Contrast-enhanced MR demonstrated more focal lesions than the SE T2 sequence in 29.4% of cases and fewer lesions in 5.9% of cases. MnDPDP-enhanced MR revealed more nodules than CT in 31.1% of cases and fewer nodules in 13.4% of cases. The additional information provided by MnDPDP enhancement led to modification of management for 12 patients (10.1%). CONCLUSION: MnDPDP is a well-tolerated contrast agent allowing better MR detection of focal hepatic lesions than non-enhanced MR or contrast-enhanced CT.

[Compression of the subscapular nerve in the spino-glenoid tract by a synovial cyst].

We observed a case of synovial cyst in the shoulder joint leading to spino-glenoid symptomatology in a weight-lifter. Explorations included standard radiography, echography and magnetic resonance imaging. Differentiation between a true cyst and a pseudo-cyst was basedose presence of a synovial membrane. Mechanical contraints appear to be the most frequently suspected aetiology of spinoglenoid cysts. Echography cannot always confirm the liquid nature of the formation due to the close bone. Magnetic resonance imaging gives good tissue characterization and good topographic analysis. Gadolinium injection appears to be important for differential diagnosis with neurogenic tumours.

[Pelvic gynecological cancers. Is hepatic echography useful?].

An examination of liver echographies performed over an 11 year period in 2,335 female patients with pelvic gynaecology cancers revealed that the incidence of benign lesions (96 cases) was greater than the number of metastases discovered (79 cases). Regular hepatic echography in cancers of the uterus leads to a median survival of 4 months. Hepatic echography can be useful for the detection of benign lesions in cases of cancer of the uterus and for the detection of benign or malignant lesions in cases of cancer of the ovary. Routine post-therapeutic hepatic echography is only useful in cancer of the cervix. For cancer of the ovary, abdomino-pelvic computed tomography should be performed; liver tests are sufficient in cases of cancer of the endometrium.

[Imaging of anaplastic carcinoma of the thyroid gland].

Sixteen cases of anaplasic thyroid carcinoma have been reviewed. In 13 cases out of 16, a diffuse invasion of one or both thyroid lobes and of the isthm associated with calcifications (50%) and necrosis (62%) was present. In 6 cases out of 8, CT Scan with contrast media injection has detected an extension into surrounding tissues. Twelve of the 16 patients presented cervical lymph nodes on the ultrasound examination and 4 of 8 patients presented mediastinal lymph nodes on the CT Scan. In 3 cases out of 16, the anaplasic carcinoma of the thyroid presented as a localized, hypoechoic nodule associated with a cervical lymph node in 1 case.

[Imaging of non-functional retroperitoneal paragangliomas. Apropos of a case].

The non functional extraperitoneal paragangliomas are rare tumors (less than 50 cases reported). We report a case with ultrasound and CT scan imaging. The tumor is usually large with important necrotic areas leading to a cystic aspect. The malignant tumors more frequent than the benign tumors present with a regional extension and delayed metastasis.

[Pleural involvement and functional effect of asbestosis in 380 exposed patients].

A large number of pleural abnormalities was detected by radiological and functional monitoring of 380 persons (mean age: 47 years) working in an asbestos processing factory. There were 127 cases of parietal pleura thickening (after 22 1/2 years' exposure), 11 cases of pleural calcifications (after 26 years' exposure) and 29 cases of obliteration of the costophrenic angle. Fourteen images of pulmonary asbestosis were encountered, 7 of which were associated with pleural effusion; they occurred at a mean age of 50 years, after 22 years' exposure. Pleural lesions had a restrictive influence on respiratory function in 24% of the cases and were sometimes painful. Small airway obstruction was detected by flow-volume loops in 9% non-smokers with pleural thickening, which suggests that intra-radiological lung lesions exist in asbestosis.
