[Localized intestinal lymphangiectases presenting as peritonitis].

The authors report a case of localised intestinal lymphangiectases in a segment of jejunum with the unusual presentation of an acute perforation. The anatomical findings did not provide a satisfactory aetiopathogenic explanation, although the most likely hypothesis is a past history of abdominal irradiation. The mechanism of the perforation also remains unclear and the presence of microscopic lesions in the wall of the remaining small intestine raises the problem of the long-term prognosis.

[A case of barium-peritoneum with neurological involvement. Importance of barium determination in biological fluids].

Barium in the peritoneum following a barium enema led to a severe and irreversible chronic barium poisoning, although from the abdominal and gastro-intestinal point of view, there were virtually no sequelae. Barium assay on the cerebro-spinal fluid of this patient revealed excessively high levels, which provides the laboratory evidence of the direct action of barium on the central nervous system. Barium assays on biological fluids after barium-peritoneum could be used to assess the degree of barium poisoning and may have prognostic value.

[Carcinoma of the pancreas with amyloid stroma and calcitonin secretion : ultrastructural study (author's transl)].

The authors report a case of anaplastic carcinoma with amyloid stroma, accompanied by the secretion of calcitonin, detected in a cervical lymph node and initially interpreted as being a metastasis from a medullary carcinoma of the thyroid. The actual pancreatic origin of the malignant tumour was proved only during the advanced stages. Ultrastructural study confirmed the endocrine nature of the tumour (neurosecretory granules). Despite its exceptional character, the existence of such a tumour is not surprising, if one accepts the concept of a diffuse endocrine system.

[Gastro-omental cystic lymphangiomas. General review and report of two cases (author's transl)].

Omental and gastric lymphangioma : a general review of cases and two personal cases. Lesser omentum tumors are rare. They exceptionnally arise from the stomach. Pathologic fractures : there are three types of lymphangioma simplex, cavernous and combined cystic ones. Microscopic examination shows that cysts are lined with endothelium and the wall is infiltrated by lymphocytes. Lymphangioma frequently occur in childhood (stomacal tumors excepted). Symptoms are those observed either in abdominal tumors, or in ascitis or in acute abdominal disease. The diagnosis is not established per-operatively. Surgical treatment consists in resection of the cyst or in simple drainage. Post-operative prognosis is excellent.

[Electromyographic analysis of the motility of the right and transverse colon in 9 colostomized patients].

The aim of this work was to study the electric pattern of proximal colonic motility in patients with permanent colostomy but without past history of an irritable bowel syndrome. Nine patients (55 to 70 years-old) were investigated using a colonoscope with 5 annular electrodes. An electromyogram was recorded during 4 h. In 5 patients, the results observed before and after a standard test meal were compared. The two usual types of electric activity (i. e. Long Spike Bursts (LSB) and Short Spike Bursts (SSB)) were recorded in the right and transverse colon. The duration of LSB and SSB activities were 8.3 +/- 5.5 and 3.7 +/- 3.5 p. 100 respectively (m +/- SD). In 6 patients, the right and transverse colons were recorded simultaneously; in these patients there was a significant difference in LSB activity between the ascending colon and the transverse colon (10.6 +/- 3.7 and 5.6 +/- 4.3 p. 100 respectively; p less than 0.01). After a meal, the right colonic LSB activity increased (greater than 30 p. 100 of the basal level) in 4 out of 5 patients whereas the transverse colonic LSB activity increased only in one patient. The most striking finding of this study was that colonic hypomotility was of a propulsive and tonic nature. These results could explain the nearly continuous flow of faeces commonly observed in patients with colostomy.

[Duodenocaval fistula. Apropos of a case. Clinical case].

Among entero-vascular fistulae, the duodenocaval type is exceptional. A case arising in a patient operated 10 months ago from a right kidney cancer, is reported. The clinical feature associated a septicemia to an intestinal hemorrhage. The diagnosis of entero-vascular fistula was performed at the operating time. In the world literature only 8 cases of duodeno-caval fistulae have been previously reported. The injury of duodenocaval area is the most frequent etiology of the communication causing moderate digestive hemorrhage and understood septicemia. The imaging explorations may suspect the diagnosis that is confirmed baparotomy. The treatment is always a surgical one: closure of digestive fistula by intestinal patch and suture of vascular defect. The prognosis depends on experience and skill of the surgeon and remains serious.
